RESUMEN

The epidemiological autopsy study of 1338 men in Riga aged 20-59 was repeated 20-25 years after the first similar investigation. Aortas were stained with Sudan IV and atherosclerotic lesions were graded by visual-planimetric method. It was the significant growth, with age, of all kinds of lesions except lipoid streaks, having the most prominent increase in men aged 40-49. In the abdominal aorta the extent of lesions was significantly higher than in the thoracic aorta beginning from the fourth decade of life. Significant increase of various lesions in aorta was observed in the same male population 20-25 years after the first examination. The tendency to the earlier transition of fibrous plaques into more severe forms of atherosclerosis (complicated lesions and calcinosis) is noted.

RESUMEN

The evaluation of atherosclerosis of 310 sets of arteries stained with sudan IV was done visually by the planimetric method using the WHO criteria. Based on the evaluation in each case of 50 arterial segments, standardized values were calculated for the 9 main basins which permitted to determine the place of each vascular area in the atherosclerotic process and to reveal its characteristic features in different basins. The development of atherosclerosis in the human arterial system was of systemic nature as indicated by significant positive correlations between various basins. The severity of the involvement of one vascular area, however, was not an adequate indication of the severity of involvement of the other systems. Since the area of the involvement did not always reflect the severity of the process, it is suggested that the severity of atherosclerosis be evaluated by strength of correlation between the area of raised lesions and severe stenoses and occlusions.

Visual-planimetric evaluation of atherosclerosis of the aorta, subclavian, iliac, and femoral arteries in men of 50--64 who had died in Riga and Samarkand was carried out. The values characterizing the extent and severity of atherosclerosis of these arteries in the indigenous and non-indigenous population of these cities were calculated. Differences in the morphological features and course of atherosclerosis of arteries of the extremities in relation to the place of residence were established. No significant differences in the development of atherosclerotic changes of arteries of the extremities between indigenous and non-indigenous residents of Riga were found. The indigenous residents of Samarkand had low values of the area of atherosclerotic changes of the blood vessels of the extremities as well as low frequency of stenoses and obliterating lesions as compared with the non-indigenous residents. The observed differences indicate the importance of climatic-geographical and genetic factors and national-environmental features of the way of life in the pathogenesis of atherosclerotic diseases of the extremities.

In accordance with the WHO programme, the peculiarities of age dynamics of atherosclerosis of the aorta and of the peripheral arteries of the extremities were studied in male residents of Riga, aged 50-64 years. By way of the visual-planimetric method an assessment was made of 310 sets of aorta, common iliac, external iliac, femoral, innominate, subclavian, axillary and brachial arteries stained with Sudan IV. The age dynamics of atherosclerosis in all the studied arteries was represented by growing lesions with a simultaneous slight reduction of the lipoidosis area. The progress of atherosclerosis was irregular with the deceleration of the augmentation tempo in the beginning of the 7th decade of life. The atherosclerotic lesions of the arteries of the upper extremities were characterized by a considerable area of changes represented by flat fibrous plaques, and therefore by rare severe stenoses and absence of occlusions. The highest incidence and area of complicated lesions is found in the aorta. In the arteries of the lower extremities the area of atherocalcinosis was greater, and the incidence of stenoses and occlusions was high. Medium degree correlation was established between the indices of atherosclerosis prevalence in the aorta and the arteries of the extremities. The area of aortic lesions, however does not permit to judge the incidence of stenotic and occlusive lesions of the arteries of the extremities.

Random studies of the base of the brain vessels in 278 men who had died at the age of 50--64 years were carried out; 29 variants of the structure of the circle of Willis were established; the most common were threadlike connective arteries and posterior trifurcations of the inner carotid arteries. The investigation of atherosclerotic changes in every vessel of the base of the brain showed that in the so-called posterior trifurcations there was a statistically reliable growth of the average degree of atherosclerosis in the intracranial region of the inner carotid artery ensuring in these cases the blood supply for a considerably greater region of the brain. In posterior connective arteries atherosclerotic changes were revealed only when their diametre exceeded 0.1 cm which testified to the bloodflow in these arteries.
