RESUMEN
These clinical trials show the efficiency of therapy with angiotensin-converting enzyme (ACE) inhibitors in essential hypertension (EH), chronic heart failure and coronary heart disease (CHD). The SMILE studies have indicated positive results of the early administration (<24 hours) of zofenopril in patients with acute myocardial infarction (AMI). These results have been confirmed in high-risk patients, including those with hypertension and diabetes mellitus, which enables zofenopril to be recommended for clinical use in patients with EH and CHD. Zofenopril has a better safety profile than other agents.

Significant role of immune inflammatory reactions along with activation of the RAAS (rennin-angiotensin-aldosterone system) and the sympatho-adrenal system in CHF (chronic heart failure) pathogenesis is considered in the present survey article. Together with haemodynamic disorders activation of inflammatory reactions, which involve anti-inflammatory cytokines, is shown to be one of the main factors affecting CHF course and prognosis. Alteration of cytokine level is associated with injury level, gravity of the disease course and its clinical outcome. Prognostic value of determination of inflammation mediators as predictors of cardiovascular diseases and clinical outcomes is evaluated. Urgency of search for new medication in addition to "traditional preparations" used for the treatment of CHF is shown.

The aim of the study was to estimate the therapeutic efficiency of complex: ACE inhibitor (perindopril) and traditional antiarrhythmic treatment of atrial fibrillation paroxysms after successful electric cardio version for prevention of repeated attacks of atrial fibrillation and for maintenance of normal sinus rhythm in patients. The proposed scheme of treatment reduced number of recurrences, maintained normal sinus rhythm and allowed reaching relatively rapid normalization of mechanical function of left atrium.

The aim of the article was to assess the efficacy of treatment of atrial fibrillation (AE) and congestive heart failure (CHF) with Enalapril in combination with Cordaron. For this reason 62 patients from 37 to 70 years old (mean age 54, 6 +/- 6,1) with CHF (class II-III NYHA) and persistent AF underwent the medical treatment. The suggested scheme of treatment improved HF functional class by NYHA, raised anti reactive effectiveness and favorably influenced on the diastolic function of the LV in this type of patients.

Atrial fibrillation (AF) is one of the most frequently seen arrhythmia. Optimal strategy of its treatment still remains a serious problem. The goal of the investigation was the evaluation of the efficacy of combined treatment of AF with Cordaron and ACE inhibitor Diroton (lizinopril) (D) in patients with chronic heart failure (CHF). 29 patients (32-78 years old) with persistive AF and CHF (II - III class NYHA) were under observation. The treatment with C 200 - 300 mg/day and D 5 -10mg/day for 12 weeks resulted in improvement of clinical status of the patients, improvement in functional class of CHF, decrease in frequency of recurrences of AF in comparison with patients who were not given ACE inhibitor. Ultrasound study showed that combined therapy increases myocardial contractility corrects left ventricular remodeling, maintains the sinus rhythm and improve prognoses in this category of the patients. The use of ACE inhibitior D in antiarrhythmic (antirecurrent) therapy together with C is optimal in patients with persistive AF CHF.
