RESUMEN

Service response to problems in 20 clinical and social competence areas was assessed in 156 long-stay in-patients, using the Needs for Care Assessment (NFC). Although there were few unmet needs for treatment, over 200 instances of unmet needs for the assessment of social skills were found, and reasons for this are discussed, as is the importance of social-skills assessment in this group. In around a third of the sample most of the social-skill areas in the NFC were not applicable.

RESUMEN

Long-stay psychiatric in-patients in South Glamorgan were reviewed using the MRC Needs for Care assessment. The majority were middle aged or elderly and were severely disabled, with problems encompassing symptoms/behavioural difficulties, social skills, and physical health. In most, long-standing psychiatric illness was compounded by physical illness and cognitive deficits as age advanced, producing care needs that were greater than existing community services could meet.

RESUMEN

A year's follow-up study of day-hospital long-attenders underlined doubts as to the adequacy of service provision in urban areas of England and Wales for the increasing numbers of chronic psychiatric patients living in the community. Very few of the 82 men and 53 women were married or capable of employment, and both psychiatric and physical morbidity were high; 2 young men committed suicide and 3 elderly men died within a year of discharge. Over the age of 45 men tended to be socially isolated and over the age of 60 they were also physically ill; a number probably required better residential care. Few women under 45 attended, possibly because the services offered were unacceptable.

RESUMEN

The theory of callosal failure in schizophrenia is examined using the test for left-hand anomia. It is argued that if the corpus callosum is affected in schizophrenia with consequent difficulty of transmission between the two sides of the brain, a greater number of left-hand errors should be found in producing the names of objects held by that hand because the speech area of the left hemisphere is no longer available for the description of the perceptions of the left hand. The findings support this view and they confirm the communication failures of the brain related to callosal disturbance.

RESUMEN

A study is described of effects of a nootropic drug on chronic schizophrenia. The nootropic drugs act on the central nervous system with the cerebral cortex as their target. Chronic schizophrenic patients on the drug showed improvement in object naming and in tests where the patient was required to indicate the number of times he had been tapped. Improvements were also noted in learning and memory tasks. In dichotic listening the patients showed a reduction in the amount of incorrect verbal responses produced. There were no improvements in symptom rating or social behaviour rating. These results suggest some cognitive improvement but little if any change in the disease state of the patient.

RESUMEN

Chronic in-patients from a sample population of 48 women and 43 men were selected for an experimental hospital-hostel and for local authority hostels. Two-thirds were found suitable for the former but only a sixth for the latter. The problems of selection for and management at the experimental hostel are described, and the overall results discussed in relation to recent proposals for the residential care of chronic patients.

RESUMEN

Various clinical and social measures were applied to forty long-stay schizophrenic patients before and after transfer from hospital to a residential home and similarly to twenty-five patients retained in hospital. The results are used to test specific hypotheses concerning the relationship between social changes and clinical state in chronic schizophrenia.
