RESUMEN

Addition of intrauterine lavage with cooled furacilin solution (0-4 degrees C) to a complex of treatment of 87 patients with postpartum endometritis resulted in a sooner recovery as against that in 61 patients administered traditional treatment alone; such a therapeutic course required lesser amounts of drugs, too. To specify endometritis severity and monitor the treatment efficacy, metro-aspirated specimen cytology was examined. Preventive lavage of the uterine cavity postpartum in cases with a 'dry' period longer than 24 h and other factors of risk of infectious complications proved to be highly effective.

RESUMEN

To increase the effect of the treatment of puerperal endometritis, long-term washing of the uterus cavity with cool solutions of furacillin was used in 57 puerpera. The most favourable results were observed in 27 patients who at the final stage of the washings were treated with antibiotic solutions administered to the uterus cavity. The recovery terms, as well as the terms of the hospitalization lowered. For confirming the diagnosis of endometritis and control of the therapy efficacy cytograms of the metroaspirates were used. High efficacy of the prophylactic administration of antibiotics to the myometrium tissues during the cesarean section with high risk of infectious complications was also shown. Both the methods were recommended for their wide use in obstetric hospitals.

RESUMEN

A study of 72 cervical biopsy specimens from puerperants revealed several pathogenetic features of endocervicitis, glandular ectropions, and authentic erosions. In particular, pseudoerosions were the result of excessive cervical gland hyperplasia that combined, in nearly all cases, with decidual transformation and chronic stromal inflammation. Ectropions were most likely to have been due to inversion of a moderately hyperplastic mucosa during delivery. In contrast, true erosions developed where the stratified squamous epithelium was hyperkeratotic and atrophic, and were often complicated by acute cervicitis.

A possibility of early cytomorphologic diagnosis of postnatal endometritis is shown on 57 observations: the number of lymphocytes and granulocytes and their ratio was determined from the scrapings of the placental basal lamina.

PIP: Ordinarily the IUD does not give rise to any unpleasant effects on the woman's organism. But the literature does contain references to peritonitis, sepsis, and even perforation of the uterus. The present case describes an IUD which passed through the uterine tube into the abdominal cavity. The patient, a 28-year old woman, had been fitted with an IUD and a month later came to the clinic with a serious case of nonspecific pneumonia. Soon after she died of cardio-pulmonary insufficiency. At autopsy, in the left uterine tube (in the ampulla) we discovered the IUD threads, while the IUD itself was hanging in the abdominal cavity. We could not find any macro- or micro-pathological changes in the uterus. The reasons might have been connected with the fact that the IUD was inserted 1 month after the woman had a stillbirth. The basic mechanism would be that the IUD was wedged into the isthmus of the uterine tube at the time of insertion and then compressed, thus facilitating its subsequent movement toward the abdominal cavity.^ieng
