RESUMEN

We performed a detailed analysis of changes in the profiles of osmotic deformability using the method of gradient ektacytometry. Changes in all determinants that form the deformation properties of red blood cells in Wistar rats in the juvenile period and before puberty were determined. The dynamics of the formation of the rheological properties of the blood after birth is characterized by a wave-like change in the studied determinants. The changes are explained by adaptive reactions to extrauterine life as a result of hematopoiesis activation and the transition of the red bone marrow to a new level of functioning with the predominant replacement of physiological reticulocytosis in newborns with mature erythrocytes. The most critical period is from 10 days to 1 month after birth. Starting from the second month, the deformation parameters of erythrocytes are stabilized.

RESUMEN

We studied the effect of different doses of ammonium chloride (ACl) and ammonium carbonate (ACr) on immunological parameters of the peripheral blood in rats during high-intensity exercise. Changes in the absolute and relative numbers of granulocytes, lymphocytes, natural killers, naive and mature effector cells one day after the end of the forced swimming cycle were found by using a hematological analyzer and a flow cytometer. Immunological indicators were analyzed relative to swimming duration on the last day of ultimate load. The revealed changes indicate the onset of the effector phase of the development of the inflammatory processes in the positive control group (physiological saline) and in rats receiving a higher dose of ACr (20 mg/kg), while administration of ACl prevented the development of inflammatory processes and shifts in the physiological balance of lymphocyte subpopulations. Immunological profiling indicates that ACl in a dose of 20 mg/kg most effectively improved physical performance in our forced swimming model.

We compared the effects of two doses of ammonium chloride and ammonium carbonate (10 and 20 mg/kg) on the duration of swimming and blood lactate level. Ammonium chloride in a dose of 20 mg/kg was more efficient than in a dose of 10 mg/kg. The efficiency of ammonium carbonate in a dose of 10 mg/kg was similar to that of ammonium chloride in a dose of 20 mg/kg. Increasing the dose of ammonium carbonate to 20 mg/kg led to a decrease in the duration of swimming. On the last day of the experiment, lactate level in 5 min after exhausting load was maximum in control rats, while in rats treated with 10 mg/kg ammonium carbonate and 20 mg/kg ammonium chloride it was lower by 27 and 33%, respectively. In the control group, the amplitude of the decrease in lactate concentration in 1 h after load was 2-fold greater than in the group receiving ammonium chloride in a dose of 20 mg/kg and 1.6-fold greater that in groups treated with ammonium carbonate in a dose of 10 mg/kg and ammonium chloride in a dose of 20 mg/kg.

Ammonium, an end-product of catabolism, in low doses can promote adaptation of metabolic pathways in erythrocytes under conditions of extreme physical exercise. We compared the effects of two ammonium salts, ammonium chloride and ammonium carbonate, in two doses on biochemical parameters of rat erythrocytes 1 day after extreme physical exercise in a 4-week cycle of forced swimming. Of 16 analyzed parameters, the maximum number of significant shifts from the control was revealed in the groups of rats receiving ammonium chloride in doses of 20 and 10 mg/kg, and the minimal number of differences was found in groups treated with ammonium carbonate in the same doses. The comparison of the levels of reduced glutathione and 2.3-bisphosphoglicerate and activities of 5'-nucleotidase and Ca2+- and Na/K-ATPases attested to more rigorous control of the mechanism of oxygen delivery to tissues by erythrocytes after administration of ammonium chloride in a dose of 20 mg/kg.

RESUMEN

Advanced multiple organ parasitic invasion is reported in the article. A thorough assessment of pathological process, surgical anatomy and preoperative examination resulted radical surgery despite multiple organ disease. Surgical procedure included extended left-sided hemihepatectomy, atypical resection of S6liver segment and pancreas, removal of themediastinal parasite with partial excision of parietal pleura and pericardium.

We studied the role of calcium-regulating structures of slow- (m. soleus, SOL) and fast-twitch (m. extensor digitorum longus, EDL) skeletal muscles of rats during adaptation to exhausting physical activity and the possibility of modulating this adaptation with decaffeinated green tea extract. It was established that EDL adaptation is mainly aimed at Са2+ elimination from the sarcoplasm by Са-ATPase and its retention in the reticulum by calsequestrin. Administration of green tea extract increased endurance due to involvement of slow-twitch muscles whose adaptation is associated with enhanced expression of all the studied genes responsible for the regulation of Ca2+ balance.

Analysed herein are the results of surgical treatment of 55 patients who from June 2013 to August 2014 underwent a total of 56 reconstructive operations on carotid arteries during an acute period of ischaemic stroke. The operations were performed within the terms varying from 24 hours to 18 days after the onset of first symptoms of ischaemic stroke. The neurological status was assesses by the neurologist according to the NIH Stroke Severity Scale (USA) and the Modified Rankin Scale. Prior to operation neurological deficit in 18 patients corresponded to 4 points by the Rankin Scale. Carotid endarterectomy was performed in 55 patients, of these in 24 according to the eversion technique. In 10 patients eversion carotid endarterectomy was performed in combination with ICA resection. One patient was simultaneously subjected to eversion carotid endarterectomy and carotid-subclavian shunting bypass grafting. Carotid endarterectomy with autovenous-patch plasty was performed in 11 patients. Resection of the occluded internal carotid artery with plasty of the external carotid artery was carried out in 7 patients and resection of the internal carotid artery for pathological tortuosity was carried out in 2 patients. At discharge from hospital, regression of neurological symptomatology was noted in 46 (83%) patients. Nine (20%) patients showed complete restoration of neurological deficit, two (4%) patients were found to have increased manifestations of neurological deficit. In the postoperative period two patients died: one on postoperative day 24 from augmenting respiratory and cardiac failure, the other one on day 29 from augmenting renal insufficiency followed by cardiac failure. In both cases, autopsy revealed no signs of secondary impairment of cerebral circulation. Surgical interventions on carotid arteries performed at a multimodality vascular centre during the first 18 days after the development of ischaemic stroke in the majority of cases are accompanied and followed by regression of neurological deficit, being safe concerning the development of haemorrhagic transformation.

The aim of this study was to examine the effect of exhaustive weightlifting exercise on electrical and biochemical variables and performance capacity in young male subjects. The onset of exercise (80-50% 1RM) was associated with a decrease in the amount of work performed, which was followed by a steady performance capacity at 40-10% 1RM. There were no significant changes of m. rectus femoris EMG maximal amplitude though it tended to be increased during the first half of exercise. A significant blood lactate concentration increase indicated that an anaerobic metabolism was a predominant mechanism of muscle contraction energy-supply. CK level in blood plasma did not change but plasma myoglobin concentration doubled immediately post-exercise. The data presented here suggest that decrease in performance capacity was likely due to progressive "refusal of work" of the fast motor units and work prolongation of weaker, intermediate and slow motor units. Unchangeable CK activity and relatively small increase in myoglobin concentration in plasma suggest that used weightlifting exercise did not induced substantial damage in myocytes' membranes in our subjects.

Oral administration of green tea extract in a dose of 6 mg/kg twice a day (before and after exercise) over 2 weeks significantly increased swimming times on week 1 and 2 in comparison with control animals receiving water. The 7-day and final exhaustive running in rats was accompanied by a significant decrease in spleen weight and iron serum levels associated with developed reticulocytosis. Administration of green tea extract in a dose of 12 mg/kg once a day (before exercise) for 2 weeks did not affect the duration of the running, but prevented the decrease in serum iron and spleen weight, that, along with a significantly increased concentration of reduced glutathione in erythrocytes, can indicate a normalizing effect of green tea extract on hemopoiesis and stimulating effect on the antioxidant system of erythrocytes.

Hematological and biochemical parameters were studied in rat blood in the dynamics of immobilization (0.5, 1, and 3 h). Immobilization was followed by a release of old erythrocytes in the circulation and the development of reticulocytosis. The neutrophil count significantly increased by the third hour of immobilization. Restraint stress induced changes in activities of antioxidant enzymes (SOD and catalase) in erythrocytes and plasma level of MDA. The correlation between each enzyme (SOD, glutathione peroxidase, and catalase) and MDA concentration was the highest 1 h after the beginning of immobilization. These data can reflect the tension of antioxidant systems in red blood cells. The biochemical parameters returned to the basal level by the third hour of immobilization. Correlation coefficients between the activities of antioxidant enzymes and MDA amount decreased after 3 h of stress and the correlation between glutathione peroxidase and reduced glutathione significantly increased. The redox status of the blood returned to the initial values after 3 h.

Activity of Na,K-ATPase (Na+,K+-adenosine triphosphatase, EC 3.6.3.9) in the whole erythrocytes was studied in dynamics of the complete rat alimentary starvation for 1, 3, 5, 7-8, and 10-12 days with water drinking ad libitum. There has been established a change of the erythrocyte Na,K-ATPase activity depending on the phase of starvation (the period connected with a certain level of metabolism). After the state on an empty stomach and adaptation to endogenous nutrition (the 0-I phase), from the 3rd to the 7-8th starvation day, the II phase, the period of compensated adaptation occurs (the euglycemia is preserved, the plateau level is preserved, the plateau level is achieved for protein loss and hormonal stimulation). Changes of the Na,K-ATPase activity level within the limits of the II phase were insignificant (p < 0.05), but loses of potassium content in plasma and erythrocytes have been from the 5th starvation day. The III phase (the 12-13th day) is the beginning of the terminal period and is characterized by a decrease of the Na,K-ATPase activity (the oubain-sensitive activity) and of Mg2+-ATPase (the oubain-independent activity), by a decrease of the plasma sodium level (prior to that, this level remained practically unchanged). Ad causes of the revealed decrease of the ATPase activities at the long-term starvation, there are considered aging of population of circulating erythrocytes (the absence of reticulocytes and young erythrocytes), depletion of cell energetic resources (hypoglycemia and glycopenia), effect of endogenous oubain, and endotoxemia.

The run of trained rats until exhaustion affected changes of red-ox balance in red blood cells (RBC) and deteriorated the acid stability of RBC. The oral administration of quercetin, 20 mg/kg, caused expansion of oxidative stress in RBC and a significant decrease in RBC acid stability. Pro-oxidant activity of quercetin could contribute accelerated intravascular hemolysis. It was accompanied by a significant reticulocytosis compared with a control group of rats no fed on antioxidant but exposed to run. Green tea extract (GTE) oral administration, 12 mg/kg, resulted in preventing oxidative stress in RBC, increasing in acid stability of RBC and reducing reticulocytosis. Thus, prooxidant activity of quercetin and the high antioxidant efficacy of GTE are indicative of the advantages of the complex preparations containing several antioxidant compounds. Such complex preparations may counteract an oxidative stress and save mechanical stability of erythrocytes under exhausted run.

RESUMEN

The article deals with outcomes of managing a total of twenty-nine patients of high operational risk with TASK II type С and D lesions of the aortoiliac segment. All patients were subjected to single-stage hybrid operations: open reconstructive interventions on the infrainguinal zone and stenting of iliac arteries. High risk of coronary complications these patients were running was confirmed in accordance with the European guidelines on perioperative assessment of the patients being elected for extracardiac operative treatment. Critical ischaemia was in all cases reliably relieved. Two patients during predilatation developed haemorrhage due to iliac artery rupture which was the cause of changing over to open operation. No lethal outcomes were observed.

In experiments on Wistar rats and guinea pigs, dynamics of blood hematological parameters and of erythrocyte deformability index (DI) was studied from birth to the 3-month age. In rats, significant changes were revealed in the picture of blood for the first month of postnatal ontogenesis. The number of erythrocytes and leukocytes sharply increased, the number of reticulocytes decreased, their ratio in the degree of maturity was changed. The mean erythrocyte volume and their acidic resistance also decreased. Prior to the 4-week age, DI increased, which was accompanied by an increase in the hemoglobin hydration and flattening of the erythrocyte shape. Then ID decreased and erythrocytes acquired the more spherical shape. After the critical period of 3-4 weeks the studied parameters reached plateau. In guinea pigs, at this period of ontogenesis the hematologic parameters had sufficiently stable values, except for a DI decrease from birth to 1.5 months, then it rose without reaching the initial values. Like in the case with rats, DI had a close correlation with the erythrocyte parameter of shape (O min).

The experience of 34 operations of omental implantation to the patients with the critical lower limb ischemia due to the obliterative thrombangiitis was analyzed. 5 patients had the procedure on the both limbs simultaneously. The operative technique had been thoroughly described in the article. The procedure, being accomplished according to the certain standards, proved to be extremely effective.

Presented herein are favourable outcomes of surgical management in six patients suffering from thromboangiitis obliterans (TO) with accompanying critical ischaemia of either the both lower extremities (5 subjects) or the both upper limbs (1 patient). They were all treated by means of a surgical intervention with simultaneous transplantation of the greater omentum on a vascular pedicle onto the both limbs. A total of 12 revascularizing procedures were performed. With an overall of the 12 extremities eventually spared, the remote limb-salvage rate amounted to 100%. A detailed description of the procedural technique is followed by analysing long-term therapeutic outcomes having proved encouraging. The obtained results confirmed a possibility of reliably relieving critical ischaemia inpatients with TO by means of transplanting the greater omentum on a vascular pedicle simultaneously onto the both limbs.

In experiments on Wistar male rats, on a specially constructed computer installation, O2 consumption by the animals in comparison with changes of hematological, biochemical, and rheological blood properties is studied after anemization--acute bloodletting (12-15% of the total blood mass). An increase of the O2 consumption by the organism and tissues by 18-28% has been revealed for the first 7 days after the bloodletting, in spite of a pronounced decrease of hematocrit, and of the amount of erythrocytes and hemoglobin in peripheral blood by 20-25% of the initial level. There was a 5-10-fold increase of the content of immature erythrocyte forms--reticulocytes and a progressive rise of cell acidic resistance, which is characteristic of young erythrocyte forms. An increase of O2 consumption at a decrease of the blood oxygen capacity (a low hemoglobin level) seems to be due to the more efficient transport and yield of O2 to tissues. At the 3rd 7th day after the bloodletting, activity of Na,K-ATPase has been shown to increase by 60% and 20%, respectively. Analysis of the erythrocyte rheological properties has shown that the maximal firmness of aggregates (Uq) and the aggregation rate (1/T) decrease progressively beginning from 3 days after the bloodletting; index of deformability (I(max)) turned out to be elevated by 7-11%, probably due to an increase of the cell membrane elasticity. The conclusion is made that changes of erythrocyte rheological properties are interconnected with changes of the Na,K-ATPase activity and are directed to optimization of blood circulation in large vessels and the capillary network.

The authors are sharing their five-year experience gained at the Department of Vascular Surgery of the Samara State Medical University in performing microsurgical autotransplantation of the greater omentum to a lower extremity in patients presenting with critical ischaemia accompanied by thromboangiitis obliterans. A total of twenty-five operations of microsurgical autotransplantation of the greater omentum on a vascular pedicle were carried out in twenty patients diagnosed with lower limb critical ischaemia accompanied by thromboangiitis obliterans, for five of them underwent surgery simultaneously on the both lower extremities. With an overall number of 22 extremities eventually spared, the remote limb-salvage rate amounted to 88%. The preoperative preparation procedures as well as surgical technique are discussed in detail.

In experiments on Wistar rats, dynamics of blood hematological and rheological parameters has been studied from birth to the 3-month age. Results of the study indicate an intensive activity of the red bone marrow anlagen during the first 3 weeks of postnatal ontogenesis. This is manifested in an increase of the number of cells in the erythron system, a change of erythrocyte acidic resistance, and a shift in the leukocytic formula. The boundary between the 2nd and 3rd postnatal weeks is critical: there occur a sharp deceleration of erythropoiesis and a change of the age-related erythrocyte composition and the blood leukocyte ratio towards those characteristic of adult rats. The rise in the number of leukocytes and erythrocytes does not cease after having passed this period. Until the 4-week age, the deformability index increases, which is accompanied by an increase of hemoglobin hydration and by improvement of the erythrocyte toroid form. Subsequently, inversion of these processes occurs.

In experiments of Wistar male rats, changes are studied of erythrocyte hematological, biochemical (activitities of transport ATPases), and rheological properties (capability for aggregation and deformability) 7 days after bloodletting of 12-15 % of the total blood mass. It has been shown that alongside with an elevation of erythrocyte volume and of the number of immature cells - reticulocytes, there was a statistically significant increase of Na,K-ATPase and Ca-ATPase activities in the whole erythrocytes and their membrane preparations - ghosts, the increment of activity in the case of Na,K-ATPase being essentially higher in the whole cells. This indicates the appearance of an enzyme activator inside the erythrocytes. There are also revealed a decrease of firmness of erythrocyte aggregates, a deceleration of spontaneous aggregation, and an increase of index of erythrocyte deformability. The conclusion is made that changes of erythrocyte rheological properties are interconnected with changes of the Na,K-ATPase activity and are directed to optimization of blood circulation in large vessels and capillary network.
