RESUMEN

The work presented in this paper comprises the methodology and results of a pilot study on the feasibility of a wireless health monitoring system designed under main EU challenges for the promotion of healthy and active ageing. The system is focused on health assessment, prevention and lifestyle promotion of elderly people. Over a hundred participants including elderly users and caregivers tested the system in four pilot sites across Europe. Tests covered several scenarios in senior centers and real home environments, including performance and usability assessment. Results indicated strong satisfactoriness on usability, usefulness and user friendliness, and the acceptable level of reliability obtained supports future investigation on the same direction for further improvement and transfer of conclusions to the real world in the healthcare delivery.

RESUMEN

Hip problems in cerebral palsy are relatively frequent (25-75%). Subluxation and dislocation of the hip is proportional to the neuromuscular involvement and is often due to alteration caused by spastic muscle forces acting on the femoral head in the acetabular cavity. The EMMA approach (Early Multilevel Minimally-invasive Approach) has been designed to restore muscle balance, decrease hip migration and prevent bone deformities thereby avoiding future pain with minimal biological cost to the patient. EMMA is suitable for most patients, especially those with increased tone, poor muscle control and selectivity, Reimer Index (R.I.) 20%. We consider age and R.I crucial prerequisites for treatment steps. EMMA 1) age 2-4 years, RI 20%: multilevel injection of botulinum toxin in case of muscular hyperactivity without morphological alterations of the couple muscle-tendon (contractures). EMMA 2) age 4-6, RI 20%: multilevel aponeurectomies in case of muscular hyperactivity with morphological alterations of the couple muscle-tendon (retraction). EMMA 3) early bone surgery (growth plates). This approach has been adopted in the last 4 years to prevent bone deformities and give early mobilisation and early control of the pain. EMMA is simple to apply even in infants, both for hip containment and to decrease spasticity.

PURPOSE: Hamartomas are rare benign breast tumors which surfaced with the entrance of mammography in the diagnostic algorithm of breast lesions. In the present study the mammography findings of breast hamartomas were analyzed in relation to the histologic confirmation of the diagnosis. PATIENTS AND METHODS: During the seven-year period between January 1990 and January 1997 we studied 45 patients, aged from 32 to 66 years, with a clinical and/or mammographic finding of a breast lump, highly suspicious of hamartoma. In 29 (64.4%) the tumor was found in the left breast and in the remaining 16 (35.6%) in the right breast. RESULTS: The preoperative mammography of 41 patients revealed a nodular opaque mass of a nonhomogeneous composition and only four had the typical hamartoma picture. The tumor diameter ranged between 0.7-6.5 cm. All tumors were excised and the histological reports confirmed the diagnosis of hamartoma. CONCLUSION: Mammography helps in the diagnosis of hamartomas. Nevertheless, the final diagnosis must be based on the histological analysis of the tumor.
