RESUMEN

OBJECTIVE: A systematic review was performed to synthesize and identify risk factors involved in TMD onset. STUDY DESIGN: Electronic searches were conducted in PubMed, Web of Science, Scopus, Embase, PsyInfo and Lilacs databases, as well as in three gray literature databases (Google Scholar, ProQuest and Open grey). The studies were blindly assessed by two reviewers and selected by a pre-defined eligibility criterion. Risk of bias of included studies was assessed using the Joanna Briggs Institute Critical Appraisal Checklist for Cohort Studies. Grading of Recommendations Assessment, Development and Evaluation (GRADE) was evaluated for most related factors. RESULTS: Twenty-one cohort studies were included. Significant factors were female gender, symptoms of depression and anxiety, perceived stress, sleep quality, symptoms of obstructive sleep apnea and presence of any comorbidity, such as Irritable Bowel Syndrome, lower back pain, headache frequency, tension-type headache, migraine and mixed headache. Moreover, high estrogen and low testosterone levels in utero, greater pain perception, jaw mobility pain, pain during palpation, orofacial anomalies, as well as extrinsic and intrinsic injuries were also significant. CONCLUSIONS: Several factors seems to be involved in TMD onset, however, more studies with standardized methodology are necessary to confirm these findings.

The biomineralisation of radicular dentin involves complex molecular signalling. Providing evidence of protein binding sites for calcium ions and mineral precipitation is essential for a better understanding of the remineralisation process. This study aimed to evaluate the functional relationship of metalloproteinases (MMPs) and non-collagenous proteins (NCPs) with mineral initiation and maturation during the biomineralisation of radicular dentin. A standardized demineralisation procedure was performed to radicular dentin slices. Samples were remineralised in a PBS-bioactive material system for different periods of time. Assessments of ion exchange, Raman analysis, and energy dispersive X-ray analysis (EDAX) with a scanning electron microscope (SEM) were used to evaluate the remineralisation process. Immunohistochemistry and zymography were performed to analyse NCPs and MMPs expression. SEM evaluation showed that the mineral nucleation and growth occurs, exclusively, on the demineralised radicular dentin surface. Raman analysis of remineralised dentin showed intense peaks at 955 and 1063 cm-1, which can be attributed to carbonate apatite formation. Immunohistochemistry of demineralised samples revealed the presence of DMP1-CT, mainly in intratubular dentin, whereas DSPP in intratubular and intertubular dentin. DMP1-CT and DSPP binding sites control carbonate apatite nucleation and maturation guiding the remineralisation of radicular dentin.

STATEMENT OF PROBLEM: Ceramic restorations are popular because of their excellent optical properties. However, failures are still a major concern, and dentists are confronted with the following question: is sleep bruxism (SB) associated with an increased frequency of ceramic restoration failures? PURPOSE: The purpose of this systematic review and meta-analysis was to assess whether the presence of SB is associated with increased ceramic restoration failure. MATERIAL AND METHODS: Observational studies and clinical trials that evaluated the short- and long-term survival rate of ceramic restorations in SB participants were selected. Sleep bruxism diagnostic criteria must have included at least 1 of the following: questionnaire, clinical evaluation, or polysomnography. Seven databases, in addition to 3 nonpeer-reviewed literature databases, were searched. The risk of bias was assessed by using the meta-analysis of statistics assessment and review instrument (MAStARI) checklist. RESULTS: Eight studies were included for qualitative synthesis, but only 5 for the meta-analysis. Three studies were categorized as moderate risk and 5 as high risk of bias. Clinical and methodological heterogeneity across studies were considered high. Increased hazard ratio (HR=7.74; 95% confidence interval [CI]=2.50 to 23.95) and odds ratio (OR=2.52; 95% CI=1.24 to 5.12) were observed considering only anterior ceramic veneers. Nevertheless, limited data from the meta-analysis and from the restricted number of included studies suggested that differences in the overall odds of failure concerning SB and other types of ceramic restorations did not favor or disfavor any association (OR=1.10; 95% CI=0.43 to 2.8). The overall quality of evidence was considered very low according to the GRADE criteria. CONCLUSIONS: Within the limitations of this systematic review, the overall result from the meta-analysis did not favor any association between SB and increased odds of failure for ceramic restorations.
