RESUMEN

OBJECTIVE: The aim of this study was to describe deployed National Guard members' and their families' perceptions of their experience with family reintegration, and the causes and conditions of challenges reintegration presents after deployment. METHODS: A total of 26 National Guard members and 19 family members participated in individual (n = 22), couples (n = 6), or focus group (n = 17) interviews. In-depth interviews were used to assess needs and maximize input from military families regarding deployment-related experiences and reintegration issues. Qualitative coding and analysis of data were completed using NVivo. RESULTS: Finding their way back in is the key process that the military members must complete to successfully reestablish their desired social connections with the family and reclaim their place within the family. Several conditions shape the degree of challenges with reintegration that veterans and their family will encounter. These include preparation for deployment, length and type of deployment, communication during deployment, and finally, awareness of how deployment changes the military member and the family. CONCLUSION: Support resources dedicated to providing National Guard members and their families with assistance in preparing for deployments and educating them about the importance of communication during deployment should be maintained and expanded. Broader educational efforts that increase awareness of what to expect regarding how deployment changes the military member and the family are needed.

PURPOSE: This study's purpose is to describe and explain how women 55 years of age and older with a family history of breast cancer make screening mammography decisions. DESIGN: A qualitative design based on grounded theory. This purposeful sample consisted of 23 women 55 years of age or older with one more first-degree relatives diagnosed with breast cancer. METHOD: Open-ended interviews were conducted with 23 women 55 years of age and older with a family history of breast cancer using a semistructured interview guide. Transcribed interview data were analyzed using constant comparative analysis to identify the conditions, actions, and consequences associated with participant's screening mammography decision making. FINDINGS: Women reported becoming aware of their breast cancer risk usually due to a triggering event such as having a family member diagnosed with breast cancer, resulting in women "guarding against cancer." Women's actions included having mammograms, getting health check-ups, having healthy behaviors, and being optimistic. Most women reported extraordinary faith in mammography, often ignoring negative mammogram information. A negative mammogram gave women peace of mind and assurance that breast cancer was not present. Being called back for additional mammograms caused worry, especially with delayed results. CONCLUSIONS: The "guarding against cancer" theory needs to be tested in other at-risk populations and ultimately used to test strategies that promote cancer screening decision making and the adoption of screening behaviors in those at increased risk for developing cancer. CLINICAL RELEVANCE: Women 55 years of age and older with a breast cancer family history need timely mammogram results, mammography reminders, and psychosocial support when undergoing a mammography recall or other follow-up tests.

RESUMEN

A statistics CD-ROM tutorial program was developed to replace a classroom course with several self-study modules. For the CD-ROM delivery method, students were asked to complete a Likert-type questionnaire with response options similar to those used for end-of-course evaluations at the nursing school. Ratings ranged from 1 to 5, with 1 equaling outstanding and 5 equaling completely unsatisfactory. Ratings for the CD-ROM were compared with those of a Web-based course taught the prior year. Mean satisfaction ratings of Web-based delivery of content was low due to technical problems with the Internet courseware used. Overall satisfaction with the CD-ROM for students who used all the components was improved substantially, compared to the Web-based delivery method.

Although several definitions and categorizations of home environmental modification strategies exist, previous researchers have not addressed whether the conceptualizations developed by clinicians and researchers match the way families think about how they modify the home environment in order to provide care to frail elders. The aim of the analysis reported here was to describe, from the family's perspective, the home environmental modification strategies that they use. Twenty-four caregivers of community-dwelling elders with a variety of impairments were interviewed. Seventeen families provided guided tours of the elder's home and allowed selected observation of some caregiving activities. Forty-four modification strategies were identified and categorized into one of seven home environmental modification purposes: organizing the home, supplementing the elder's function, structuring the elder's day, protecting the elder, working around limitations or deficits in the home environment, enriching the home environment, and transitioning to a new home setting. More research is needed on the processes families use to generate and refine the home environmental modification strategies identified in this study.

A cross-sectional survey of members was used to examine the feasibility of developing a virtual health and wellness center in the Oregon Air National Guard. The survey asked about the health behaviors members wanted to change and explored their current access to, and potential use of, the Internet to make these changes. Surveys were mailed to 1,368 members, and 521 (38%) were returned. Most respondents (92.5%) had contemplated making one or more health behavior changes in the past year, which included becoming more physically active (94.6%), improving eating habits (86%), losing weight (81%), improving stress management (62%), and reducing tobacco use (46%). Popular sources of health behavior change information used included reading material (95.5%) and the Internet (91.3%). Many respondents had Internet access (82%), and average computer self-efficacy, rated on a scale of 1 to 5, was very high (mean = 4.05, SD = 0.91). Use of the Internet by health professionals to promote health behavior change among Oregon Air National Guard personnel seems feasible and desirable.
