BACKGROUND: Access to essential diagnostics is crucial for primary healthcare (PHC) in low-and-middle income countries (LMICs). Many LMICs have invested in equipping PHC with point-of-care (PoC) diagnostics for infectious diseases, however there has been no similar investment to improve PHC capacities for clinical chemistry. The biochemistry gap is among the deterrents to universal health coverage. METHODS: A social sciences project was conducted with the aim to understand the key PHC stakeholders' insights on the pertinence of PoC biochemistry for PHC in LMICs. Data generation was conducted between July-November 2023 in Mongolia, Nigeria and Peru. Decision-makers in healthcare delivery, healthcare professionals, and patient and community advocates were engaged using a combination of sampling techniques. Unstructured individual and group conversations, and non-participant observation were conducted. Analysis involved an inductive line-by-line coding on printed transcripts, followed by a deductive coding and theme-by-theme analysis on digitized transcripts. RESULTS: Fifteen, 51 and 20 informants from Mongolia, Nigeria and Peru, respectively, participated. Fifty-five of the 94 informants were female. Most informants considered that PoC biochemistry in PHC would be pertinent, from a clinical and a resources-saving perspective. Those households that currently bear the burden of referrals (i.e., the poor, the bedridden, the older adults) would benefit the most from the deployment of PoC biochemistry for essential biochemistry parameters. Improved access to PoC glycated hemoglobin (HbA1c), lipid, liver and kidney profile was perceived as helpful to inform clinicians' decision-taking. The value of PoC biochemistry for the management of noncommunicable diseases (diabetes, hypertension) and infectious conditions (dengue, malaria, tuberculosis), to improve child health outcomes (severe dehydration in children with diarrhea and/or malnutrition) and to reduce preventable causes of death (dengue-related renal failure) was highlighted. CONCLUSIONS: PoC biochemistry holds potential to revert the impact that the biochemistry gap has for patient care in some LMICs' PHC settings. PoC equipment for parameters such as HbA1c, urea, creatinine or electrolytes could enhance community-level management of preventable causes of mortality, improve service delivery for patients affected by locally-prevalent infectious conditions, and improve the psychosocial and economic wellbeing of patients facing the burden of referrals to remote biochemistry-equipped centers. RESUMEN Introducción: El infarto de miocardio sin obstrucciones coronarias significativas (MINOCA) es una conocida entidad que se ha redefinido recientemente al excluir la injuria miocárdica no isquémica. ReSCAR fue un registro prospectivo, multicéntrico de pacientes hospitalizados por síndrome coronario agudo (SCA) desde enero hasta agosto de 2022. Objetivo: Un objetivo principal del registro fue definir la prevalencia de pacientes con MINOCA, sus características basales clínicas y paraclínicas y evolución intrahospitalaria. Los resultados del análisis respectivo son la base de esta publicación. Material y métodos: Registro nacional prospectivo y multicéntrico de SCA. Diagnóstico de MINOCA de acuerdo con la Cuarta Definición Universal de Infarto de Miocardio. Análisis de las características basales, evolución y diferencias significativas respecto de los pacientes con coronariopatía obstructiva. Los centros participantes debían contar con determinación de troponina de alta sensibilidad y la posibilidad de realizar cinecoronariografía. Resultados: Se incluyeron 984 pacientes de 15 centros. Ochenta y cinco pacientes (8,6 %) tuvieron diagnóstico final de MINOCA al alta. Cuarenta y cuatro pacientes (51,8 %) eran mujeres, la mediana de edad fue de 65 años (53-63), 55 (64,7 %) pacientes eran hipertensos, 44 (51,8 %) dislipidémicos, 9 (10,6 %) diabéticos y 23 (27,1 %) fumadores. La mediana de troponina al ingreso (expresada como múltiplo del percentilo 99) fue de 2,42 (0,85-10,21) y el 71,8 % se presentó con un ECG sin cambios isquémicos. La angiografía coronaria fue normal en el 72,9 % de los pacientes y con lesiones menores de 50 % en el resto. Se realizó resonancia cardíaca a 16 pacientes. La mediana del score GRACE fue de 115 puntos (98-139), lo cual perfila un riesgo intermedio; con una tasa de eventos combinados de accidente cerebrovascular/infarto o muerte de 1,2 %, sin haberse registrado sangrados BARC 2 o más. La prescripción al alta de aspirina fue del 72,9 %, clopidogrel 27,1 %, estatinas 88,2 %, beta bloqueantes 67,1 % y antagonistas cálcicos 22,4 %. Conclusión: En este registro, los pacientes con MINOCA representan una proporción significativa de aquellos con SCA. La tasa de complicaciones intrahospitalarias, incluida la mortalidad, fue baja. Impresiona existir una fuerte oportunidad para profundizar los estudios para confirmar este diagnóstico, sus mecanismos fisiopatológicos y su tratamiento.
ABSTRACT Background: Myocardial infarction with non-obstructive coronary arteries (MINOCA) is a well-known condition that has been recently redefined after excluding non-ischemic myocardial injury. ReSCAR was a prospective, multicenter registry of patients hospitalized for acute coronary syndrome (ACS) from January to August 2022. Objective: One of the main objectives of the registry was to define the prevalence of patients with MINOCA, their baseline clinical and paraclinical characteristics, and in-hospital outcome. This publication focuses on the results of the respective analysis. Methods: We conducted a nationwide, multicenter, prospective study of patients with ACS. The diagnosis of MINOCA was made following the Fourth Universal Definition of Myocardial Infarction. The baseline characteristics of the patients wereanalyzed, as well as their outcome and significant differences with patients with obstructive coronary artery disease. The participating centers should have the availability to measure high-sensitivity cardiac troponin and perform coronary angiography. Results: A total of 984 patients from 15 centers were included. Eighty-five patients (8.6%) had a final diagnosis of MINOCA at discharge. Median age was 65 years (53-63), 48 (51.8%) were women, 55 (64.7%) had hypertension, 44 (51.8%) had dyslipidemia, 9 (10.6%) were diabetics and 23 (27.1%) were smokers. Median high-sensitivity cardiac troponins on admission (expressed as multiples of the 99th percentile) were 2.42 times higher (0.85-10.21) and the ECG had no ischemic changes in 71.8% of the patients. Coronary angiography was normal in 72.9% of the patients and the rest of the cases had coronary artery stenoses < 50%. Sixteen patients underwent cardiac magnetic resonance imaging. The median GRACE score was 115 (98-139), which corresponds to intermediate risk. The event rate for the composite outcome of stroke/myocardial infarction or death was1.2%, and there were no bleeding events BARC ≥type 2. The discharge prescription rate was 72.9% for aspirin, 27.1% for clopidogrel, 88.2% for statins, 67.1% for beta-blockers, and 22.4% for calcium channel blockers. In this registry, patients with MINOCA represent a significant proportion of those with ACS. Conclusion: The rate of in-hospital complications, including mortality, was low. There seems to be a strong opportunity for further investigations to confirm the diagnosis, pathophysiological mechanisms, and treatment of MINOCA.

PURPOSE: Although the intestinal subtype of gastric cancer (GC) is most prevalent around the world, a relatively high prevalence of the diffuse subtype has been reported in some populations of Central American countries, including Guatemala. This study aimed to investigate whether differences exist in the prevalence of the two GC subtypes in the two main ethnic groups in Guatemala, namely Mayan and Mestizo (known as Ladino in Guatemala), between whom significant socioeconomic disparities exist, and to determine whether there is an association with Helicobacter pylori/CagA seropositivity. MATERIALS AND METHODS: Participants included 65 patients with GC and 135 age-/sex-matched controls. Data on ethnicity, H. pylori and CagA seropositivity status, as well as tumor subtype (diffuse or intestinal) were collected. Logistic regression models were fitted to examine the relationship between predictor variables (age, sex, ethnicity, H. pylori, and CagA) and the binary response variable (tumor type). Model selection was based on the Akaike information criterion. RESULTS: The prevalence of diffuse GC was found to be significantly higher in the Mayan compared with the Mestizo population in Guatemala. Although seropositivity for CagA was significantly higher in patients with GC, there were no significant differences between the two GC subtypes. CONCLUSION: This study suggests that there are differences in the prevalence of intestinal and diffuse GC histologic subtypes between the two main ethnic groups in Guatemala. Further studies are warranted, given the potential higher prevalence of the more severe GC subtype in the most vulnerable population.

BACKGROUND/OBJECTIVES: Neck circumference (NC) has been proposed as a simple measurement to identify patients with overweight and obesity. It has been found that adipose tissue at the cervical level is associated with the presence of metabolic alterations. The aim of this study was to estimate the association between NC and indicators of Metabolic Syndrome (MS) to subsequently estimate its capacity to identify the risk of MS compared to waist circumference (WC) and Body Mass Index (BMI). METHODS: A cross-sectional study was carried out with a sample of 286 children 6-9 years old who attended six public primary schools in Jalisco, Mexico. Pearson's correlation coefficients along with sensitivity and specificity tests were performed to analyze the relationship between NC and MS indicators. Odds ratio (OR) and concordance analyses were performed considering the Kappa index. RESULTS: NC showed statistically significant correlations with all MS indicators except for LDL cholesterol and total cholesterol. The cut-off points of NC to identify MS according to sex was >27.4 cm for girls and >29.8 cm for boys. The association of NC with values above the cutoff point and the presence of MS was OR: 21.6 (CI: 7.11-65.74). CONCLUSIONS: NC represents a simple and cost-effective alternative to identify children at risk of MS when compared to BMI and WC.

Nanocompounds are widely used in many fields such as environmental, medicine, or agriculture. Nowadays, these nanocompounds are mainly synthesized by chemical methods, causing environmental pollution and potential health problems. Thus, microorganisms have been investigated as potential nanoparticle green biosynthesizers. The main research is focused on the synthesis of nanoparticles (NPs) using algae, yeast, bacteria, and fungi. Among them, fungi have been the most used, due to their simple and effective mycosynthesis. Fungi as well as other organisms involved in green synthesis of NPs use their secondary metabolites (SMs) to mediate and catalyze the reactions to produce metal nanoparticles (MNPs) as well as being able to act as capping agents producing different physicochemical characteristics and biological activities in the MNPs. Among the various fungi used for mycosynthesis are Trichoderma species, which mediate the production of Ag, Cu, CuO, Zn, ZnO, and other MNPs. Here, we review the main SMs from Trichoderma that have been reported or suggested to contribute to synthesize or act as capping agents and their applications, as well as present the main challenges faced by this type of synthesis.

OBJECTIVE: Diagnostic testing is an important tool to combat the COVID-19 pandemic, yet access to and uptake of testing vary widely 3 years into the pandemic. The WHO recommends the use of COVID-19 self-testing as an option to help expand testing access. We aimed to calculate the cost of providing COVID-19 self-testing across countries and distribution modalities. DESIGN: We estimated economic costs from the provider perspective to calculate the total cost and the cost per self-test kit distributed for three scenarios that differed by costing period (pilot, annual), the number of tests distributed (actual, planned, scaled assuming an epidemic peak) and self-test kit costs (pilot purchase price, 50% reduction). SETTING: We used data collected between August and December 2022 in Brazil, Georgia, Malaysia, Ethiopia and the Philippines from pilot implementation studies designed to provide COVID-19 self-tests in a variety of settings-namely, workplace and healthcare facilities. RESULTS: Across all five countries, 173 000 kits were distributed during pilot implementation with the cost/test distributed ranging from $2.44 to $12.78. The cost/self-test kit distributed was lowest in the scenario that assumed implementation over a longer period (year), with higher test demand (peak) and a test kit price reduction of 50% ($1.04-3.07). Across all countries and scenarios, test procurement occupied the greatest proportion of costs: 58-87% for countries with off-site self-testing (outside the workplace, for example, home) and 15-50% for countries with on-site self-testing (at the workplace). Staffing was the next key cost driver, particularly for distribution modalities that had on-site self-testing (29-35%) versus off-site self-testing (7-27%). CONCLUSIONS: Our results indicate that it is likely to cost between $2.44 and $12.78 per test to distribute COVID-19 self-tests across common settings in five heterogeneous countries. Cost-effectiveness analyses using these results will allow policymakers to make informed decisions on optimally scaling up COVID-19 self-test distribution programmes across diverse settings and evolving needs.

resumen Introducción: Se denomina patología dual a la coocurrencia del trastorno por consumo de sustancias con al menos otro trastorno mental, que a su vez se caracteriza por una clínica heterogénea difícil de diagnosticar y de pobre respuesta al tratamiento. Por esto es necesario la identificación y validación de biomarcadores. Dentro de este grupo, se han reportado posibles biomarcadores electroencefalográficos útiles en el diagnóstico, el tratamiento y el seguimiento, tanto en condiciones neuropsiquiátricas como en trastornos por consumo de sustancias. Este artículo tiene como objetivo revisar la literatura existente acerca de biomarcadores electroencefalográficos en patología dual. Métodos: Revisión narrativa de la literatura. Se realizó una búsqueda bibliográfica en las bases de datos PubMed, Science Direct, OVID, BIREME y Scielo, con las palabras clave: biomarcador electrofisiológico y trastorno por uso de sustancias, biomarcador electrofisiológico y trastornos mentales, biomarcador y patología dual, biomarcador y trastorno por uso de sustancias, electroencefalografía y trastorno por uso de sustancias o trastorno mental comórbido. Resultados: Dado que se ha hallado mayor cantidad de literatura en relación con la electroencefalografía como biomarcador de enfermedades mentales y trastornos por consumo de sustancias y pocos artículos sobre patología dual, se organiza la evidencia como biomarcador en psiquiatría para el diagnóstico y la predicción del riesgo y como biomarcador para patología dual. Conclusiones: Aunque la evidencia no es concluyente, indica la existencia de subconjunto de sitios y mecanismos donde los efectos de las sustancias psicoactivas y la neurobiología de algunos trastornos mentales podrían traslaparse o interactuar.
abstract Introduction: The co-occurrence of substance use disorder with at least one other mental disorder is called dual pathology, which in turn is characterised by heterogeneous symptoms that are difficult to diagnose and have a poor response to treatment. For this reason, the identification and validation of biomarkers is necessary. Within this group, possible electroencephalographic biomarkers have been reported to be useful in diagnosis, treatment and follow-up, both in neuropsychiatric conditions and in substance use disorders. This article aims to review the existing literature on electroencephalographic biomarkers in dual pathology. Methods: A narrative review of the literature. A bibliographic search was performed on the PubMed, Science Direct, OVID, BIREME and Scielo databases, with the key**words: electrophysiological biomarker and substance use disorder, electrophysiological biomarker and mental disorders, biomarker and dual pathology, biomarker and substance use disorder, electroencephalography, and substance use disorder or comorbid mental disorder. Results: Given the greater amount of literature found in relation to electroencephalography as a biomarker of mental illness and substance use disorders, and the few articles found on dual pathology, the evidence is organised as a biomarker in psychiatry for the diagnosis and prediction of risk and as a biomarker for dual pathology. Conclusions: Although the evidence is not conclusive, it suggests the existence of a subset of sites and mechanisms where the effects of psychoactive substances and the neurobiology of some mental disorders could overlap or interact.

AIMS: Development of non-invasive and minimally invasive glucose monitoring devices (NI-MI-GMDs) generally takes place in high-income countries (HICs), with HIC's attributes guiding product characteristics. However, people living with diabetes (PLWD) in low-income and middle-income countries (LMICs) encounter different challenges to those in HICs. This study aimed to define requirements for NI-MI-GMDs in LMICs to inform a target product profile to guide development and selection of suitable devices. METHODS: This was a multiple-methods, exploratory, qualitative study conducted in Kyrgyzstan, Mali, Peru and Tanzania. Interviews and group discussions/activities were conducted with healthcare workers (HCWs), adults living with type 1 (PLWD1) or type 2 diabetes (PLWD2), adolescents living with diabetes and caregivers. RESULTS: Among 383 informants (90 HCW, 100 PLWD1, 92 PLWD2, 24 adolescents, 77 caregivers), a range of differing user requirements were reported, including preferences for area of glucose measurement, device attachment, data display, alert type and temperature sensitivity. Willingness to pay varied across countries; common requirements included ease of use, a range of guiding functions, the possibility to attach to a body part of choice and a cost lower than or equal to current glucose self-monitoring. CONCLUSIONS: Ease-of-use and affordability were consistently prioritised, with broad functionality required for alarms, measurements and attachment possibilities. Perspectives of PLWD are crucial in developing a target product profile to inform characteristics of NI-MI-GMDs in LMICs. Stakeholders must consider these requirements to guide development and selection of NI-MI-GMDs at country level, so that devices are fit for purpose and encourage frequent glucose monitoring among PLWD in these settings.

En la primera parte del presente documento se expone la situación de salud y del Desarrollo Infantil Temprano (DIT) de los niños y niñas en sus primeros 5 años de vida, la situación de salud de las madres y las principales barreras para el ejercicio de los derechos de unos y otras, en el marco de la agenda de los Objetivos de Desarrollo Sostenible (ODS). Para la primera parte se privilegió la información obtenida de fuentes secundarias, de instituciones como el Organismo Andino de Salud Convenio Hipólito Unanue (ORAS-CONHU), la OPS/OMS, UNICEF, UNESCO), el Banco Mundial y otras. Con el fin de contar con una referencia más allá de los seis países miembros del ORAS-CONHU, en varios casos se incorporó información del conjunto de países de ALC (Anexo 1). La segunda parte del documento presenta las políticas, programas, planes u otras intervenciones que se vienen implementando o aplicando en los países andinos, que contribuyan a la salud materna e infantil y que se constituyen en avances y acciones hacia el alcance del óptimo DIT

The current study compares the antibacterial activity of zinc oxide nanostructures (neZnO). For this purpose, two bacterial strains, Escherichia coli (ATCC 4157) and Staphylococcus aureus (ATCC 29213) were challenged in room light conditions with the aforementioned materials. Colloidal and hydrothermal methods were used to obtain the quasi-round and quasi-diamond platelet-shape nanostructures. Thus, the oxygen vacancy (VO ) effects on the surface of neZnO are also considered to assess its effects on antibacterial activity. The neZnO characterization was achieved by X-ray diffraction (XRD), a selected area electron diffraction (SAED) and Raman spectroscopy. The microstructural effects were monitored by scanning electron microscopy (SEM) and transmission electron microscopy (TEM). Furthermore, optical absorption ultraviolet visible spectrophotometry (UV-Vis) and X-ray photoelectron spectroscopy (XPS) analyses complement the physical characterization of these nanostructures; neZnO caused 50 % inhibition (IC50 ) at concentrations from 0.064 to 0.072â mg/mL for S. aureus and from 0.083 to 0.104â mg/mL for E. coli, indicating an increase in activity against S. aureus compared to E. coli. Consequently, quasi-diamond platelet-shaped nanostructures (average particle size of 377.6±10â nm) showed enhanced antibacterial activity compared to quasi-round agglomerated particles (average size of 442.8±12â nm), regardless of Vo presence or absence.

Resumen El objetivo de este estudio es realizar un análisis comparativo del funcionamiento cognitivo de un grupo de consumidores de cannabis y controles sanos, mediante la aplicación de pruebas neuropsicológicas y potenciales relacionados con eventos mismatch negativity (MMN) y P3a en un EEG de 32 canales, y evaluar su asociación con características de consumo. La muestra estuvo conformada por 21 consumidores de cannabis (CN) y 22 controles sanos (SN) entre los 18 a 35 años. El grupo CN puntuó más alto en la lista de palabras IA de la WMS-III y el tiempo del Stroop parte A, mientras que el grupo SN presentó reducción en la amplitud de P3a en F3, C3 y FCZ y mayor latencia en F4. En relación a las características de consumo, la abstinencia al cannabis presentó una correlación fuerte con el porcentaje de retención en la memoria verbal, la latencia del MMNd/P3a en C4 y la amplitud de la P3a en F3, mientras que el número de cigarrillos de cannabis presentó una correlación fuerte con la latencia de P3a en FCZ. En conclusión, nuestros hallazgos corroboran la presencia de alteraciones específicas. Se destaca la P3a como posible marcador neurofisiológico en investigación clínica y traslacional en esta población.
Abstract The objective of this study is to perform a comparative analysis of the cognitive functioning of a group of cannabis users and healthy controls, by applying neuropsychological tests and potential events related to mismatch negativity (MMN) and P3a in a 32-channel EEG, and evaluate its association with consumption characteristics. The sample consisted of 21 cannabis users (CN) and 22 healthy controls (SN) between 18 and 35 years old. The CN group scored higher on the WMS-III IA word list and Stroop time part A, while the SN group had reduced P3a amplitude in F3, C3, and FCZ and higher latency in F4. In relation to the characteristics of consumption, abstinence from cannabis presented a strong correlation with the percentage of retention in verbal memory, the latency of MMNd/P3a in C4 and the amplitude of P3a in F3, while the number of cannabis cigarettes presented a strong correlation with the latency of P3a in FCZ. In conclusion, our findings corroborate the presence of specific alterations. P3a stands out as a possible neurophysiological marker in clinical and translational research in this population.

OBJECTIVES: To assess the values of and attitudes towards the use of rapid SARS-CoV-2 antigen-detection tests for self-testing in a rural and an urban area in Peru. DESIGN: Cross-sectional, street-based population survey. SETTING: A series of over 400 randomly selected street points in Valle del Mantaro and in Lima. PARTICIPANTS: 438 respondents (203 female) participated. They were all older than 17 years and provided informed consent for participation. INTERVENTION: All respondents answered on the spot, a 35-item questionnaire developed in KoboToolbox. PRIMARY AND SECONDARY OUTCOME MEASURES: Primary outcomes of interest were: likelihood to use a SARS-CoV-2 self-test; willingness to pay for a SARS-CoV-2 self-test and likelihood to comply with recommended actions following a positive SARS-CoV-2 self-test result. Bivariate analyses and Poisson regression (PR) analyses were performed to identify significant associations between dependent variables and independent variables pertaining to respondents' characteristics, risk perception and previous experiences with conventional COVID-19 testing. RESULTS: Of the 438 respondents, 51.49% had previous experience with conventional COVID-19 testing; 20.37% had COVID-19 disease; 86.96% accepted the idea of SARS-CoV-2 self-testing; and, 78.95% would be likely to use it if needed. Almost all (94.75%) would pay for a self-testing device (mean acceptable payment: US$10.4) if it was not provided free of charge by health authorities. Overall, 93.12%, 86.93% and 85.32% would self-isolate, report the results and warn their contacts, respectively. Being a female (adjusted PR 1.05, 95% CI 1.00 to 1.09, p<0.018), having completed secondary education (adjusted PR 1.18, 95% CI 1.02 to 1.37, p<0.024) and expressing likelihood to use self-testing (adjusted PR 1.08, 95% CI 1.01 to 1.16, p<0.0.24) could be predictors of willingness to pay for a self-test. CONCLUSIONS: Self-testing is perceived as an acceptable approach. Health authorities in Peru should facilitate access to this approach to complement healthcare facilities-led testing efforts for COVID-19. Future research is necessary to understand the impact of self-testing in case detection and pandemic control.

BACKGROUND: The COVID-19 pandemic heavily impacted many low- and middle-income countries (LMICs), such as Peru, overwhelming their health systems. Rapid antigen detection self-tests for SARS-CoV-2, the virus that causes COVID-19, have been proposed as a portable, safe, affordable, and easy-to-perform approach to improve early detection and surveillance of SARS-CoV-2 in resource-constrained populations where there are gaps in access to health care. OBJECTIVE: This study aims to explore decision makers' values and attitudes around SARS-CoV-2 self-testing. METHODS: In 2021, we conducted a qualitative study in 2 areas of Peru (urban Lima and rural Valle del Mantaro). Purposive sampling was used to identify representatives of civil society groups (RSCs), health care workers (HCWs), and potential implementers (PIs) to act as informants whose voices would provide a proxy for the public's attitudes around self-testing. RESULTS: In total, 30 informants participated in individual, semistructured interviews (SSIs) and 29 informants participated in 5 focus group discussions (FGDs). Self-tests were considered to represent an approach to increase access to testing that both the rural and urban public in Peru would accept. Results showed that the public would prefer saliva-based self-tests and would prefer to access them in their community pharmacies. In addition, information about how to perform a self-test should be clear for each population subgroup in Peru. The tests should be of high quality and low cost. Health-informed communication strategies must also accompany any introduction of self-testing. CONCLUSIONS: In Peru, decision makers consider that the public would be willing to accept SARS-CoV-2 self-tests if they are accurate, safe to use, easily available, and affordable. Adequate information about the self-tests' features and instructions, as well as about postuse access to counseling and care, must be made available through the Ministry of Health in Peru.

BACKGROUND: Brazil is among the countries in South America where the COVID-19 pandemic has hit the general population hardest. Self-testing for SARS-CoV-2 infection is one of the community-based strategies that could help asymptomatic individuals at-risk of COVID-19, as well as those living in areas that are difficult for health personnel to reach, to know their infectious status and contribute to impeding further transmission of the virus. METHODS: A population-based survey was conducted in November 2021, to assess the acceptability of rapid SARS-CoV-2 antigen self-testing among the population of São Paulo. Survey respondents were approached at more than 400 different street-points that were randomly selected using a five-stage randomization process. A 35-item structured questionnaire was used. Dependent variables for our analyses were the likelihood to use and willingness to pay for self-testing, and the likelihood of taking preventive measures to prevent onward transmission of SARS-CoV-2 following a reactive self-test result. Bivariate and multivariate regression analyses were performed. RESULTS: Overall, 417 respondents (44.12% female) participated; 19.66% had previously had COVID-19 disease. A minority (9.59%) felt at high-risk of COVID-19. The majority of both females and males (73.91% and 60.09%, respectively) were in favor of the idea of SARS-CoV-2 self-testing. Overall, if self-tests were available, almost half of the sample would be very likely (n = 54, 12.95%) or likely (n = 151, 36.21%) to use one if they felt they needed to. Upon receiving a positive self-test result, the majority of respondents would communicate it (88.49%), request facility-based post-test counseling (98.32%), self-isolate (97.60%), and warn their close contacts (96.64%). CONCLUSION: Rapid SARS-CoV-2 antigen self-testing could be an acceptable screening tool in São Paulo. The population would be empowered by having access to a technology that would allow them to test, even if asymptomatic, when traveling, or going to work or school. If there is a surge in the incidence of cases, self-testing could be a good approach for mass case detection by Brazil's already overstretched Unified Health System.

Introducción: La infección por Helicobacter pylori es la causa principal de enfermedades gastroduodenales (gastritis crónica, úlceras pépticas y cáncer gástrico). En Guatemala existen pocos estudios sobre la prevalencia de H. pylori y su relación con enfermedades gastrointestinales, particularmente con cáncer. Objetivos: Identificar la presencia de lesiones premalignas (atrofia gástrica, metaplasia intestinal y displasia) y su relación con la infección por H. pylori en pacientes de consulta externa en unidades de gastroenterología de dos hospitales nacionales de la ciudad de Guatemala. Métodos: El diagnóstico histopatológico y bacteriológico se realizó por medio de las tinciones de H & E y Giemsa, cultivo e identificación bioquímica, detección de anticuerpos específicos mediante la prueba ELISA, diagnóstico molecular por la amplificación del gen glmM y genotipificación por PCR para identificar los genes VacA y CagA. Se analizaron datos clínico-epidemiológicos de los pacientes, la prevalencia de la infección por H. pylori y la genotipificación de la bacteria. Resultados: En 293 de los pacientes estudiados (83 por ciento) se encontró algún tipo de lesión premaligna; las más frecuentes fueron la atrofia gástrica (70 por ciento), metaplasia intestinal (11 por ciento) y displasia gástrica (2 por ciento). El 17 por ciento de los pacientes no presentó lesiones premalignas. Se halló una prevalencia de infección por H. pylori del 58 por ciento, y el gen cagA se detectó en 118 (57 por ciento) de los pacientes infectados. Conclusiones: La mayoría de los pacientes presentó atrofia gástrica (70 por ciento) y el 43,5 por ciento estaba infectado por H. pylori, principalmente con cepas CagA positivo. Introduction: Helicobacter pylori infection is the main cause of gastroduodenal diseases (chronic gastritis, peptic ulcer and gastric cancer). In Guatemala few studies have been carried out on the prevalence of H. pylori and its relationship with gastrointestinal diseases, particularly with cancer. Objective: To identify the presence of premalignant lesions (gastric atrophy, intestinal metaplasia and dysplasia) and their relationship with H. pylori infection in outpatients in gastroenterology units in two national hospitals in Guatemala City. Methods: Histopathological and bacteriological diagnostic testings were performed by H & E and Giemsa stain, culture and biochemical identification, detection of specific antibodies by ELISA, molecular diagnosis by glmM gene amplification, and genotypification by PCR to identify vacA and cagA genes. Clinical and epidemiological data from patients, prevalence of H. pylori infection, and bacterium genotypification were analyzed. Results: Among the studied patients, 293 (83 percent) presented some type of premalignant lesion. The most prevalent were gastric atrophy (70 percent), intestinal metaplasia (11 percent), and gastric dysplasia (2 percent). Seventeen percent of the patients did not have any premalignant lesions. The prevalence of H. pylori infection was 58 percent, and cagA gene was identified in 118 (57 percent) of the infected patients. Conclusions: The majority of the patients presented gastric atrophy (70 percent), and 43.5 percent were infected by H. pylori, mainly with positive cagA strains. This finding confirms the importance of studying H. pylori and its relationship with gastric cancer(AU)

Resumen Antecedentes: la esquizofrenia es una enfermedad crónica que genera gran discapacidad, para la cual se han reportado biomarcadores potenciales, pero sin suficiente validez clínica. El mismatch negativity (MMN) y el P3a son potenciales relacionados con eventos que han demostrado ser indicadores neurofisiológicos del procesamiento auditivo pre-atencional y potenciales biomarcadores. Objetivo: evaluar el MMN y P3a en pacientes con diagnóstico de esquizofrenia y su relación con variables sociodemográficas y clínicas. Método: estudio cuantitativo transversal de 23 sujetos con esquizofrenia (ESQ) y 22 controles sanos (SN). Las amplitudes promedio y latencias del MMN/P3a para la condición infrecuente en duración y frecuencia fueron obtenidas mediante un paradigma oddball auditivo en un EEG de 32 canales. Resultados: se encontraron diferencias para la condición frecuencia en la amplitud del MMN (p=0.046; CI 95% 0.009; 0.87) y la amplitud del P3a (p=0.042; CI 95% 0.025; 1.24) entre los grupos; la amplitud del MMN fue menor en el grupo ESQ (-0.36 DE 0.51 µV) en comparación con los participantes del grupo de SN (-0.81 DE 0.89 µV), mientras que la amplitud del P3a fue menor en el grupo SN (0.18 DE 0.97 µV) versus el grupo ESQ (0.82 DE 1.05 µV). En relación con las variables sociodemográficas y clínicas, las asociaciones con el P3a fueron moderadas y con el MMN débiles. Conclusiones: la reducción de la amplitud del MMN a la condición frecuencia exhibe mayor utilidad que el P3a como medida de alta estabilidad en pacientes con esquizofrenia, lo que reitera su posible uso como biomarcador.
Abstract Background: schizophrenia is a chronic disease that generates great disability, which currently has potential biomarkers but without sufficient clinical validity. Mismatch negativity (MMN) and P3a are event-related potentials that have been shown to be neurophysiological indicators of pre-attentional auditory processing and potential biomarkers. Objective: to evaluate MMN and P3a in patients with a diagnosis of schizophrenia and their relationship with sociodemographic and clinical variables. Method: a quantitative cross-sectional study of 23 subjects with schizophrenia and 22 healthy controls was performed. The average amplitudes and latencies of the MMN/P3a for the condition infrequent in duration and infrequent in frequency were obtained using an auditory oddball paradigm on a 32-channel EEG. Results: differences were found for the frequency condition in the amplitude of the MMN (p=0.046; 95% CI 0.009; 0.87) and the amplitude of the P3a (p=0.042; 95% CI 0.025; 1.24) between the groups; MMN amplitude was lower in schizophrenia (-0.36 SD 0.51 µV) compared to healthy controls (-0.81 SD 0.89 µV), while P3a amplitude was lower in healthy controls (0.18 SD 0.97 µV) versus the group with schizophrenia (0.82 SD 1.05 µV). In regard to sociodemographic and clinical variables, the associations with P3a were moderate, and showed weak MMN. Conclusions: MMN amplitude reduction to the frequency condition exhibits greater utility than P3a as a measure of high stability in schizophrenia, restating its potential use as a biomarker.

Free-roaming dogs are a worldwide problem, with Chile having some of the highest human-to-dog ratios in the world. In 2017, Law 21.020 was promulgated and the federal government developed a national responsible pet ownership program. The objectives of this article are to describe and discuss the dog-related components of the program, to design a tool for determining human-to-dog ratios in Chile, and to make recommendations to managers to improve the program outcomes. The overarching goal of the program was to mitigate the conflict between humans and dogs, but many of the interventions were animal-focused and the indicators did not consider the perception of the Chilean public. Using human density data and known dog populations, we found that as the human density increased, there were fewer dogs per person. Veterinary services and sterilizations were the mainstay of the program and were offered for free to citizens. Education was offered to all ages through public events, as well as municipality and organization activities. The identification of dogs was obligatory for dog owners. Enforcement was not included in the program. The recommendations are to conduct preintervention baseline data collections and to tailor interventions and indicators appropriately; to use dog population size estimates determined at the local level rather than a country-wide estimate; to replace free veterinary services with low-cost sterilization campaigns; to create sustainable plans for education; and to create enforcement teams in communities.

ABSTRACT BACKGROUND: The intensity of the thromboprophylaxis needed as a potential factor for preventing inpatient mortality due to coronavirus disease-19 (COVID-19) remains unclear. OBJECTIVE: To explore the association between anticoagulation intensity and COVID-19 survival. DESIGN AND SETTING: Retrospective observational study in a tertiary-level hospital in Spain. METHODS: Low-molecular-weight heparin (LMWH) status was ascertained based on prescription at admission. To control for immortal time bias, anticoagulant use was analyzed as a time-dependent variable. RESULTS: 690 patients were included (median age, 72 years). LMWH was administered to 615 patients, starting from hospital admission (89.1%). 410 (66.7%) received prophylactic-dose LMWH; 120 (19.5%), therapeutic-dose LMWH; and another 85 (13.8%) who presented respiratory failure, high D-dimer levels (> 3 mg/l) and non-worsening of inflammation markers received prophylaxis of intermediate-dose LMWH. The overall inpatient-mortality rate was 38.5%. The anticoagulant nonuser group presented higher mortality risk than each of the following groups: any LMWH users (HR 2.1; 95% CI: 1.40-3.15); the prophylactic-dose heparin group (HR 2.39; 95% CI, 1.57-3.64); and the users of heparin dose according to biomarkers (HR 6.52; 95% CI, 2.95-14.41). 3.4% of the patients experienced major hemorrhage. 2.8% of the patients developed an episode of thromboembolism. CONCLUSIONS: This observational study showed that LMWH administered at the time of admission was associated with lower mortality among unselected adult COVID-19 inpatients. The magnitude of the benefit may have been greatest for the intermediate-dose subgroup. Randomized controlled trials to assess the benefit of heparin within different therapeutic regimes for COVID-19 patients are required.

BACKGROUND: Health perception is a subjective predictor of long-term morbidity and mortality. Few studies address the perception that pregnant women have of their oral health. OBJECTIVE: The objective of this study was to explore the relationship between socioeconomic factors and self-assessment of oral health in pregnant women from Cali, Colombia. METHOD: A cross-sectional study was carried out with a sample of 998 pregnant women, calculated using the formula to estimate a proportion in finite populations, with a confidence level of 95%. A questionnaire was applied for sociodemographic characterization, as well as to enquire about oral health perception, knowledge, and practices of oral health. RESULTS: The mean age of the surveyed mothers was 24.7, with a standard deviation of 6.1, of which 23.6% were adolescents. The perception they had about their oral health status was considered good by 60.8%. Of the 82.9% who reported having attended dentistry, more than half perceived good oral health. Pregnant women with no history of oral problems, with a perception of medium or high income, and with good oral hygiene practices tend to have a good perception of their oral health. CONCLUSION: Pregnant women with no history of oral problems, with a perception of medium or high income, and with good oral hygiene practices tend to have a good perception of their oral health.
