RESUMEN

Results of organo-saving operations in 807 patients (72% men, 28% women) with duodenal ulcers were analyzed. Bilateral truncal vagotomy (with pyloro-, duodenoplasty, gastroenterostomy, gastroduodenostomy, economical stomach resection) were performed in 27.1% patients, selective--in 1.4%, selective proximal--in 65.5%, anterior selective and posterior truncal vagotomy--in 6.0%. Lethality was 1.6%, long-term results were excellent and good in 75.6% cases. Satisfactory results were seen in 8.6% patients, bad results--in 15.8%.

RESUMEN

Features of regulation of gastric acid production in 82 patients with favorable and unfavorable results of duodenal ulcers (DU) surgical treatment were studied. Current radioimmunoassays were used for assessment of hormonal and acid production function of operated stomach. It is demonstrated that an increase in gastrointestinal and pancreatic hormones secretion in postvagotomic syndromes depends on postoperative disorders degree and terms after gastric surgery that may be regarded as hyperactivity of regulatory systems. It is concluded that primary state of acid production and its regulators may be criteria of outcomes assessment of DU surgical treatment. Timely and targeted correction of these hormonal disorders in the postoperative period is the measure of prophylaxis of many postoperative complications.

RESUMEN

Vagotomy in experiments induces compensatory dilatation of the lymph vessels of the small intestine, which is directed at correction of the microcirculatory shifts encountered in the postoperative period in the form of venous stasis and edema of the wall of the small intestine. Vagotomy leads to slowing down and terminal shift of digestion and exclusion of the duodenal lymph vessels from the process of absorption and transportation of fat. The restoration of the morphology and function of the lymph vessels in the late postoperative period is determined by the type of vagotomy. Microcirculation and the state of the lymphatic system improve more rapidly and fully after selective proximal vagotomy.
