RESUMEN

We study energetics of a Josephson tunnel junction connecting a superconducting loop pierced by an external magnetic flux (an rf SQUID) and coupled to two independent thermal reservoirs of different temperature. In the framework of the theory of quantum dissipative systems, we analyze energy currents in stationary states. The stationary energy flow can be periodically modulated by the external magnetic flux exemplifying the rf SQUID as a quantum heat interferometer. We also consider the transient regime and identify three distinct regimes: monotonic decay, damped oscillations and pulse-type behavior of energy currents. The first two regimes can be controlled by the external magnetic flux while the last regime is robust against its variation.

BACKGROUND: Otolaryngologists caring for patients with hearing and balance disorders are also responsible for advising patients about their increased risk of falling and informing them of fall prevention measures. This review will give a brief overview of appropriate programs. METHODS: This systematic review is based on a selective literature search. RESULTS: Intrinsic and extrinsic fall risk factors can be distinguished. The former include not only hearing and balance disorders, but also increasing age, nocturia, dementia, limited mobility and poor nutritional status. Extrinsic factors include, for example, unfixed carpet edges, poor lighting and poor footwear. Fall prevention can be achieved through appropriate counselling about risk factors and fall prevention courses. DISCUSSION: The frequency of falls--with potentially very adverse consequences--increases continuously beyond the age of 60 years. Furthermore, the risk of falling is significantly increased in patients with hearing and balance disorders. Otolaryngologists caring for this patient group should inform them about their fall risk and advise appropriate countermeasures during counselling. A basal knowledge of fall prevention measures is therefore helpful.

RESUMEN

We study superconducting and non-superconducting nanorings and look for non-classical features of magnetic flux passing through nanorings. We show that the magnetic flux can exhibit purely quantum properties in some peculiar states with quadrature squeezing. We identify a subset of Gazeau-Klauder states in which the magnetic flux can be squeezed and, within tailored parameter regimes, quantum fluctuations of the magnetic flux can be maximally reduced.

BACKGROUND: The remarkable quality of modern hearing aids enables most hearing impaired individuals to take part in verbal communication without great effort. As a result, auditory training seems to have fallen into oblivion, even though its use is certainly still justified today. This article aims to review the development of these diverse therapy methods. METHODS: For this systematic review, a selective literature research in PubMed using the keywords "auditory training" = 211 hits and "auditory rehabilitation" = 166 hits has been carried out. In addition, handbooks and monographs were taken into consideration. RESULTS: For around 2,000 years, people have tried to restore hearing by acoustic stimulation. Initially it was believed that hearing could be "woken up" by means of acoustic stimulation. From the 19th Century onwards, auditory training was meant to support residual hearing and help understanding in verbal communication. Towards the end of the 20th Century, systematic approaches that were intended to enhance cognitive skills, such as concentration and attentiveness by acoustic stimulation, were developed. DISCUSSION: The wish to integrate deaf individuals into the auditory verbal oriented society, the tremendous number of soldiers suffering from noise-induced hearing loss after World War II, modern hearing aids and cochlear implants, and finally availability of personal computers has fostered the development of auditory training methods. Further research has to investigate whether auditory training combined with drug therapy or other stimulation methods are beneficial.

BACKGROUND: Chronic hoarseness, globus sensation or mucous obstruction are among a wide range of symptoms possibly related to laryngopharyngeal reflux (LPR). METHOD AND RESULTS: A selective literature research was carried out using PubMed, in which original articles and reviews in German and English were included. Altogether, 2541 articles were found. Despite the large number of publications, there are still open questions regarding aspects of LPR. DISCUSSION: It can be assumed that refluxate reaching the larynx can damage it. Due to the fact that the symptoms possibly related to LPR can, however, be linked to other causes, a careful consideration of the patient's medical history is of the utmost importance, followed by mandatory laryngoscopy. Diagnostic procedures, such as the trial administration of proton pump inhibitors (PPI) or long-term pH measurement should be used selectively. Depending on the individual symptoms, breathing or voice therapy and/or PPI therapy might be useful. Lifestyle changes including dietary measures should also be contemplated.

RESUMEN

Children need to acquire approximately 50 words within their first 24 months. This enables them to start combining words to make short sentences, which is also the start of learning rules governing grammar. When children start to construct sentences, the mean length of their utterances correlates with their implicit grammar knowledge. Therefore, the mean length of utterance estimation can be used to assess and monitor the progress of grammar acquisition.

Sensorineural hearing loss in early childhood may severely interfere with normal speech and language development. This by itself requires pursuit of an effective therapeutic procedure. Here we report the results of therapy with a rheological agent and procaine-HCl with or without the addition of steroids. Immunological testing was also studied for possible circulating antibodies in sera of affected children. Retrospective analysis of therapeutic results showed that the therapeutic procedure employed may be beneficial, since hearing improved considerably in some cases. Since an autoimmune-related sensorineural hearing loss has been postulated for some years, results presented here could be used as an argument for the presence of antibody-related inner ear disorders in early childhood. These findings suggest that circulating antibodies in children suffering from sensorineural hearing losses produce an inflammatory process involving inner ear structures. Circulating antibodies may also indicate a secondary immune reaction. However, sensory hearing loss occurring in early childhood may be steroid sensitive.

A complaint about a malfunctioning hearing aid (HA) usually will require thorough testing of all aspects of hearing aid functions by the hearing aid dispenser. However, ENT physicians can also detect defects easily by following the simple screening procedure described here. Before testing the HA function, the following questions should be answered: Are the body of the hearing aid and ear mold intact? Is the ear canal free of irritation and is the acoustic isolation of the ear canal by the HA sufficient? A rough estimate of HA function can be accomplished by covering the HA with the hand. If the HA is switched on, a feedback, high-pitched sound should be produced. HA amplification can be assessed by using a stethoclip. After the examination is completed, all switches should be put back into their previous positions. Although these basic procedures are not equivalent to diligent and comprehensive acoustical testing, they are still useful for the otologist to detect major faults in the HA before referring the patient to the hearing aid dispenser.

Orocaecal transit time (OCTT) using the lactulose hydrogen breath test was investigated in 16 patients (age 10-19.4 years) with active Crohn disease (CD). Disease activity was assessed by the paediatric CD activity index (PCDAI). OCTT was prolonged in all patients (mean 149.9 min, SD 32.7) relative to healthy age-matched controls (mean 56.9 min, SD 11.1). PCDAI was increased (median 48.8, range 32.5), indicating moderate to severe disease in all patients. A close correlation between OCTT and PCDAI (Spearman r = 0.90) was observed. Following nutritional therapy with a semi-elemental diet over a period of 6 weeks OCTT and PCDAI declined, OCTT becoming normal in 4 and PCDAI in 4/12 patients respectively. OCTT provides a sensitive, non-invasive method for the assessment and followup investigations in patients with CD.

Some time ago audiological screening in infants was mostly performed using musical instruments or equivalents. The use of acoustic evoked potentials and oto-acoustic emissions changed the strategies of hearing assessment in newborns and infants, however, musical instruments are still in use. An adequate interpretation of screening results obtained with musical instruments necessitates a profound knowledge of frequencies and intensities derived from these instruments. In this study spectral analyses of sounds from these instruments were performed. The results may be of value for those using musical instruments as a tool for audiological screening. In addition, the results show that with some instruments intensities able to cause inner ear damage can be generated.

Small glomus jugulare tumors can be operated on successfully. The intraoperative risk of bleeding may be reduced by preoperative irradiation or angiographic embolisation. Patients with advanced tumors (bone destruction, paralysis or cranial nerves and/or invasion of brain) are at high risk if they are operated on. In these cases radiotherapy is an effective alternative with a fairly low complication rate. Prerequisites for successful irradiation are assessment of treatment volume and treatment planning by CT, sophisticated stereotactic irradiation techniques, application of high energy photons and reliable immobilisation measures. Furthermore it is possible to deliver higher doses, leading to regression of the tumor and its symptoms. The technique is illustrated by two characteristic cases. Although irradiation alone can achieve tumor remission the long term prognosis remains doubtful.

Creatinine clearance as a measure of the glomerular filtration rate is calculated from the serum creatinine, urine creatinine concentration and the 24-h urine volume (MCC). Cockroft and Gault as well as Davila and Gardener have shown that estimation of the creatinine clearance (ECC) from the serum creatinine, age, sex, and weight of the patient is at least as accurate as measurement of creatinine clearance (MCC). ECC is not influenced by inaccurate collections of urine or incorrect analyses of urine creatinine. 125 MCC was investigated in 51 patients receiving cisplatin. If ECC is less than or equal to 80, then MCC should eliminate inaccurate urine collections. If the value of ECC exceeds 80, it is not necessary to measure MCC.
