RESUMEN

Founded in 2009, the Online Journal of Public Health Informatics (OJPHI) strives to provide an unparalleled experience as the platform of choice to advance public and population health informatics. As a premier peer-reviewed journal in this field, OJPHI's mission is to serve as an advocate for the discipline through the dissemination of public health informatics research results and best practices among practitioners, researchers, policymakers, and educators. However, in the current environment, running an independent open access journal has not been without challenges. Judging from the low geographic spread of our current stakeholders, the overreliance on a small volunteer management staff, the limited scope of topics published by the journal, and the long article turnaround time, it is obvious that OJPHI requires a change in direction in order to fully achieve its mission. Fortunately, our new publisher JMIR Publications is the leading brand in this field, with a portfolio of top peer-reviewed journals covering innovation, technology, digital medicine and health services research in the internet age. Under the leadership of JMIR Publications, OJPHI plans to expand its scope to include new topics such as precision public health informatics, the use of artificial intelligence and machine learning in public health research and practice, and infodemiology in public health informatics.

RESUMEN

Pseudomonas putida is an uncommon cause of bacteremia, that usually seeds from multiple sources including soft tissues. Immunocompromised states predispose patients to fulminant infections, and sometimes death. Broad-spectrum antibiotics, such as fourth-generation cephalosporins, are usually indicated for treatment. Herein, we present a 71-year-old gentleman with fever and left leg swelling, found to have P. putida bacteremia. Intravenous (IV) ceftazidime was initiated, yielding clearance of blood cultures and clinical improvement.

INTRODUCTION: Diabetes Mellitus imbricates inflammatory processes on their pathophysiology, it could affect cardiac electrophysiology. Cardiac inflammatory process leads electrocardiogram changes. Nevertheless, there are discrepancies about whether it prolongs or decreases waves and intervals such as QRS and QT. Furthermore, QRS dispersion has not been studied. OBJECTIVE: To identify QRS complex and QT interval modifications in type 2 diabetic patients. METHODS: A descriptive cross-sectional study was carried out in 59 type 2 diabetic patients selected by non-probabilistic sampling. Electrocardiogram was performed, QRS and QT interval were measured manually by two observers. Dispersion of both variables was calculated to compare with normal values. RESULTS: Two variables showed differences for sex, QRS dispersion was higher in females (45.84), p=0.0001 vs. reference value; QT dispersion (63.12) showed significance difference (p=0.0246) vs. reference value for males. Greater than five years of illness was related to higher QRS values (124.11 vs. 137.28), p=0.005 and corrected QT dispersion (61.81 vs. 78.79), p=0.022. CONCLUSIONS: The electrocardiographic differences between sexes may suggest a gender influence. The longer duration of diabetes diseases could increase cardiovascular risk of arrhythmias due to greater QRS duration and corrected QT interval prolongation.

RESUMEN

Sclerosing mesenteritis (SM) is a rare inflammatory condition with unknown etiology that affects the mesenteric adipose tissue. We present a case of a 49-year-old male with severe abdominal pain who underwent abdominal biopsy confirming the presence of adipose inflammation and necrosis. The diagnosis of SM was made, and the patient was treated with prednisone and tamoxifen. As this condition is rare, there are no standard guidelines for management. This case aims to outline a possible treatment plan.

Pharmacovigilance is the science and activity relating to the collection, detection, assessment, monitoring, and prevention of adverse effects with pharmaceutical products. Pharmacovigilance basically targets safety of medicine. Pharmacists have crucial role in health systems to maintain the rational and safe use of medicine for they are drug experts who are specifically trained in this field. Effective use of pharmacists' workforce will improve the outcome of the pharmacotherapy as well as decrease global health costs. Given their advanced training, pharmacists can utilize pharmacovigilance systems interfaced with electronic health records to monitor the performance of the drugs they fill and also identify adverse drug reactions earlier than non-pharmacists, thereby reducing high healthcare costs.

PURPOSE: The recent and rapid growth of social networking site (SNS) use presents a unique public health opportunity to develop effective strategies for the recruitment of hard-to-reach participants for cancer research studies. This survey investigated childhood cancer survivors' reported use of SNS such as Facebook or MySpace and their perceptions of using SNS, for recruitment into survivorship research. METHODS: Sixty White, Black, and Hispanic adult childhood cancer survivors (range 18-48 years of age) that were randomly selected from a larger childhood cancer study, the Chicago Healthy Living Study, participated in this pilot survey. Telephone surveys were conducted to understand current SNS activity and attitudes towards using SNS as a cancer research recruitment tool. RESULTS: Seventy percent of participants reported SNS usage of which 80 % were at least weekly users and 79 % reported positive attitudes towards the use of SNS as a recruitment tool for survivorship research. IMPLICATIONS FOR CANCER SURVIVORS: The results of this pilot study revealed that SNS use was high and regular among the childhood cancer survivors sampled. Most had positive attitudes towards using SNS for recruitment of research. The results of this pilot survey suggest that SNS may offer an alternative approach for recruitment of childhood cancer survivors into research.

Background. There is a lack of expertise in the procedure of open radical retropubic prostatectomy in West Africa therefore necessitating the training of urologists in the subregion in this procedure. Aim. This report looks at the early outcomes of a single surgeon in this procedure after an SIU fellowship. Methodology. A prospective study of the initial twenty consecutive patients with clinically localized prostate cancer that underwent open radical retropubic prostatectomy at the Korle Bu Teaching hospital, Accra. Results. The mean followup was 19.5 months (range 7 months-36 months). The mean age was 62.7 yrs. For the clinical stage, 60% were T1c and 40% T2a with a mean Gleason score of 6.5. The mean estimated blood loss was 1140.0 mLs with a transfusion rate of 70%. For the pathologic stage, pT2 cancers formed 60%, pT3 25%, and pT4 5% with a mean Gleason score of 6.8. No lymph node involvement was noted. The perioperative complications rate was 15%, a postoperative potency recovery rate of 78.6% with all the patients being continent of urine. The tPSA of 95% of the patients had remained less than 0.4 ng/mL. Conclusion. The SIU scholarship offers an avenue for training in radical prostatectomy for sub-Saharan Africa.

The photophysical properties of 4-[2-(6-hydroxy-2-naphthalenyl)-ethenyl]-1-methyl-pyridinium (HNEP(+)) and its deprotonated form (NEP), a benzofused derivative of Brooker's merocyanine (BM), were investigated through a combined spectroscopic and computational approach. Despite their structural similarities and similar pK(a) values, HNEP(+)/NEP and BMH(+)/BM differ in the extent of charge delocalization in the ground and excited states. NEP exhibits the spectral characteristics of a charge transfer species in solvents in which BM exists in a charge-delocalized quinoid; however, quantum chemical calculations show that the CT absorption of NEP is not necessarily a consequence of the zwitterionic character. HNEP(+) displays larger Stokes shifts than BMH(+), and NEP demonstrates enhanced solvatochromism relative to BM as a consequence of benzofusion.

OBJECTIVE: To investigate factors affecting the internal brain drain of healthcare professionals in the Kingdom of Saudi Arabia. METHODS: A cross-sectional study was conducted using an anonymous self-administered online questionnaire sent to all Saudi students enrolled in healthcare profession programs in North America. The data was collected between January and March 2008 at the University of Illinois at Chicago, Chicago, Illinois, United States of America. Results were analyzed using logistic regression analyses. RESULTS: A total of 377 completed questionnaires were returned. Results revealed that 71% of respondents intended to return to work within the 2 major urban cities Riyadh and Jeddah. Respondents who completed their undergraduate studies in a large city were more likely to work in the same city (odds ratio [OR]=3.2; p=0.000; 95% confidence interval [CI] = 2.0-5.2). Furthermore, 51% of the students were willing to work in a rural area for a 50% or more increase in their salary. Finally, men were more willing to work in a rural area for a financial incentive (OR=2.3; p=0.006, 95% CI = 1.3-4.3). CONCLUSION: This study suggests that realistic financial incentives would probably not suffice to attract Saudi healthcare providers to rural areas. The provision of medical schools in smaller cities and rural areas is predicted to be a more effective method for improving the current maldistribution of healthcare providers.

The photophysics of 1-ethyl-4,6-bis(4-methoxyphenyl)-2(1H)-pyrimidone (1) and 1-ethyl-4,6-bis(4-(dimethylamino)phenyl)-2(1H)-pyrimidone (2) were investigated to determine the mechanisms of emission switching in response to protonation. UV-vis and steady state emission spectroscopy of the protonated and unprotonated forms across a range of solvents reveal the polarity dependence of the vertical excitation energies. Emission lifetimes and quantum yields show the solvent dependency of the excited states. Emission enhancements were observed in polyethylene glycol solutions and in the solid state (both thin film and single crystal), demonstrating the role of intramolecular rotation in thermal relaxation of the excited states. TD-DFT calculations provide insights into the excited state geometries and the role of intramolecular charge transfer. The collected data show that emission of diphenylpyrimidones can be modulated by four factors, including the identity of the electron-donating auxochrome, protonation state, solvent polarity, and viscosity.

RESUMEN

A series of stilbazolium dimers were synthesized and investigated as sterically demanding ligands targeting the norepinephrine transporter (NET). The environmentally sensitive fluorescence of the dyes enables their use as self-reporting ligands; binding to and displacement from NET can be monitored by fluorescence microscopy.
