RESUMEN

The performance evaluation of services through instruments is relevant, as it can support thoughts, actions and political approaches to meet a social need. The objective of the article was to develop and validate the Quality and Strengthening of Primary Care Questionnaire (QSPC-Q) for professionals and users based on Starfield attributes and Donabedian pillars. A mixed sequential study was performed to develop the QSPC-Q. The test was applied to 149 doctors and 795 users of basic health units. Psychometric properties was assessed by testing internal consistency using Cronbach's alpha and exploratory factor analysis. Reproducibility od scale was assessed using intraclass cognitive and test-retest correlation. The final version of the follow-up consisted of 45 items aimed at physicians (Cronbach's alpha = 0.921) and 33 at users (Cronbach's alpha = 0.86); the intraclass respiratory capacity was 0.88. An exploratory factor analysis identified 13 factors associated with the pattern components. A short version with 29 items for professionals was also elaborated. Professional QSPC-Q (short and braided versions) and user QSPC-Q were valid.

Resumo Este estudo investigou a mediação da autoestima nos comportamentos de saúde bucal dos adolescentes. Para avaliar a autoestima usou-se a escala de Rosenberg, já as características sociodemográficas e comportamentais foram analisadas por questões validadas em outros inquéritos. Com exceção da alimentação, a maior proporção dos adolescentes possuía bons comportamentos de saúde bucal. O número de adolescentes com alto nível de autoestima foi relevantemente menor que aqueles com baixa autoestima. A utilização dos serviços odontológicos, embora associado ao alto nível de autoestima, perdeu significância após ajustado pelo sexo, idade e escovação dos dentes. Porém, a análise de regressão logística múltipla, através das estimativas não-ajustadas e ajustadas com seus respectivos Intervalos de Confiança de 95%, evidenciou associação da autoestima com idade (p-valor=0,001) e frequência de escovação (p-valor=0,019). Independente do sexo, escolares maiores de 16 anos e com autoestima elevada, escovam seus dentes com maior frequência, adquirindo, possivelmente, melhor saúde bucal. Isto confirma a modulação da autoestima sobre os comportamentos de saúde bucal, e atesta a necessidade de implementar a análise e o exercício desse fator psicossocial na assistência à saúde bucal dos jovens.


Abstract This study investigated the mediation of self-esteem in adolescents' oral health behaviors. The Rosenberg self-esteem scale was used to assess self-esteem, whereas data from socio-demographic and behavior characteristics were analyzed by questions validated in previous surveys. The teenagers had good oral health behavior, except unhealthy diet. The number of adolescents with high self-esteem was a lot smaller than those with low self-esteem. The use of dental services, even when associated with high self-esteem, lost significance after being adjusted by sex, age and tooth brushing frequency. Nevertheless, multiple logistic regression analysis, using unadjusted estimates and adjusted with their respective Confidence Intervals of 95%, showed a relationship of self-esteem with age (p-value=0.001) and tooth brushing frequency (p-value=0.019). Regardless of the sex, students over 16 years old with high self-esteem brush their teeth more often, having probably better oral health. These results confirm the modulation of self-esteem in oral health, and then it is necessary the analysis and the use of these psychosocial factors in the young oral health care.

Since the co-occurrence of several health-risk behaviors is associated with an increase in chronic diseases, the study of clustering is relevant. The aim of this study was to evaluate how seven types of general and oral health-risk behaviors, cluster among adolescents. A cross-sectional analysis was performed with a sample of high school students from state public schools in São Lourenco da Mata, State of Pernambuco, Brazil (n = 1154). Data were obtained using a self-reported questionnaire. To identify cluster solutions of seven general and oral health-risk behaviors, Hierarchical Agglomerative Cluster Analysis (HACA) was performed. Most of the adolescents participating in the study was female (54.3%) and aged <16 years old (77.1%). HACA indicated two broad stable clusters for the seven health-risk types of behaviors. The first cluster included following behaviors: smoking, drinking and less frequent tooth brushing. The second cluster reveals the combination of high bread, pasta and snack intake; high intake of sweets; high intake of soft drinks; low intake of fruits and vegetables. Results provided by HACA identified two groups of health-risk behaviors. The first cluster mainly shows risk (problematic) behaviors, whereas the second cluster denotes the non-adhesion of preventive behavior (non-healthy diet). Health-compromising behaviors are common among teens and occur in distinct clusters. These findings could be used by schools, health promotion authorities and other stakeholders to design and implement tailored preventive interventions in Pernambuco, Brazil. Therefore, clustering of several types of behavior has important implications for a comprehensive strategy in health promotion policies and practices.

Este trabalho teve por objetivo conhecer as expectativas, projetos profissionais e projeções de êxito nas atividades laborais futuras dos estudantes de odontologia da Universidade Federal de Pernambuco. O estudo foi realizado no âmbito da própria universidade, nas dependências do curso de odontologia. A população de estudo foi de estudantes do 1º período diurno e noturno (n=43), 5º período (n=41) e 10º período (n=21). A maioria dos estudantes entrevistados consistia de jovens, solteiros (99%) e do sexo feminino (65,7%), com renda familiar de até R$ 5.100,00 ao mês (80,9%). Observou-se que 83,8% optaram por estudar odontologia por se identificar com a profissão. Os alunos também reconheceram o padrão de qualidade das Instituições Federais de Ensino Superior e este foi o motivo de escolha pela universidade (74,3%). Não houve diferenças quanto a ingressar futuramente no mercado laboral público ou privado, sendo ambas opções viáveis na concepção dos entrevistados. A promoção de saúde foi o fator relatado como principal fonte de realização profissional (71,4%), especialmente para os alunos de períodos iniciais do curso (p=0,026). A estimativa inicial de renda está dentro dos limites de renda familiar atual da maioria dos entrevistados (59%) e 98,1% pretendem fazer curso de pós-graduação, tendo nas especializações o objetivo mais comum (78,6%). Especialidades como cirurgia bucomaxilofacial, ortodontia e implantodontia serão as mais procuradas (AU).


This study aimed to know the expectations, professional projects and projections of success in the work activities of dental students from the Federal University of Pernambuco. The study population consisted of dental students from 1st daytime and nighttime periods (N = 43), 5th period (N = 41) and 10th period (N = 21). Most students interviewed were young, unmarried (99%) and female (65.7%), having a family income of up to R$ 5,100.00 per month (80.9%). We found that 83.8% of them chose to study dentistry because they felt identified with the profession. Students also recognized the quality standard of Federal Institutions of Higher Education and this was the reason for choosing the university (74.3%). There were no differences regarding to join the public or private labor markets in the future, being both viable options. Health promotion was reported as the main source of job satisfaction (71.4%), especially for students from early periods of the course (p=0.026). The initial estimate of income is within the limits of current family income for the majority of respondents (59%). Of respondents, 98.1% plan to join a postgraduate course and specializations was the most common goal (78.6%). Specialties such oral and maxillofacial surgery, orthodontics and implantology are most sought (AU).
