RESUMEN

BACKGROUND: Introducing irinotecan and oxaliplatin in to the treatment of advanced colorectal cancer substantially improved the therapeutic results for this malignancy. The first results of clinical trials with these two drugs were published in 2000. METHODS AND RESULTS: Between 1999 to 2004 we treated 51 patients with the combination of irinotecan 180mg/m2 on day 1 and two hour infusion of leucovorin 200mg/m2 and 5-FU push of 400mg/m2 followed by infusion of 5- FU for 22 hours on days 1 and 2 every 2 weeks. Six patients (11.7%) achieved complete response, 11 (21.57%) partial response, stabilisation was observed by 23 patients (45.1%) and 21 patients were progressive (21.5%). The median survival time was 18 months (95% CI, 16.93-19.7), median duration of response was 9 months (Cl 95% 8.25-11.5). CONCLUSIONS: The combination of FOLFIRI is an effective and tolerable treatment of advanced colorectal cancer. However new treatment modalities to improve further the results of the treatment are still warranted.

RESUMEN

In the course of chronic pancreatitis diverse complications may arise. Intraparenchymal splenic complications are very rare. In the following case the authors describe rupture of splenic pseudocyst with acute intraabdominal bleeding as a complication of chronic pancreatitis.

BACKGROUND: The aim of our study was to determine validity of computed tomography in staging of non-small cell lung cancer. METHODS AND RESULTS: Sixty-two patients with NSCLC were operated at our department between March and September 2003. Lymph nodes with the shortest diameter over 10 mm on CT were considered abnormal. Primary tumor was correctly determined by CT scans in 77 % of cases, lymph nodes involvement in 63 %. Stage of NSCLC was correct in 53% of all patients. Negative predictive value for N1 and N2 was 80 %, resp. 88 %. CONCLUSIONS: Even with improvement in CT technology, validity of CT in staging of NSCLC remains low. We consider that mediastinoscopy can be avoided in the presence of normal mediastinal CT findings due to high negative predicative value of nodal improvement.

RESUMEN

Intestinal duplications are described as uncommon congenital anomalies rarely diagnosed in adults. A case of young woman with obscure abdominal symptomatology is demonstrated. After investigation the cause of it was disclosed surgically. During operation a cystic ileal duplication was exposed and resected. Despite of difficulties calling for another operation, the case was successfully solved. Based on the author's experience some aspects of these diseases--etiopatogenesis, symptoms, possibilities of examination and complications are discussed and surgery as the main treatment is emphasized.
