RESUMEN

Introduction: : Dental adhesion is responsible for the chemical processes of dentin-enamel union for a correct choice before its application in restorative treatments; it is important to know properly the generation to which it belongs, its properties, application techniques, etc. All this is to ensure success in the procedures to be performed. Currently, adhesive systems have evolved, and 8 generations are available in the market, each with a better-quality standard than the previous one. Objective: To identify which generation of the adhesive system is the most viable to be applied in direct dental restorative treatments. Materials and methods: We reviewed and compiled 133 articles published from 20 years ago to date. After analyzing them according to their clinical relevance, 88 were excluded, the remaining articles were reanalyzed, of which only those focused on describing the generations of adhesive systems applied in direct restorations were selected, leaving a total of 56 articles, which included clinical case reports. The Google Scholar search engine, SciELO and databases of interest, such as PubMed, Scopus and Medigraphic, were used, three reviewers independently performed the analysis and search of data, and the keywords were used; adhesive systems, generations of adhesive systems, evolution, and direct restorations. Results: : 56 articles were evaluated, according to their relationship with the subject and with the information previously sought, it was determined that the seventh generation of adhesive systems is the most viable to be applied in dental treatments, due to the improvements it presents in its characteristics, and the results with good prognosis mentioned by both dental professionals, as in research. Conclusion: The advancement of technology implies new knowledge; therefore, the effectiveness of adhesive systems has changed significantly. Scientific evidence has shown that seventh-generation adhesives are more effective in direct restorations.

RESUMEN Introducción: Buscando satisfacer la demanda con procedimientos adhesivos más rápidos, menos sensibles a la técnica y facilidad de uso, se han desarrollado los adhesivos universales, que brindan al profesional la opción de seleccionar la estrategia de adhesión con diferentes modos de grabado, ya sea con grabado y enjuague, autograbado o grabado selectivo. Debido a la poca información existente, se han generado dudas sobre el rendimiento, propiedades y la aplicación de una técnica adhesiva adecuada sobre los tejidos dentales. Objetivo: Determinar el nivel de adhesión con el uso de adhesivos universales en modo de grabado y enjuague y autograbado. Métodos: Se realizó una revisión de la literatura, a través de la búsqueda electrónica en las bases de datos: SciELO, Scopus, PubMed, Springer y EBSCO. Inicialmente se seleccionaron por título y resumen, artículos duplicados se consideraron solo una vez, se incluyeron artículos de acceso libre y pagos, escritos en idioma español e inglés, publicados desde el año 2015, de los cuales 39 artículos fueron seleccionados para el estudio. Conclusiones: El uso de adhesivos universales sobre dentina presenta un nivel de adhesión mayor cuando se aplica en modo de autograbado. La aplicación activa del adhesivo universal en esmalte mejora la fuerza y resistencia de unión. El nivel de adhesión en el esmalte es mayor cuando se aplica en modo de grabado y enjuague. La resistencia inicial y por fatiga del adhesivo, no mejora con los tiempos prolongados del grabado ácido.


ABSTRACT Introduction: universal adhesives have been developed to satisfy the demand with faster adhesive procedures, less sensitive to the technique and ease of use, which offer the professional the option of selecting the adhesion strategy with different etching modes, either with etch-and-rinse, self-etch or selective etching ones. Doubts have been generated about the performance, properties and application of an adequate adhesive technique on dental tissues due to the little existing information. Objective: to determine the level of adhesion with the use of universal adhesives in etch-and-rinse and self-etch modes Methods: an electronic search of the literature was carried out in SciELO, Scopus, PubMed, Springer and EBSCO databases. Initially, articles were selected by title and abstract, duplicate ones were considered only once, free and paid access articles, written in Spanish and English and published since 2015 were included; 39 of them were selected for the study. Conclusions: the use of universal adhesives on dentin presents a higher level of adhesion when applied in self-etch mode. The active application of the universal adhesive improves the enamel bond strength. The level of adhesion on the enamel is higher when applied in etch-and-rinse. The initial fatigue resistance of the adhesive does not improve with prolonged acid etching times.

RESUMEN El presente reporte tuvo como objetivo presentar la planificación y ejecución de un tratamiento rehabilitador con el empleo de la técnica de lentes de contacto dentales para la resolución estética de la forma y tamaño de las piezas dentales. Se reportó el caso de un paciente de sexo masculino de 20 años de edad que presentaba inconformidad con su sonrisa por la presencia de diastema en los incisivos centrales de la arcada superior; por lo que se efectuó un tratamiento estético con el empleo de lentes de contacto dentales de disilicato de litio, en el cual se realizó un desgaste mínimo de la superficie dental para garantizar una correcta inserción y adaptación de los laminados de cerámica. Al año del seguimiento no se detectaron infiltraciones, sensibilidades o fracturas en los dientes rehabilitados con esta técnica. Entre las ventajas de lentes de contacto dentales o lumineers, como también es conocida, presenta una máxima conservación de la estructura dental, además, de altos niveles de estética por su gran translucidez y brillo similar a la de los dientes naturales.


ABSTRACT The objective of this report was to present the planning and execution of a rehabilitative treatment with the use of the dental contact lens technique for the aesthetic resolution of the shape and size of the teeth. The case is a 20-year-old male patient who presented dissatisfaction with his smile due to the presence of diastema in the central incisors of the upper arch. An aesthetic treatment was carried out with the use of lithium disilicate dental contact lenses. Minimal wear of the dental surface was implemented, to guarantee a correct insertion and adaptation of the ceramic laminates. After one year of follow-up, no infiltrations, sensitivities or fractures were detected in the teeth rehabilitated with this technique. Among the advantages of dental contact lenses or lumineers, as it is also known, is that it presents a maximum preservation of the dental structure, as well as high levels of aesthetics due to its great translucency and brightness similar to that of natural teeth.


RESUMO O objetivo deste relatório foi apresentar o planejamento e execução de um tratamento reabilitador com a utilização da técnica de lentes de contato odontológicas para a resolução estética da forma e do tamanho dos dentes. Foi relatado o caso de um paciente do sexo masculino, 20 anos, que apresentava insatisfação com o sorriso devido à presença de diastemas em incisivos centrais da arcada superior; Para tanto, foi realizado um tratamento estético com a utilização de lentes de contato odontológicas de dissilicato de lítio, no qual foi realizado um desgaste mínimo da superfície dentária para garantir a correta inserção e adaptação dos laminados cerâmicos. Em um ano de acompanhamento, não foram detectadas infiltrações, sensibilidades ou fraturas nos dentes reabilitados com esta técnica. Dentre as vantagens das lentes de contato ou luminárias odontológicas, como também é conhecido, apresenta uma preservação máxima da estrutura dentária, bem como elevados níveis de estética devido a sua grande translucidez e brilho semelhante ao dos dentes naturais.
