RESUMEN

G(-50) --> T promoter polymorphism of gene encoding human epoxygenase 2J2 in 107 patients with acute coronary syndrome and in 104 practically healthy people was determined. It was shown that interrelation of genotypes G/G, G/T and T/T was 91%, 9% Ta 0% correspondingly (in control--92%, 7%, 1%; P > 0.05 by chi2-test). The data indicate that epoxygenase 2J2 gene polymorphism is not a risk factor of acute coronary syndrome in Ukrainian population.

RESUMEN

Endothelial NO-synthase (eNOS) gene allelic polymorphism in 221 patients with acute coronary syndrome and in 83 practically healthy people was determined. It was shown that interrelations of normal homozygotes, heterozygotes and pathologic homozygotes in T/C promoter polymorphism analysis accout 48%, 36% and 16% correspondingly (in control--48%, 46%, 6%; P < 0.05 by chi2-test); in G894 --> T polymorphism ofexon 7 analysis--34%, 58%, 8% (in control--29%, 67%, 4%; P > 0.05), and in determination of 4a/4b polymorphism of intron 4--64.5%, 31% and 4.5% (in control--62.5%, 32.5%, 5%; P > 0.05). Obtained data show that eNOS C/C promoter variant is a risk factor of acute coronary syndrome in Ukrainian population.

RESUMEN

Large multifunctional protease LMP2 (Arg60-->His) and LMP7 (Lys145-->Gln) gene allelic polymorphism in 200 patients with acute coronary syndrome and in 80 practically healthy people was determinated. It was shown that interrelation of genotypes Arg/Arg, Arg/His and His/His in LMP2 gene polymorphism is 52, 40.5 and 7.5 % correspondingly (in control group 53.8, 38.7, 7.5 %; P > 0.05 by chi2-test). Analysis of LMP7 gene polymorphism has shown that Lys/Lys - 89.5 %, Lys/Gln - 10.5 %, Gin/Gin - 0 % (in control group 93.8, 6.2, 0 % correspondingly; P > 0.05). The data show that LMP2 and LMP7 gene polymorphism is not a risk factor of acute coronary syndrome in Ukrainian population.

RESUMEN

The aim of the study was to discover determinants of the onset of life threatening ventricular tachyarrhythmia. 102 patients with acute myocardial infarction and 32 patients with postinfarction cardiosclerosis were examined. The data gained at programmed ventricular stimulation, high resolution ECG, Holter ECG monitoring, spectral analysis of cardiac rhythm variability and two-dimension echocardiography proved the importance of early postinfarction left ventricular dilatation in the onset of electrical cardiac instability, establishment of anatomic substrate of monomorphic ventricular tachycardias, low variability of cardiac rhythm. The findings evidence that MI patients with late ventricular potentials and end-diastolic left ventricular index above 100 ml/m2 in subacute period may be considered as a group of high risk in need of individual approach and further therapy. The discovered lowering of informative value of non-invasive methods in patients late after MI dictates the necessity of new approaches to detection of electrophysiological disturbances and sudden death risk in registration of postinfarction dilatation of the heart.

RESUMEN

A group was formed of patients presenting with electrical instability of myocardium (those persons manifesting induction of ventricular tachicardia, ventricular fibrillation) after performing programmed stimulation of ventricles of the heart in 63 patients suffering from myocardial infarction and unstable angina pectoris. The main noninvasive markers of electrical instability were shown to be postinfarction cardiosclerosis, circulatory insufficiency, evolution of acute aneurysm of the heart. Institution of differentiated therapy involving correction of circulatory insufficiency with capoten, low doses of beta-blockers, specified use of antiarrhythmic drugs tailored to the individual, employing electrophysiologic testing, make it possible to improve the intrahospital diagnosis in myocardial infarction and select a group of patients to be surgically tested.

RESUMEN

Having studied 455 cases of documented ventricular fibrillations and ventricular tachycardias leading to circulatory arrest, the authors distinguished clinical factors of unfavourable prognosis of the disease at early and late ventricular tachyarrhythmias, their primary and secondary forms. The possible role is stressed of prolonged persistence of electrical instability of the myocardium in late primary tachyarrhythmias which also include arrhythmias of the 2nd myocardial infarction day. The necessity is substantiated of early electrophysiological examination in myocardial infarction complicated by circulatory arrest or during high risk of its development.

RESUMEN

The authors studied the clinical course of myocardial infarction during the acute period, the central and peripheral hemodynamics, immunological reactivity, dynamic electrocardiography by the Holter method in 107 patients with macrofocal and transmural myocardial infarction. It is concluded that it is rational include into the complex treatment of this condition the silicic enterosorbent "Polysorba". The authors recommend to begin polysorba treatment from the first day of onset of the disease especially in the presence of clinical signs of acute circulatory insufficiency. It is suggested to increase the bioavailability and bioassimilability of medicinal agents using the enterosorbent.

The authors compare the psychologic reactions to acute stress situations due to acute myocardial infarction and in patients operated on for acquired heart disease. To evaluate the emotional experience of the patients psychodiagnostic methods were used. Some of them were specially developed by the authors. New data were obtained on the specific psychosocial adaptation and psychosocial defense of these categories of patients.

RESUMEN

The authors analyze the efficacy of their method of prolonged analgesia in acute myocardial infarction. A comparison of the clinical efficacy of epidural analgesia with obsidan and local anesthetics hemodynamic effects of epidural and intravenous administration of obsidan indicates that use of the new method of analgesia ensures adequate anesthesia, provides an improvement of the clinical course of the disease and has no adverse effects on the circulation.
