RESUMEN

Retrospective analysis of the ultrasonograms of 42 hepatic amoebic abscesses in 34 patients was performed. All lesions were less echogenic than normal liver. All but 1 were contiguous with the liver capsule and had slight distal sonic enhancement. Twenty-three were predominantly homogeneous with fine, low-level echoes. This pattern is highly suggestive of hepatic amoebic abscess. Nineteen abscesses did not show this pattern and could not be diagnosed based on ultrasonographic criteria.

RESUMEN

The results of this investigation reveal that 39 per cent of patients in a study group of 46 patients with heart valve prostheses had gallstones if they survived 18 months or longer following valve replacement. In contrast, the prevalence of gallstones in a general population of autopsied rheumatic heart disease patients, including those who had been operated for severe valvular heart disease and had not survived for more than one month, was only 12 per cent. These findings suggest that gallstones are a frequent late complication of heart valve replacement.

RESUMEN

Acute appendicitis with abscess often produced a distinctive extrinsic pressure deformity of the cecum. If the abscess is large or disseminated, other parts of the gastrointestinal tract may be secondarily involved. Abscesses usually produce extrinsic compression defects with benign radiologic characteristics, but occasionally the lesions may simulate malignant disease. Even careful evaluation of the clinical symptoms and signs may not be helpful in the differentiation of benign from malignant disease. Three examples of atypical acute appendicitis are described in patients with clinical and radiologic findings strongly suggestive of malignant disease.

RESUMEN

A double-blind comparison of two intravenous cholangiocholecystographic agents of similar molecular structure, meglumine iodoxamate and meglumine iodipamide, was carried out in 90 patients. Each produced a comparably high percentage of satisfactory examinations. A similar pattern of excretion speed and intensity was noted for each, though meglumine iodoxamate appeared to be slightly more effective in patients with elevated serum bilirubin. Less evidence of hepatic toxicity and fewer and less serious adverse side effects were observed in patients given the newer agent, meglumine iodoxamate.

RESUMEN

The intravenous administration of sincalide, the C-terminal octapeptide fragment of cholecystokinin, affords safe and effective means for gallbladder contraction with resultant cystic and common bile duct visualization. Intravenous sincalide circumvents the problem of unpredictability of response of the gallbladder to a fatty meal and variability in the rate of release of endogenous cholecystokinin. Peak gallbladder contraction occurs earlier than with a fatty meal.

A preliminary study of 18 patients following a single bolus intravenous injection of 10 to 30 cc. of meglumine iodoxamate (Cholovue), a new contrast medium for cholangio-cholecystography, showed no significant adverse clinical or laboratory effects with 10 to 20 cc., and only transient, minor changes with 30 cc. Early and persistent visucalization of the subsegmented and major bile ducts and the gallbladder was obtained. Good to excellent opacification was obtained in all cases with doses ranging between 0.11 cc./kg. (19.7 mg. iodine) to 0.59 cc./kg. (108 mg. iodine). Meglumine iodoxamate appears to be highly effective agent for cholangio-cholecystography.
