RESUMEN

AIM: Determination of the degree of phylogenetic relationship of Yersinia pestis strains iso- lated from the territories of natural foci of plague from the Caucasus using VNTR-typing by 25 loci (MLVA25). MATERIALS AND METHODS: 26 strains of Y pestis from Russian natural foci of the Caucasus were used in the study. 25 loci of tandem repeats in Y pestis genome by Le Fleche scheme were used for execution of multi-locus VNTR-analysis. Deciphering of nucleotide sequences was carried out in automatic sequencer ABI 3130 Genetic Analyser. Analysis of confinement of clusters to certain territories, objects and time of isolation of strains was carried out. using Arc GIS 10.1 program. RESULTS: Groups of MLVA25-types of various levels of discrimination were formed: clusters, groups and subgroups. Clusters were formed by strains ofvarious taxonomic membership: main and subspecies of Y pestis. Subgroups reflect membership of strains in certain foci, and MLVA25-types - the degree of genetic relationship. CONCLUSION: Genetic <> of plague causative agents obtained using MLVA25-types circulating in various natural-focal territories allow to solve problems ofboth theoretical and practical character: from interpretation of microevolution processes to the search of the source of infection and ways of its spread during possible epidemic complications.

RESUMEN

AIM: Conducting epizootological and epidemiological zoning of the territory ofNorth Caucasus -Federal Region (NCFR) by the degree of non-welfare by anthrax using Arc GIS10 program. MATERIALS AND METHODS: 2 parameters were used during zoning ofthe territories of subjects of NCFR (Stavropol Region, Karachaevo-Cherkessia, Kabardino-Balkaria, Chechen, North Ossetia-Alania, Ingushetia, Dagestan Republics) by non-welfare by anthrax: integrated parameter of infection risk and epizootics risk. The parameters were input into the Arc GIS 10 program. Materials on infection of humans and animals with anthrax for the last 55 (1960 - 2014) years were used. RESULTS: The regions of the territories of NCFR subjects were distributed by non-welfare degree into 4 groups: group 1 - regions with a low degree of non-welfare, group 2 - regions with an average degree of non-welfare, group 3 - regions with a high degree of non-welfare, group 4 - regions with a very high degree-of non-welfare. CONCLUSION: Taking into account high parameters of spread and inten- sity of epizootological and epidemiological situation, regions of groups 3 and 4 could be classified as endemic territories for anthrax, whereas all the other regions could be attributed to the zone of sporadic infection manifestation.

RESUMEN

The retrospective and operative epidemiological analysis of leptospirosis morbidity on the administrative territories of the Southern Federal District during the period of 1990-2002 was carried out. The dynamics and tendencies of morbidity in individual administrative units of the region were shown. Materials on the negative influence of the emergency situation arising in the region as the result of the natural calamity in summer 2002 were presented. As shown in this work, timely decisions on the stabilization of epidemiological situation taken at the period of high flood and inundation in summer 2002 in the Southern Federal District and their efficient execution made it possible to keep the epidemiological situation in leptospirosis under control.

RESUMEN

The data on leptospirosis morbidity in the Krasnodar Territory, one of the most epidemiologically unsafe areas in this infectious disease in the Russian Federation, are presented. High morbidity registered in this territory greatly depends on the epidemic outbreaks appearing due to defects in the realization of a complex of sanitary and veterinary measures. The Krasnodar Territory is liable to be affected by natural calamities. In 1997 a severe outbreak of leptospirosis took place here in connection with the high flood. Due to timely prophylactic measures the epidemiological consequencies of the similar calamity occurring in 2002 were brought to a minimum.

RESUMEN

Information on the epizootic situation in plague in the natural foci of North Caucasus and on the influence of a number of anthropogenic and natural factors on this situation is presented. The data given in this work indicate that under the conditions of the anthropogenic transformation of landscapes the character of the epizootic manifestations of plague is changed and new factors, capable of aggravating epidemiological situation, appear. In addition, some other factors must be considered, such as the insufficient financing of reliable field surveys at present, the impossibility of making reliable epizootological studies due to causes of the social character (armed conflicts), thus making it impossible to evaluate, with a sufficient degree of reliability, the real epizootic state of a number of territories and, therefore, the risk of human infection. In this connection the necessity to carefully plan prophylactic measures and measures aimed at the localization and liquidation of the probable foci of infection arises.

RESUMEN

Materials dealing with the specific features of the territory of Southern Russia, appearing as a consequence of natural, technogenic and some other factors, are presented. These materials indicate that, due to the high flood in summer of 2002, a real danger of a sharp aggravation of the epidemiological situation in acute enteric infections, as well as other infections, arose on a vast territory. As the result of the timely evaluation and analysis of changes in the factors of the natural and social environment, occurring as the consequence of the high flood, prophylactic measures were urgently carried out, thus making it possible to prevent the aggravation of the epidemiological situation.

RESUMEN

The data on the morbidity in infectious diseases in the Chechen Republic in recent years are presented. As revealed from these data, under the conditions of the crisis situation the tendency towards the deterioration of the epidemiological situation began to show from the beginning of the 1990s. The growth of morbidity in some infections was registered, the outbreaks of dangerous infections (cholera, poliomyelitis, diphtheria, typhoid fever) appeared. The armed conflicts led to the disintegration of the sphere of social services of the republic and were accompanied by the destruction of the systems of the life support of the population, the deterioration of the quality of antiepidemic measures and by the practically complete incapacity of the local sanitary and epidemiological service to ensure the realization of measures for epidemiological surveillance, as well as measures for the control of infections and their prophylaxis.

RESUMEN

The epidemiological situation in tuberculosis in the Republic of Ingushetia is evaluated. The unsatisfactory sanitary and hygienic state of camps for migrants (persons, forced to quit the place of their permanent residence), has been found to create favorable conditions for the spread of tuberculosis. Recommendations on the improvement of the organization and carrying out of measures for controlling tuberculosis among both temporarily displaced persons and the local population, as well as on the improvement of the material base of the laboratory diagnostics of tuberculosis, are proposed.

RESUMEN

Information on the recent sanitary and epidemiological situation in the Republic of Ingushetia is presented. In some years an almost twofold growth in the number of population was registered in the republic due to the mass influx of temporarily displaced persons. Such factors as excessive population density, the absence of conditions for the observation of elementary sanitary and hygienic norms, malnutrition, complications with the supply of drinking water meeting sanitary norms contribute to the possibility of epidemic complications. The data on the state of water supply and sewage in populated localities and on the state of morbidity in infectious diseases are presented. By the year 2000 the aggravation of the epidemiological situation in viral hepatitis A and tuberculosis took place. Morbidity in acute enteric infections somewhat increased. There is the danger of the appearance of some infections from the natural foci.

RESUMEN

The results of the epidemiological investigation of the outbreak of typhoid fever in Achkhoi-Martan region are presented. During the period from December 1999 to April 2000 the disease was diagnosed in 43 persons. The cases of the disease were registered in 3 settlements: Lermontov-Yurt, Kulary and Sernovodsk. The pronounced focal character of the disease was noted with the prevalence of family foci with two or more cases of the disease. This conclusion was based on the fact that at the initial stages of the formation of the foci the leading role belonged to the water factor of the transmission of the disease: water from irrigation ditches used by the population for both drinking and household needs. The unusual season when this outbreak took place (the cold time of the year) was due to highly unfavorable sanitary and hygienic conditions formed as the result of the antiterrorist operation.

RESUMEN

The results of complete identification revealed the phenotypical similarity of V. cholerae strains isolated from surface water reservoirs im Grozny in 1995 and 2000. The strains under study proved to be atoxigenic, hemolysis-positive and phage resistant. The modified method of subculturing from peptone water 1 to peptone water 2 was more labor-consuming, but it was used in the bacteriological laboratory of the specialized antiepidemic brigade with the aim of more profound study of open reservoirs for the presence of V. cholerae with the positive result.

RESUMEN

Information on the main trends in the activity and on the organization of the work of the specialized antiepidemic brigade of the Stavropol Research Institute for Plague Control in Grozny in the first half of the year 2000 is presented. At the initial stage of this activity the specialists of the brigade were entrusted with all functions of the local state sanitary and epidemiological service which had lost its qualified personnel and material resources. At the initial stages of the work the main attention was drawn to the organization of the registration of patients with infectious diseases, to the control of drinking water and to the realization of immunoprophylactic measures, etc. The specialists of the brigade carried out their work in close cooperation with the medical service of the Territorial Head Office of the Ministry of Emergency Situations of the Russian Federation in the Chechen Republic.

RESUMEN

The working experience of the analytical group as the section of the specialized antiepidemic brigade for the organization and realization of antiepidemic measures, made under the conditions of the antiterrorist operation in the Chechen Republic in the year 2000, is generalized. The necessity for equipping the specialized brigade with a modern computer with peripherals and the necessary specialized computer programs, as well as with modem and fax-telephone facilities, is shown. The necessity of equipping this brigade with the electronic variant of the information and legal data base (references on methods, sanitary norms and other normative documents on sanitation, hygiene, epidemiology, bacteriology and the organization of health service) is indicated.

RESUMEN

The influence of Mn, Fe, Co, Ni, Cu and Zn on experimental Yersinia pestis infection in three species of gerbils (Rhombomys opimus, Meriones meridianus, M. tamariscinus) under conditions, imitating natural biogeochemical anomalies, was studied. The addition of Co and Ni to the fodder given to the animals for the period of 2-4 weeks prior to infection aggravated the course of infection. In similar experiments Cu, Fe and Mn aggravated or, on the contrary, inhibited the development of infection, depending on the environmental conditions. The addition of Zn produced practically no effect on the course of the disease, but its use in combination with Cu, depending on the dose, inhibited or enhanced the action of this element. The addition of Cu, Fe and Mn to fodder made it possible to reproduce the effect of protection from plague, observed in nature in the presence of some biogeochemical anomalies. This result may serve as an argument in support of the concept stating that the chemical composition of the environment is a factor capable of regulating the infectious process in animals in nature.
