RESUMEN

A 6-week-old girl with a hemangioma with thrombocytopenia, consumption coagulopathy, extensive bleeding, disappearance of the right pubic bone, and destruction of part of the right iliac bone was successfully treated with prednisone. Treatment lasted 30 weeks. Bleeding ceased within a few days; bone rebuilding was first observed at 30 days and was extensive at the end of treatment. When the child was 3 years old, pelvic films were normal. The occurrence of Kasabach-Merritt syndrome associated with complete osteolysis as in our patient is rare and bone reconstruction is unusual.

RESUMEN

Various cases are presented demonstrating the role of computed tomography (CT) in the assessment of serosal and bowel wall pathology. Reference is made to the morphology of the lesions. Illustrative examples of tumors, secondary malignant dissemination, irradiation injury to the gut, and intramural gas associated with ulcerative colitis, are all illustrated.

RESUMEN

Between 1964 and 1980 we reviewed 103 patients with benign renal cysts. Their treatment ranged from nephrectomy to observation and follow-up only. It was concluded that the best form of therapy for this condition is regular follow-up.

RESUMEN

Seventeen patients with arterial occlusive disease, of whom 14 were diabetics, were studied by angiography after amputation of the lower extremity. In one patient the angiography was normal and the stump wound healed well. In two patients the angiographic findings indicated extremely severe impairment of blood supply and wound healing was considerably delayed. Of the remaining 14 patients: in five the superficial femoral artery alone was occluded and wound healing was normal and in nine patients, both the superficial and deep femoral arteries were occluded and wound healing was delayed. Wound healing of the stump after amputation of the lower extremity due to extensive arterial occlusive disease is decided by the patency of the deep femoral artery.

RESUMEN

Enlarged Brunner glands (EBG) are an occasional finding in upper gastro-intestinal barium studies. The clinical manifestations vague upper abdominal symptoms or bleeding are rare. Brunner gland secretion is stimulated by different stimuli but especially by secretin. In our report E.B.G. has been observed in 9/15 chronically hemodialysed patients. This uncommon frequence of E.B.G. can be related to the high blood levels of secretin found in patients with chronic renal failure. E.B.G. could explain some of the gastro-intestinal complications occuring in these patients.

RESUMEN

Failure of the contrast medium to enter the duodenum during the operative cholangiography following the common bile duct exploration and instrumentation may be caused by transient obstruction of the common bile duct. It is attributed to spasm and oedema of the sphincter choledochus of Boyden and not of the sphincter of Oddi. The knowledge of the possibility of the pseudo-obstruction of the common bile duct following its exploration may save the surgeon from unnecessary re-exploration and performing sphincterotomy or choledochoduodenostomy. The exploration is unnecessary if, during the common duct exploration, the dilators and the catheter passes easily into the duodenum and on the cholangiogram the obstructed area is smooth and there is no filling defect. A normal cholangiogram performed 8-10 days following the operation proves that the obstruction was transient and not caused by calculus stricture or tumour.
