RESUMEN

Duplex ultrasonic scanning was used to examine blood supply to the ciliary body. The study helped quantitatively assess the ciliary body hemodynamics. The hemodynamic parameters in diseases of the anterior segment of the eye (cataracts, glaucoma, corneal dystrophy, etc.) significantly differed from the norm: pulsative index 2.32 vs. the normal value of 1.57, systolic/diastolic ratio 7.29 vs. 6.23, the peak systolic rate 12.59 cm/sec vs. 34.36 cm/sec, maximal rate 6.56 cm/sec vs. 14.36 cm/sec, mean rate 4.0 cm/sec vs. 8.76 cm/ces, volumic rate 1.47 ml/min vs. 3.66 ml/min. The examinations helped detect early disorders in the blood supply to the ciliary body. New ultrasonic equipment: Quantascope (Vital Science, England) and DRF-1000 (Piasonics, USA) was used, that proved highly informative and reliable.

RESUMEN

For investigation of the state of optic nerve blood supply a duplex ultrasound scanning was used. It is known that such equipment begins to gain popularity in general diagnostics. A special method for investigation of the state of optic nerve blood supply (both of the disc and its orbital part) was worked out and allowed to receive 7 hemodynamic parameters characterizing the state of hemodynamics in norm and in optic nerve atrophy or its suspicion. Thus, a pulsatile index being 1.57 +/- 0.21 in norm, increased to 2.11 +/- 0.13 in atrophy. Systolic-diastolic ratio in norm 5.01 +/- 0.21 increased to 7.25 +/- 0.13 in atrophy. The peak systolic rate of blood supply in norm 32.7 +/- 1.8 cm/c decreased to moderate indices 16.9 +/- 1.3 cm/c. Maximum and moderate rates were also decreased twice as compared with norm. Volume rate of blood flow being 5.02 ml/min. in norm, decreased to 2.05 +/- ml/min. in atrophy. The data obtained are statistically authentic. Thus, duplex equipment can be used for investigation of the state of hemodynamics in short posterior ciliary arteries that nourish the optic nerve.

Thirty-five patients suffering from atherosclerosis with carotid artery stenosis of varying severity have been examined. The thickness of the choroid has been paramacularly measured by the ultrasonic methods. The studies have revealed a significant decrease of the choroid thickness in both eyes of patients with bilateral hemodynamically significant stenoses of the carotid arteries and with bilateral stenoses on the point of occlusion. Of the 12 patients with hemodynamically insignificant stenoses, the choroid thickness has reduced only in 3. Surgical treatment (vascular restoration operations on the carotid arteries) has resulted in recovery of the choroid thickness in all the patients. This has lead the authors to a conclusion that isolated measurements of the choroid thickness in patients with impaired arterial extracranial blood stream cannot serve the diagnostic ophthalmologic test for the detection of patients with carotid insufficiency, but such measurements carried out by ultrasonic techniques, combined with other functional ophthalmologic methods, give valuable information on the time course of changes in the eyeball blood content.

RESUMEN

Results of superficial temporal artery dissection performed in 42 patients (44 eyes) with preglaucoma and initial open-angle glaucoma have shown no impairement of visual field, visual acuity, optic disc in the period from 0.5 to 1.5 years after it. The increased linear rate of bloodflow after the operation remained stable in 17 eyes of patients with preglaucoma and in 14 eyes of patients with glaucoma. Intraocular pressure became normalized in 20 and 19 eyes as well as intraocular fluid outflow in 14 and 13 eyes, respectively. In both groups there was a tendency to normalization of retinal functions and hydrodynamics of the eye. Thus, the operation produces improvement of regional bloodflow, tonographic and electrophysiologic indices. Its usage is considered to be reasonable, but, when determining indications to it, an individual approach is necessary.
