RESUMEN

We examine a possibility to exploit the nonlinear lens effect-the initial stage of self-focusing to localize initially broad field distribution into the small central area where wave collapse is arrested-the nonlinear beam tapering. We describe two-dimensional localized solitary waves (ring solitons) in a physical system that presents a linear medium in the central core, surrounded by the cladding with the focusing Kerr nonlinearity. The standard variational analysis demonstrates that such solitons correspond to the minimum of the Hamiltonian.

AIM: Determination of the degree of phylogenetic relationship of Yersinia pestis strains iso- lated from the territories of natural foci of plague from the Caucasus using VNTR-typing by 25 loci (MLVA25). MATERIALS AND METHODS: 26 strains of Y pestis from Russian natural foci of the Caucasus were used in the study. 25 loci of tandem repeats in Y pestis genome by Le Fleche scheme were used for execution of multi-locus VNTR-analysis. Deciphering of nucleotide sequences was carried out in automatic sequencer ABI 3130 Genetic Analyser. Analysis of confinement of clusters to certain territories, objects and time of isolation of strains was carried out. using Arc GIS 10.1 program. RESULTS: Groups of MLVA25-types of various levels of discrimination were formed: clusters, groups and subgroups. Clusters were formed by strains ofvarious taxonomic membership: main and subspecies of Y pestis. Subgroups reflect membership of strains in certain foci, and MLVA25-types - the degree of genetic relationship. CONCLUSION: Genetic <> of plague causative agents obtained using MLVA25-types circulating in various natural-focal territories allow to solve problems ofboth theoretical and practical character: from interpretation of microevolution processes to the search of the source of infection and ways of its spread during possible epidemic complications.

AIM: Conducting epizootological and epidemiological zoning of the territory ofNorth Caucasus -Federal Region (NCFR) by the degree of non-welfare by anthrax using Arc GIS10 program. MATERIALS AND METHODS: 2 parameters were used during zoning ofthe territories of subjects of NCFR (Stavropol Region, Karachaevo-Cherkessia, Kabardino-Balkaria, Chechen, North Ossetia-Alania, Ingushetia, Dagestan Republics) by non-welfare by anthrax: integrated parameter of infection risk and epizootics risk. The parameters were input into the Arc GIS 10 program. Materials on infection of humans and animals with anthrax for the last 55 (1960 - 2014) years were used. RESULTS: The regions of the territories of NCFR subjects were distributed by non-welfare degree into 4 groups: group 1 - regions with a low degree of non-welfare, group 2 - regions with an average degree of non-welfare, group 3 - regions with a high degree of non-welfare, group 4 - regions with a very high degree-of non-welfare. CONCLUSION: Taking into account high parameters of spread and inten- sity of epizootological and epidemiological situation, regions of groups 3 and 4 could be classified as endemic territories for anthrax, whereas all the other regions could be attributed to the zone of sporadic infection manifestation.

RESUMEN

We present the results of experimental and theoretical study of an energy absorption of femtosecond laser pulse in fused silica. Fundamental and second harmonics of ytterbium laser were used in experiment while general case was considered theoretically and numerically. More efficient absorption at the second harmonics is confirmed both experimentally and numerically. Quantitative characterization of the theoretical model is performed by fitting key parameters of the absorption process such as cross-section of multi-photon absorption and effective electronic collision and recombination times.

To improve the sanitary and epidemiological surveillance at the Olympic Games has developed a system of GIS for monitoring objects and situations in the region of Sochi. The system is based on software package ArcGIS, version 10.2 server, with Web-java.lang. Object, Web-server Apach, and software developed in language java. During th execution of the tasks are solved: the stratification of the region of the Olympic Games for the private and aggregate epidemiological risk OCI various eti- ologies, ranking epidemiologically important facilities for the sanitary and hygienic conditions, monitoring of infectious diseases (in real time according to the preliminary diagnosis). GIS monitoring has shown its effectiveness: Information received from various sources, but focused on one portal. Information was available in real time all the specialists involved in ensuring epidemiological well-being and use at work during the Olympic Games in Sochi.

RESUMEN

Official statistics of Republican SES on infectious diseases are used. The characteristics of the current epidemiological situation in the Republic of Abkhazia is given. The analysis of infectious dis- eases from 2012 to 2014 is presented. It was found that the most widespread infectious diseases are acute respiratory viral and acute intestinal infection. During the analyzed period in the country outbreaks of dysentery, whooping cough and measles were reported. Epidemic threat is the presence of active natu- ral foci of infectious diseases and permanently disadvantaged anthrax points on the territory ofAbkhazia. Activities carried out by the Ministry of Health of the Republic of Abkhazia with the help and active participation of Rospotrebnadzor contributed stabilization the epidemiological situation.

AIM: Development of a cadaster of stationary unfavorable by anthrax points in Krasnodar region by using GIS technologies. MATERIALS AND METHODS: Ageographical information system (GIS) of ESRI program--Arc GIS 10.1 was used for the composition of an electronic cadaster of stationary unfavorable by anthrax points. Topographic basis--electronic maps of Krasnodar region (M 1:200 000). RESULTS: An evaluation of safety by anthrax of Krasnodar region areas bythe amount of stationary unfavorable by anthrax points situated in its territories, by their activity during the 76 year observation period was given. Spatial adjoint analysis of ecological timing of stationary unfavorable by anthrax points to soil and landscape zones in Krasnodar region was carried out. CONCLUSION: Electronic cadaster of stationary unfavorable by anthrax points in Krasnodar region may serve as a basis for purposeful implementation of prophylaxis measures.

RESUMEN

Problems of the perspective of geoinformational systems (GIS) use for improvement of epidemiologic control of infectious diseases including during mass events are examined. A scheme of creation of the decision making support system (DMSS) is proposed, the system is based on GIS, has general primary data and analytical-prognostic tool base including subsystems: real-time primary information intake, information analysis, epizootic and epidemic activity prognosis on the certain territories and the federal level. Specialized personnel training, creation of infrastructure for DMSS functioning in primary Federal Service of Surveillance for Protection of Consumers Rights and Human Welfare units are designated as priority tasks.

We analyze the physical mechanisms limiting optical fiber resonator length and report on the longest ever laser cavity, reaching 270 km, which shows a clearly resolvable mode structure with a width of approximately 120 Hz and peak separation of approximately 380 Hz in the radio-frequency spectrum.

PURPOSE: Clinical studies produce conflicting results on pain relief for shock wave lithotripsy (SWL). We performed a systematic review and meta-analysis to compare non-steroidal anti-inflammatory drugs (NSAIDs) and opioids in pain relief for SWL powered by an electromagnetic generator. MATERIAL AND METHODS: A search of MEDLINE and EMBASE was performed and all randomized controlled trials comparing NSAIDs and opioids in pain relief for SWL using modern electromagnetic lithotripters were included in the analysis. Data from 3 trials (244 patients) were pooled. The primary outcome measure was adequate analgesia, defined as "if no additional pain relief was used". The difference in the proportion of patients with adequate anesthesia was compared between the NSAIDs and opioids groups as an odds ratio and odds ratio were pooled across the 3 trials with a fixed effects model. RESULTS: There was no statistically significant difference between using NSAIDs and opioids for pain relief during SWL using modern electromagnetic lithotripters (odds ratio 0.886, 95% CI 0.446-1,760, p = 0.730). CONCLUSIONS: Our analysis shows that in relieving pain during SWL using modern electromagnetic lithotripters NSAIDs are as effective as opioids.

RESUMEN

We present the first experimental demonstration of a resolvable mode structure with spacing c/2nL in the RF spectra of ultralong Raman fiber lasers. The longest ever demonstrated laser cavity (L=84 km), RF peaks of ~100 Hz width and spacing ~1 kHz have been observed at low intracavity powers. The width of the peaks increases linearly with growing intracavity power and is almost independent of fiber length.

INTRODUCTION: Management of urolithiasis in morbidly obese patients is usually associated with higher morbidity and mortality compared to non-obese patients. In morbidly obese patients, since the kidney and stone are at a considerable distance from the skin (compared to non-obese patients) difficulty may be found in positioning the patient so that the stone is situated at the focal point of the lithotripter. OBJECTIVE: To evaluate the outcomes and cost-efficiency of extracorporeal shock wave lithotripsy (ESWL) in the treatment of renal pelvicalyceal stones sized between 6 and 20 mm in morbidly obese patients. MATERIALS AND METHODS: Using various aids, such as mobile overtable module, extended shock pathway and abdominal compression 37 patients with body mass index more than 40 kg/m2 were treated using the Siemens Lithostar-plus third generation lithotripter. The size of renal pelvicalyceal stones was between 6 and 20 mm. Treatment costs for shock wave lithotripsy were calculated. RESULTS: The overall stone free rate at 3 months of 73% was achieved. The mean number of treatments per patient was 2.1. The post-lithotripsy secondary procedures rate was 5.4%. No complications, such as subcapsular haematoma or acute pyelonephritis were recorded. The most effective (87% success rate) and cost-efficient treatment was in the patients with pelvic stones. The treatment of the patients with low caliceal stones was effective in 60% only. The cost of the treatment of the patients with low calyceal stones was in 1.8 times higher than in the patients with pelvic stones. CONCLUSION: We conclude that ESWL with the Siemens Lithostar-plus is the most effective and cost-efficient in morbidly obese patients with pelvic stones sized between 6 and 20 mm. 87% success rate was achieved. The increased distance from the skin surface to the stone in those patients does not decrease the success rate provided the stone is positioned in the focal point or within 3 cm of it on the extended shock pathway. ESWL should not be considered as the first line of treatment in the morbidly obese patients with low caliceal stones where the stone was positioned more than 1 cm from the focal point on the extended shock pathway.

RESUMEN

A new method for the preparation of nonstoichiometric hydroxyapatite (HA) "dense" and porous granules, round in form and up to 8 mm in sizes designed for application in orthopaedic surgery has been developed. The "dense" granules' porosity was up to 32% and they only contained micropores. They differed from that kind of granules by increased values of compression strength (up to 48 MPa). The porous granules contained a system of interconnected micro- and macropores. The porosity value (up to 70%) and the porosity structure were similar to those in the mineral framework of a spongy bone. The compression strength of the porous granules (up to 25 MPa) was high enough for various kinds of application. Granules of both sorts were used in performing 42 operations on the locomotor system. Depending upon localization and supposed level of the injured area loading, "dense" or porous granules were used. The postoperative observations (up to four years in length) have attested to the high quality of the granules.

RESUMEN

Information on emergency situations (ES) on the territory of the North Caucasus from the 90s of the last century till the present moment is presented. The importance of the specific immunoprophylaxis of infections for the liquidation of epidemiological aggravations in ES and for their prevention is shown. Information on the scope of specific immunoprophylaxis in the zones of ES and on different approaches to its carrying out. The conclusion was made on the expediency of making corrections in the normative documents of the Ministry of Health of the Russian Federation, regulating planned vaccinations and vaccinations made on epidemic indications in connection with different kinds of ES.

RESUMEN

The retrospective and operative epidemiological analysis of leptospirosis morbidity on the administrative territories of the Southern Federal District during the period of 1990-2002 was carried out. The dynamics and tendencies of morbidity in individual administrative units of the region were shown. Materials on the negative influence of the emergency situation arising in the region as the result of the natural calamity in summer 2002 were presented. As shown in this work, timely decisions on the stabilization of epidemiological situation taken at the period of high flood and inundation in summer 2002 in the Southern Federal District and their efficient execution made it possible to keep the epidemiological situation in leptospirosis under control.

RESUMEN

The data on leptospirosis morbidity in the Krasnodar Territory, one of the most epidemiologically unsafe areas in this infectious disease in the Russian Federation, are presented. High morbidity registered in this territory greatly depends on the epidemic outbreaks appearing due to defects in the realization of a complex of sanitary and veterinary measures. The Krasnodar Territory is liable to be affected by natural calamities. In 1997 a severe outbreak of leptospirosis took place here in connection with the high flood. Due to timely prophylactic measures the epidemiological consequencies of the similar calamity occurring in 2002 were brought to a minimum.
