BACKGROUND AND PURPOSE: The aim was to investigate whether neurofilament light chain (NfL) and profilin-1 (PFN-1) might qualify as surrogate disease and treatment-response biomarkers by correlating their concentrations dynamic with clinical status in a cohort of 30 adult spinal muscular atrophy type 3 patients during nusinersen therapy up to 34 months. METHODS: Neurofilament light chain was measured in cerebrospinal fluid at each drug administration with a commercial enzyme-linked immunosorbent assay (ELISA); PFN-1 concentrations were tested in serum sampled at the same time points with commercial ELISA assays. Functional motor scores were evaluated at baseline, at the end of the loading phase and at each maintenance dose and correlated to biomarker levels. The concurrent effect of age and clinical phenotype was studied. RESULTS: Neurofilament light chain levels were included in the reference ranges at baseline; a significant increase was measured during loading phase until 1 month. PFN-1 was higher at baseline than in controls and then decreased during therapy until reaching control levels. Age had an effect on NfL but not on PFN-1. NfL was partially correlated to functional scores at baseline and at last time point, whilst no correlation was found for PFN-1. CONCLUSION: Cerebrospinal fluid NfL levels did not qualify as an optimal surrogate treatment biomarker in adult spinal muscular atrophy patients with a long disease duration, whilst PFN-1 might to a greater extent represent lower motor neuron pathological processes. The observed biomarker level variation during the first 2 months of nusinersen treatment might suggest a limited effect on axonal remodeling or rearrangement.

BACKGROUND: 'Scared Straight' and other programmes involve organised visits to prison by juvenile delinquents or children at risk for criminal behavior. programmes are designed to deter participants from future offending through first-hand observation of prison life and interaction with adult inmates. These programmes remain in use world-wide despite studies and reviews questioning their effectiveness. OBJECTIVES: To assess the effects of programmes comprising organised visits to prisons by juvenile delinquents (officially adjudicated or convicted by a juvenile court) or pre-delinquents (children in trouble but not officially adjudicated as delinquents), aimed at deterring them from criminal activity. SEARCH STRATEGY: Handsearching by the first author in identifying randomised field trials 1945-1993 relevant to criminology was augmented by structured searches of 16 electronic data bases, including the Campbell SPECTR database of trials and the Cochrane CCTR. Experts in the field were consulted and relevant citations were followed up. SELECTION CRITERIA: Studies that tested the effects of any program involving the organised visits of juvenile delinquents or children at-risk for delinquency to penal institutions were included. Studies that included overlapping samples of juvenile and young adults (e.g. ages 14-20) were included. We only considered studies that randomly or quasi-randomly (i.e. alternation) assigned participants to conditions. Each study had to have a no-treatment control condition with at least one outcome measure of "post-visit" criminal behavior. DATA COLLECTION AND ANALYSIS: We report narratively on the nine eligible trials. We conducted one meta-analysis of post-intervention offending rates using official data. Information from other sources (e.g. self-report) was either missing from some studies or critical information was omitted (e.g. standard deviations). We examined the immediate post-treatment effects (i.e. "first-effects") by computing Odds Ratios (OR) for data on proportions of each group re-offending, and assumed both fixed and random effects models in our analyses. MAIN RESULTS: The analysis show the intervention to be more harmful than doing nothing. The program effect, whether assuming a fixed or random effects model, was nearly identical and negative in direction, regardless of the meta-analytic strategy. REVIEWER'S CONCLUSIONS: We conclude that programmes like 'Scared Straight' are likely to have a harmful effect and increase delinquency relative to doing nothing at all to the same youths. Given these results, agencies that permit such programmes must rigorously evaluate them not only to ensure that they are doing what they purport to do (prevent crime) - but at the very least they do not cause more harm than good.

RESUMEN

In recent years, there have been several proposals for the realization of models inspired to biological solutions for pattern recognition. In this work we propose a new approach, based on a hierarchical modular structure, to realize a system capable to learn by examples and recognize objects in digital images. The adopted techniques are based on multiresolution image analysis and neural networks. Performance on two different data sets and experimental timings on a single instruction multiple data (SIMD) machine are also reported.

The aims of this investigation were to cephalometrically study the short-term skeletal and dental modifications induced by rapid palatal expansion in a sample of 20 patients (10 male, 10 female), aged 6-10 years (mean age 8 years) in mixed dentition with a uni- or bilateral posterior crossbite, a mild skeletal Class II malocclusion, and an increased vertical dimension (FMA, SN/\GoGn), and to compare them with an untreated matched control group of 20 subjects (10 male and 10 female), mean age 8 years. Cephalometric analysis showed that the maxilla displayed a tendency to rotate downwards and backward, resulting in a statistically significant increase of the SN/\PP angle (T0 = 9*95 degrees, T1 = 11*60 degrees, P < 0*01) and the SN-ANS linear value (T0 = 49*50 mm, T1 = 51*10 mm, P < 0*05). In addition, there was a statistically significant alteration of the anterior total facial height N-Me (T0 = 113*15mm, T1 = 114*15 mm, P < 0*05) and for the dental upper molar measurement U6-PP (T0 = 19*70 mm, T1 = 20*30 mm, P < 0*05). The small alterations found in the anterior total facial height and in the sagittal angles agree with previous studies, and suggest that RPE can be also used in subjects with a tendency to vertical growth and a skeletal Class II malocclusion.

RESUMEN

The author examines the role of mediators and moderators in the evaluation of programs for children. The terms are defined and examples of each are presented. Using bibliometric analysis, the author examines how evaluators use mediators and moderators in treatment studies in education, juvenile justice, health care, child protection, and mental health. The use of mediators and moderators is sporadic and vague at best. An agenda for improvement is outlined that includes greater use of program theory and intensive case studies to find out why researchers in prevention and health promotion incorporate mediators and moderators more effectively in their evaluations.

RESUMEN

A new parallel thinning algorithm with two subcycles is proposed and compared with other parallel thinning algorithms in terms of 8-connectedness degree, erosion, stability under pattern rotation, and boundary noise sensitivity. Computational issues are also reported based on the implementation of the thinning algorithm on the SIMD machines CM-200 and MasPar MPP-12000.

The chemiluminescence of alveolar macrophages was determined in presence of luminol or lucigenin, either before or after the macrophage isolation by adherence to plastic. In presence of luminol, the purified and non-purified macrophages behave almost in the same way, and their chemiluminescence activity is stimulated both by zymosan and phorbol ester. On the other hand, the lucigenin chemiluminescence of non-purified macrophages is considerably higher than that obtained in presence of luminol, but is almost completely insensitive to the zymosan or phorbol ester stimulation. Moreover, the adherence purification considerably inhibits the lucigenin chemiluminescence. These results can be indicative of functionally different macrophage states.
