RESUMEN

A prototype of an integrated closed system for fish-plankton aquaculture was developed in Iquitos (Peruvian Amazonia) in order to cultivate the Tiger Catfish, Pseudoplatystoma punctifer (Castelnau, 1855). This freshwater recirculating system consisted of two linked sewage tanks with an intensive rearing unit (a cage) for P. punctifer placed in the first, and with a fish-plankton trophic chain replacing the filters commonly used in clear water closed systems. Detritivorous and zooplanktivorous fishes (Loricariidae and Cichlidae), maintained without external feeding in the sewage volume, mineralized organic matter and permitted the stabilization of the phytoplankton biomass. Water exchange and organic waste discharge were not necessary. In this paper we describe the processes undertaken to equilibrate this ecosystem: first the elimination of an un-adapted spiny alga, Golenkinia sp., whose proliferation was favored by the presence of a small rotifer, Trichocerca sp., and second the control of this rotifer proliferation via the introduction of two cichlid species, Acaronia nassa Heckel, 1840 and Satanoperca jurupari Heckel, 1840, in the sewage part. This favored some development of the green algae Nannochloris sp. and Chlorella sp. At that time we took the opportunity to begin a 3-month rearing test of P. punctifer. The mean specific growth rate and feed conversion ratio (FCR) of P. punctifer were 1.43 and 1.27, respectively, and the global FCR, including fish in the sewage part, was 1.08. This system has proven to be suitable for growing P. punctifer juveniles out to adult, and provides several practical advantages compared with traditional recirculating clear water systems, which use a combination of mechanical and biological filters and require periodic waste removal, leading to water and organic matter losses.

PURPOSE: In congenital heart disease, the exact determination of the right ventricular function is of high importance for therapeutic and especially surgical planning. The aim of this study was to correlate the parameters of the right ventricular function in MRI and 3D echocardiography to determine the agreement of both modalities. MATERIALS AND METHODS: In 18 patients suffering from congenital heart disease, 3D echocardiography was performed using a Philips Sonos 7500 system. In MRI short axis slices with a 4-mm distance were acquired using an SSFP sequence on a Siemens Sonata or Symphony System. Volumetry for both modalities was performed on an external workstation (Tomtec) using the EchoView software. RESULTS: Enddiastolic and endsystolic volumes showed a highly significant correlation with coefficients of 0.996 and 0.990, respectively. In echocardiography there was a systematic slight underestimation of enddiastolic volumes and overestimation of endsystolic volumes. The Wilcoxon test did not show significant differences between the volumes and ejection fractions assessed by both modalities. CONCLUSION: There is an excellent correlation in the quantification of right ventricular volumes in MRI and 3D echocardiography, which allows a comparison of acquired volumes in clinical follow-up.

We report about our initial experience for the in 2005 modified Helex (Gore) device for closure of atrial septal defects (ASD) and persistent foramen ovale (PFO). Major changes were made at the delivery system for simplifying the Helex implantation procedure. We treated 11 patients, 8 children and 3 adults, with ages between 3 and 62 years. In 10 patients the diagnosis was a relevant ASD with volume overload of the right heart (Left to right shunts between 30 and 50%). One adult (age 58 years) have had a small left to right shunt with a PFO-like defect and the history of 2 neurologic embolic events. In 3 patients we found 2 defects. In all patients a Helex occluder was implanted successfully. The mean fluoroscopy time was 8,4 minutes. The immediate occlusion rate after 24 hours was 91%. In all cases there was a very good adaptation of the device to the anatomical structures. In this small series, the Helex occluder appears to offer a reliable system of occlusion for small and moderate ASDs and for PFO with minimal risk of major complications.

RESUMEN

Evaluating complex cardiac defects in small children preoperatively requires multiple diagnostic procedures including echocardiography, and also invasive methods such as cardiac catheterisation, computer-tomography and magnetic resonance imaging. We assessed the complex anatomy of the atrioventricular valves in atrioventricular septal defect using bedside real-time three-dimensional echocardiography and comparing these results to the anatomic findings at the time of operative intervention.

The purpose of this study was to establish normal left ventricular acoustic quantification (AQ) reference values for children from infancy to adolescence and to compare AQ-derived parameters with traditional M-mode and Doppler indices. We studied 150 normal, healthy children aged 2 days to 14 1/2 years. Left ventricular parasternal end diastolic area and left ventricular end diastolic volume calculated by AQ were linearly related to the body surface area to the 1.1 and 1.4 powers. AQ parasternal peak filling rate and atrial fractional area change in the neonatal period were 140% and 142% of the adolescent's values, respectively, and decreased to 110% and 112% by 36 months of age. The duration of the rapid filling phase and the rapid filling contribution, as identified by AQ, was shorter or lower than that measured by the Doppler method. Interobserver variability of AQ parameters ranged from 8.1% for the ejection fraction to 18.2% for the peak filling rate. Manual biplane determinations of volumes were slightly higher than AQ calculations with highly significant correlations (p <0.001). Our data permit the determination of normal ranges of AQ parameters in relation to body surface area or age.

The alpha-Gal trisaccharide Gal(alpha)(1-->3)Galbeta(1-->4)GlcNAc 11 was synthesized on a homogeneously soluble polymeric support (polyethylene glycol, PEG) by use of a multi-enzyme system consisting of beta-1,4-galactosyltransferase (EC 2.4.1.38), alpha-1,3-galactosyltransferase (EC 2.4.1.151), sucrose synthase (EC 2.4.1.13) and UDP-glucose-4-epimerase (EC 5.1.3.2). In addition workup was simplified by use of dia-ultrafiltration. Thus the advantages of classic chemistry/enzymology and solid-phase synthesis could be united in one. Subsequent hydrogenolytic cleavage afforded the free alpha-Gal trisaccharide.

BACKGROUND AND STUDY AIMS: Interpleural blockade with bupivacaine has been used in operations and for the treatment of pain with promising results. Endoscopy may be followed by serious complications associated with the use of intravenous drugs. Drug-induced hypoxemia has an important role in the pathophysiology. PATIENTS AND METHODS: A randomized study to assess whether interpleural bupivacaine blockade compared with a standard procedure reduced the risk of hypoxemia and the need of medication in patients undergoing endoscopic retrograde cholangiopancreatography (ERCP). Other variables studied were its acceptability to patients and morbidity. RESULTS: Ninety patients were included. Interpleural blockade (n = 43) was followed by minor complications. The blockade itself induced mild hypoxemia in four patients (9%). Overall, eight patients (27%) developed hypoxemia. There were correlations between age, drugs given and hypoxemia in the total series and in the two study groups individually. During ERCP eight (19%) in the blockade group and 16 (34%) in the standard procedure group developed hypoxemia. During the recovery period the opposite pattern was observed: (26%) compared with four (9%) (p = 0.05). Drug requirements did not differ. Procedure-related discomfort did not differ. More patients in the blockade group would prefer another sedative procedure. CONCLUSIONS: Interpleural bupivacaine blockade did not contribute to patients' comfort or safety during ERCP.

Determination of the transcriptional activity of the beta-adrenoceptor (beta-AR) gene in human mononuclear leucocytes (MNL) may provide important information concerning variations in receptor expression in clinical studies. We used a new method with highly fluorochrome-labeled gene probes for in situ hybridization which allows a rapid detection of mRNA at the cellular level. MNL from venous blood of healthy volunteers were permeabilized, fixed and attached to adhesive glass slides. After incubation with fluorochrome-labeled cloned hamster beta AR-DNA probes the amount of DNA-mRNA hybrids formed in the cytoplasm of individual cells was analyzed by microfluorimetry of specifically bound fluorochrome. In nine of ten normal subjects studied so far a fraction of beta AR-mRNA positive MNL could be detected. The proportion of positive cells as well as their signal intensity revealed a high interindividual variance.

Fifty patients undergoing endoscopy (laryngoscopy, bronchoscopy, mediastinoscopy) were anaesthetised in a double-blind prospective trial using total intravenous anaesthesia. Half of the patients were anaesthetised with an infusion of a solution of 250 mg ketamine and 12.5 mg midazolam. The other patients received an infusion of a solution of 250 mg ketamine and 20 mg diazepam. In addition, both groups were given increments of 50-100 micrograms of fentanyl. The immediate awakening time (t1) was not significantly different between groups, but the patients who had received midazolam-ketamine, had a significantly shorter time to more complete recovery (t2), a significantly lower frequency of emergence reactions and were more satisfied with the anaesthetic than the patients who had received diazepam-ketamine. There was no difference between groups with respect to intraoperative heart rate and blood pressure. No awareness during anaesthesia was reported.

Atropine and glycopyrrolate given intravenously before the induction of a ketamine anaesthesia to diminish salivary secretion were compared for their effect on psychotomimetic side-effects, awakening time and heart rate. Though atropine is a tertiary amine that crosses the blood-brain barrier, which glycopyrrolate as a quaternary ammonium compound does not, it did not increase the incidence of psychotomimetic side-effects nor did it significantly prolong the awakening time after ketamine anaesthesia. During intubation the increase in heart rate was significantly higher following atropine than following glycopyrrolate.
