RESUMEN

OBJECTIVE: Amnioinfusion was used in pregnancy complicated by severe oligohydramnios to enable a more comprehensive ultrasonographic evaluation of the pregnancy. STUDY DESIGN: Thirteen patients were referred to the Division of Obstetrical and Gynecological Ultrasound at the University of Miami because of severe oligohydramnios in which a definitive diagnosis could not be made. Nine patients were in the second trimester and four were in the early third trimester. All patients underwent amnioinfusion with warm physiologic saline solution and instillation of indigo carmine. Genetic amniocentesis was performed in 11 patients. RESULTS: After amnioinfusion a satisfactory comprehensive ultrasonographic examination could be performed on all patients. In most cases we were able to achieve a definitive diagnosis. CONCLUSION: Amnioinfusion with the addition of indigo carmine was a satisfactory technique to enable a more comprehensive ultrasonographic evaluation of pregnancy complicated by severe oligohydramnios.

RESUMEN

We present a case of the Goldston syndrome--that is, cystic renal dysplasia and the Dandy-Walker malformation. The condition was diagnosed by ultrasound in a 635 g fetus in the seventeenth week of gestation. Ultrasound studies showed the fetal head to be somewhat enlarged with slight dilatation of the lateral ventricles and marked dilatation of the fourth ventricle. The kidneys were symmetrically enlarged and multicystic. Autopsy revealed evidence of the oligohydramnios syndrome. The kidneys were typical of the Goldston syndrome as were the microscopic lesions in the liver. To our knowledge this is the first reported case in which this diagnosis was made during intrauterine life.

RESUMEN

A group of pregnant women received video and verbal feedback during three ultrasound examinations. This group was compared with a no-feedback group on measures of pregnancy anxiety, fetal activity, and neonatal outcome. The feedback appeared to reduce pregnancy anxiety and fetal activity, particularly for the primiparous women. These women also experienced fewer obstetric complications and gave birth to neonates who were greater weight, more appropriate weight-for-length, less active and irritable, and showed better performance on the Brazelton neonatal behavior assessment.

RESUMEN

A linear relationship between growth of fetal femur length (FL) and biparietal diameter (BPD) after 22 weeks' gestation is described. The normal ratio of femur length to BPD (FL/BPD ratio) was found to be 79 +/- 8%. Effective uses of the FL/BPD ratio include its use as a quality control check on femur length and BPD measurements and its use to diagnose short-limbed dwarfism, hydrocephalus, and microcephaly. Pregnancy dating by means of BPD can now be cross checked by use of femur measurements and new observations on normal and abnormal fetal growth will now be possible.
