RESUMEN

The paper compares the apparent blood viscosity (BV)-shift rate (SR) curves, by using uni- and multidimensional dispersion analysis. The study has indicated that when admitted to hospital, patients with maxillofacial phlegmons have higher BV due to worse blood rheological and coagulative properties. An analysis of the BV-SR curves has shown that there are the most significant differences between the parameters at moderate and high SR while the differences are frequently insignificant at low SR (on admission, on days 3 and 10). For hemocorrection of increased BV, co-administration of disaggregatory agents is indicated during the treatments proposed. Uni- and multidimensional analyses used to examine the curves of the apparent BV to the individual value of SR and throughout its range are the most informative tools for assessing the quality of treatment in patients with maxillofacial phlegmons and a statistically warranted method for multiple comparisons.

RESUMEN

Possibilities of modern methods of maths statistics for comparative evaluation of efficacy of different methods of treatment of patients with suppurative-inflammatory diseases of maxillo-facial region were analyzed and an integral index of treatment efficacy was suggested. Application of the integral index of treatment efficacy showed that intravenous injection of sodium hypochlorite and autohemotherapy with oxidized blood led to the closest apposition of cluster center of these patients to the cluster center of healthy persons. It testifies to greater efficacy of such treatment of the disease in comparison to the other methods.

Clinical and immunological studies were carried out in patients with maxillofacial phlegmons treated by traditional methods and by traditional therapy supplemented by immunocorrection (leukinferon). Addition of leukinferon to basic therapy had a pronounced clinical and immunomodulating effect, which resulted in shorter hospitalization period and virtually complete recovery of the main immunological parameters, in contrast to traditional therapy alone. The most demonstrative parameters were the expression of the neutrophil granulocyte membrane receptors and the relationships between T-lymphocyte subpopulations, which prompted using them as the most significant diagnostic markers indicating the disease severity and treatment efficiency.

Twenty-five patients with pyoinflammatory maxillofacial diseases treated in an inpatient setting in the Regional Dentistry Clinic of Krasnodar and 8 healthy volunteers were examined. After opening and draining of the purulent focus the patients were divided into 2 groups. Group 1 consisted of 21 patients treated by intravenous 0.04% sodium hypochlorite (SHC) and intravenous infusions of SHC-oxidized autoblood for 2-3 days. Group 2 (6 pts) was treated by intravenous infusions of SHC-oxidized autoblood for 6 days. The criteria of intoxication were erythrocyte resistance tests in hypotonic sodium chloride and at different concentrations of urea in isotonic medium. A new method for evaluating erythrogram deviations from reference values is proposed, which helps adequately assess the severity of intoxication.

RESUMEN

Morphological reactions in tissue adjacent to mandibular angular fracture were studied in guinea pigs treated with sodium succinate and laser magnetotherapy. Due to succinate therapy the exudative component of inflammation was less expressed in comparison with the control, macrophagal reaction and neoangiogenesis were activated, the volume of damaged muscle tissue and the incidence of suppurations decreased. The number of osteoblasts increased and new bone structures acquired a lamellar pattern earlier than in the control. Sodium succinate therapy in combination with laser magnetotherapy had a more pronounced positive effect as regards activation of macrophagal reaction and neoangiogenesis and a decrease in the area of fibrosclerotic changes in the zone of damaged muscles, where newly formed myosymplasts differentiated into myotubes and even in muscle fibers. Suppuration of the wound was prevented. Bone tissue in the fracture zone formed without preliminary formation of cartilaginous tissue, which resulted in more rapid osteogenesis (lamellar bone growth in the fracture zone).

RESUMEN

The proposed method of surgical treatment of maxillary micro- and retrognathia radically differs from the known methods: osteotomy of the osseous base of the hard palate is carried out parallel to the vomer to reach the level of the first premolars by a median dissection of the mucosa and periosteum, and vertical osteotomy of the anterior portion of the vomer is carried out. Horizontal osteotomy is performed from the vestibule of the mouth. Subsequent osteotomy of the anterior and lateral surfaces of the upper jaws under mucosal periosteal "tunnels" and detachment of the maxillary tuberosities from the coracoid processes permit a full-scale displacement of the bone fragment forward to attain the position planned for it. The method was clinically used in 7 patients with good functional and esthetic results.

RESUMEN

A new method for surgical treatment of upper micrognathia is offered. Osteotomy of only the anterior segment of nasal septum is carried out to reduce blood loss, make the operation less traumatic and circulatory disturbances less severe in the mobilized fragment of the maxilla. Osteotomy of tubercles and anterior surfaces of the maxilla is carried out subperiosteally. Use of this method in 12 patients with upper micrognathia after cheilo- and uranoplasty helped attain good esthetic and anatomicofunctional results.
