RESUMEN

OBJECTIVE: An observational non-interventional study was conducted to obtain data on the efficacy and safety of Prospekta in the treatment of postpsychotic asthenia in patients with cognitive impairment (CI). MATERIAL AND METHODS: We selected 69 patients aged 18-75 years with asthenic disorders that developed after suffering psychotic conditions and CI, who were prescribed Prospekta. At four visits (at baseline, after 2, 4 and 8 weeks), the doctor collected complaints, anamnesis, examined the patient, assessed the severity of asthenia on the MFI-20 (The Multidimensional Fatigue Inventory-20) scale, CI - on the MMSE (The Mini-mental state examination). Concomitant diseases and maintenance therapy of the underlying disease were recorded, and the safety of treatment with Prospect was evaluated. At the last visit, the doctor's clinical impression was assessed using the CGI-I (Clinical Global Impression - Global Improvement Scale). RESULTS: The analysis included data from 69 patients (mean age 45.7 years), of which 27 (33.4%) were women. Prospekta reduced the severity of asthenia on the MFI-20 scale from 85.7±6.6 to 51.6±7.1 points, including general asthenia, mental and physical asthenia, and contributed to an increase in activity and motivation (p<0.001). 8-week treatment with Prospekta improved cognitive function on the MMSE scale from 25.7±3.7 to 28.8±1.5 points (p<0.001). There was no effect of the drug on blood pressure, heart rate. 76 adverse events (AEs) were detected in 22 patients, of which 62 AEs (82%) were of mild severity, 14 AEs (18%) were of moderate severity. A causal relationship of AEs with taking Prospekta, according to doctors, was absent in 48 (63%) cases. CONCLUSION: Prospekta is an effective and safe drug for the treatment of asthenic disorders that have developed after suffering psychotic conditions in patients with CI.

RESUMEN

The only proven cure for Shwachman-Diamond syndrome (SDS) bone marrow failure is allogeneic hematopoietic stem cell transplantation (HSCT). However HSCT with donors other than HLA-identical siblings is associated with high mortality and unfavorable prognosis. This paper presents the first experience of HSCT treatment of SDS using an unaffected HLA-identical sibling produced through preimplantation genetic diagnosis (PGD). The patient was a 6-year-old blood transfusion-dependent SDS baby girl with secondary myelodysplastic syndrome, for whom no HLA-identical donor was available. As a result of PGD, two unaffected HLA matched embryos were identified; one of them was randomly selected for transfer, resulting in a clinical pregnancy and birth of an apparently healthy child. The patient underwent allogeneic transplantation of cord blood hematopoietic stem cells, together with bone marrow from this sibling, resulting in complete hemopoietic recovery. The patient was no longer transfusion-dependent and had normal blood values 160 days after transplantation.

Fifteen odd years have passed since the first application of a gene-therapeutic modality in clinical practice for treatment of lower-limb chronic ischaemia. Over this time, vast experience has been gained worldwide, with not less than one thousand patients treated by gene-based therapies, thus making it possible to generalise the published findings of these clinical trials. Resulting from such an analysis, it should be recognized that the least dangerous gene therapeutic modalities available so far are plasmid ones, with the most efficient being those containing the gene of vascular endothelial growth factor VEGF(165). The most convincing results were obtained while treating chronic ischemia of the lower extremities, whereas gene-based therapy used for treatment of coronary artery disease failed to have yielded, as of yet, clear cut positive results.

RESUMEN

The formation of new blood vessels is essential for the reparation of ischemic tissue. Animal model studies of myocardial ischemia showed that cell transplantation, gene therapy, and application of growth factors are able to promote collateral circulation development. Subsequent clinical trials conducted with these methods used for treatment of coronary artery disease demonstrated the safety and clinical benefits of this novel therapeutic approach called therapeutic angiogenesis. This method of treatment of ischemic lesions associated with cardiovascular diseases opens new clinical options in the area of tissue regeneration.

RESUMEN

Original recombinant adenoviral construction carrying E. coli beta-galactosidase LacZ gene designed by the authors is convenient for labeling and monitoring of bone marrow mesenchymal (stromal) progenitor cells and myocardial and skin fetal cells transplanted in damaged rat tissues (in the perinecrotic zone of the myocardium and onto burnt skin surface) for their reparation. This genetic construction after pre-inactivation of endogenous beta-galactosidase allows to detect transplanted cells in the foci of injury; positive effects of transplantation on tissue reparation processes can be attributed to the presence of transplanted cells.

RESUMEN

The effects of autologous bone marrow stromal cells immobilized on ElastoPHB membranes on reparative processes were studied on a model of rat skeletal muscle injury. Bone marrow stromal cells inhibited substitute (sclerosing) regeneration and activated reparative (reconstructive) regeneration of tissues.

Female patient with extensive skin burn (I-II-IIIAB skin burn, total area 40%, area of IIIB degree 30%) was treated using transplantation of allogenic fibroblast-like bone marrow mesenchymal stem cells onto the surface of deep thermal burn. The study of wound healing dynamics after transplantation of allogenic fibroblast-like mesenchymal stem cells confirmed high tempo of wound regeneration in the presence of active neoangiogenesis. Due to this, autodermoplasty of burn wounds could be carried out with good results as early as on day 4 after transplantation of fibroblast-like mesenchymal stem cells; this led to more rapid healing of donor zones and accelerated rehabilitation of the patient.

RESUMEN

Transplantation of suspention of allogenic fibroblast-like mesenchymal stem cells (AFMSC) of the bone marrow and AFMSC immobilized on the biodegradable membrane (BM) stimulates healing of deep burn wounds compared to control (without cell transplantation) because these cells produce biologically active substances into the wound. AFMSC immobilized on BM activate repair processes in the wounds earlier. This may be due to their higher functional activity created in the monolayer by adequate intercellular interactions. To avoid complications caused by low diffuse properties of BM (protein precipitation), BM should be removed from the surface of the burn wound maximum 3 days after its application.

RESUMEN

The paper deals with a universal component-oriented architecture for creating the telemedicine applications. The worked-out system ensures the ECG reading, pressure measurements and pulsometry. The system design comprises a central database server and a client telemedicine module. Data can be transmitted via different interfaces--from an ordinary local network to digital satellite phones. The data protection is guaranteed by microchip charts that were used to realize the authentication 3DES algorithm.

RESUMEN

The effect of cell transplantation into cryodamaged rat myocardium was studied on isolated hearts by increasing functional load to the left ventricle. Transplantation of allogeneic fetal cardiomyocytes improved the function of the left ventricle under conditions of considerably increased preload. Transplantation of autologous mesenchymal stem cells repaired left-ventricular function under conditions of increased pre- and afterload.

RESUMEN

We describe the possibility of obtaining cardiomyocyte-like cell cultures from rat, guinea pig, and human bone marrow stromal stem cells. The content of troponin I-positive cells attains 35-45% of the total number of cells in the cultures and persists at this level for up to 4 months under differentiation conditions. Spontaneous contractions of cardiomyocyte-like cells were observed after the formation of cell monolayer under differentiation conditions.

RESUMEN

Nitric oxide (NO) is the molecule that is one of the main elements of the antipathogenic activity of phagocytes that are involved in the provision of the body's resistance to the penetration and development of infections, including tuberculous one. The levels of final NO metabolites (nitrites and nitrates) in the blood, bronchoalveolar lavage fluid, and cultured alveolar macrophageal supernatant were measured during examination of 74 patients with focal and infiltrative pulmonary tuberculosis. There were decreased levels of final NO metabolites in all biological fluids in the examinees as compared with healthy individuals. The degree of the decreases in NO concentrations correlated with the severity and extent of a specific pulmonary process; it was the least in focal tuberculosis and the greatest in the infiltrative process with decay.

The paper addresses the issue of device management system design for software agents compliant with IEEE 1073 device communication standard. Based on middleware architecture the device control layer represents a universal versatile object-oriented application-programming interface. The approach presented in the paper allows to implement plug-and-play integration and interoperability of medical acquisition devices within the medical device system be means of common middleware services. Adherence to Medical Data Information Base nomenclature, component part of IEEE 1073 communication standard, adds necessary consistency to presented component-based infrastructure.

RESUMEN

The paper deals with general questions of the Component Object Model (COM) architecture for ECG software applications. Here their common structure is presented, the benefits and constraints of component-based applications are considered. Being a component model, COM allows one to build any application the functionally complete modules. Each module is developed by the particular protocol that gives developers a possibility to provide effective interoperability between such components. The separate part of the article is devoted to reliability and performance issues of the component-based ECG applications. The approach described in this article permits one to solve a number of problems facing medical software developers by using the effective and flexible component architecture. As implementation of the discussed architecture, a software ECG system being developed by a team of authors is presented.
