RESUMEN

Antibodies (Ab) reacting with the myocardial sarcolemma (SL) and interstitial connective tissue (ICT) were determined in 1333 rheumatic disease (RD) patients and in 286 healthy subjects of various age using indirect immunofluorescence technique. The age-related levels of SL and ICT Ab are responsible for the heterogeneous immune response to SL and ICT antigens. The presence of SL Ab is characteristic for young patients, of ICT Ab, for older patients. Children are found to have ICT Ab more infrequently, Sl Ab are more common in rheumatism than in other rheumatic diseases. The higher inflammatory process activity is associated with a more frequent presence of SL Ab and rare ICT Ab in each age group. In RD patients, SL Ab are considered to be risk factor of cardiac disease development. The ICT antigen is found to incorporate alpha-galactose. The reasons for the absence of ICT Ab in the healthy young and in patients with maximum inflammation are discussed.

Asymmetric affection of the major lower limb joints is a characteristic feature of the joint syndrome in yersiniosis-associated arthritis. The sacroiliac articulations are frequently (47% cases) involved. In addition, yersiniosis-associated arthritis concurs with the signs and symptoms of systemic disease--gastroenterocolitis, myocardiopathy and myocarditis, erythema nodosum, hepatitis, urethritis, conjunctivitis, myositis and myalgia, enteropathy; changes in the CNS typical for the astheno-neurotic syndrome are frequently present. Comparison of the immunological assay data in complicated and uncomplicated yersiniosis shows equally high levels of IgG and CIC. High anti-DNA antibody titres are more frequently found in the serum of uncomplicated yersiniosis patients. ELISA quantitation of specific IgA, IgM, and IgG class antibodies in yersiniosis-associated arthritis patients demonstrated persistence of all the three antibody classes or of IgA-IgG combination in cases with most severe of the joint syndrome. In the presence of cardiac disease, patients were found to have high titres of antibodies reactive with the cardiac interstitial tissue, while in authentically diagnosed myocarditis cases with the sarcolemma. The investigation findings strongly suggest a high degree of involvement of immune and autoimmune processes in the pathogenesis of arthritides secondary to Yersinia infection.

Antibodies (Ab) reacting with myocardial fibre sarcolemma (SL) and interstitial connective tissue (ICT) were determined by means of indirect immunofluorescence technique. The Ab level was found to be dependent on the ABO. Subjects having O and A blood groups are most potent ICT Ab producers, while those with B group, the weakest. The inflammatory process activity exerts varying effects on the level of ICT and SL antibodies depending on the ABO. The ICT Ab level reduction and SL Ab level elevation in maximal inflammation activity were revealed only in O and A blood groups, but not in B-group patients. The differences are discussed. The use of ICT Ab level determination is proposed to be an indicator of the utilization system of damaged connective tissue and aging erythrocytes according to the age and ABO. The presence of SL Ab is believed to be a risk factor of cardiac disease only in O and A blood group patients.
