RESUMEN

Vaccination coverage has been dropping in Brazil and other countries. In addition, immune responses after vaccination may not be homogeneous, varying according to sociodemographic and clinical factors. Understanding the determinants of incomplete vaccination and negative antibody test results may contribute to the development of strategies to improve vaccination effectiveness. In this study, we aimed to investigate the frequency of vaccine adherence, factors associated with incomplete vaccination for measles, mumps, rubella (MMR) and hepatitis A, and factors associated with the seronegative test results for measles, mumps and hepatitis A at 2 years of age. This was a population-based cohort that addressed health conditions and mother/infant nutrition in Cruzeiro do Sul city, Brazil. Vaccination data were obtained from official certificates of immunization. The children underwent blood collection at the two-year-old follow-up visit; the samples were analyzed using commercially available kits to measure seropositivity for measles, mumps, and hepatitis A. We used modified Poisson regression models adjusted for covariates to identify factors associated with incomplete vaccination and negative serology after vaccination. Out of the 825 children included in the study, adherence to the vaccine was 90.6% for MMR, 76.7% for the MMRV (MMR + varicella), and 74.9% for the hepatitis A vaccine. For MMR, after the adjustment for covariates, factors associated with incomplete vaccination included: white-skinned mother; paid maternity leave; raising more than one child; lower number of antenatal consultations; and attending childcare. For hepatitis A, the factors included: white-skinned mother and not having a cohabiting partner. The factors with statistically significant association with a negative antibody test result included: receiving Bolsa Familia allowance for measles and mumps; incomplete vaccination for measles; and vitamin A deficiency for mumps. Strategies to improve the efficiency of vaccine programs are urgently needed. These include improvements in communication about vaccine safety and efficacy, and amplification of access to primary care facilities, prioritizing children exposed to the sociodemographic factors identified in this study. Additionally, sociodemographic factors and vitamin A deficiency may impact the immune responses to vaccines, leading to an increased risk of potentially severe and preventable diseases.

This article aims to assess the consumption of ultra-processed foods (UPFs) during the first year of life and associated factors. We analyzed data from the one-year follow-up of the MINA-Brazil birth cohort. Socioeconomic, demographic and obstetric data were collected in the baseline study and at one-year follow-up (n = 774). Dietary intake during the previous day was assessed using a food frequency questionnaire to determine UPF consumption. Multiple Poisson regression with robust variance was performed to estimate prevalence ratios (PR) and confidence intervals (95%CI) for factors associated with the consumption of ≥ 3 UPFs. Prevalence of UPF consumption and consumption of ≥ 3 UPFs was 87.5% and 40.5%, respectively. Consumption of ≥ 3 UPFs was associated with lower maternal education (≤ 9 years of schooling [PR: 2.02 95%CI 1.42; 2.87] and between 10-12 years of schooling [PR: 1.55, 95%CI 1, 11; 2.14]), below-average wealth index (PR: 1.26, 95%CI 1.04; 1.53), and having a teenage mother (PR:1.19, 95%CI 1.00; 1.49). Lack of dietary diversity was inversely associated with the outcome (RP: 0,65, IC95% 0,51; 0,81). The consumption of ≥ 3 UPFs was associated with lower wealth index, education and maternal age. Dietary diversity was associated with higher levels of UPF consumption.


O objetivo deste artigo é avaliar o consumo de alimentos ultraprocessados (AUP) e fatores associados no primeiro ano de vida. Trata-se da análise de dados do seguimento de um ano da coorte de nascimentos MINA-Brasil. O consumo ≥ 3 AUP se associou às características maternas de menores escolaridade, índice de riqueza e idade materna, e a diversidade da dieta ao maior consumo de AUP.

Abstract Objectives: to investigate the factors associated with continued breastfeeding (BF). Methods: All the parturients at a local maternity from July 2015 to June 2016 were invited to participate in a cohort study in Cruzeiro do Sul, Acre. Data on socioeconomic, demographic, obstetric and neonatal of the babies were obtained for the interview. Multiple Poisson regression models with robust variance were used to estimate the prevalence ratio (PR) and 95% confidence intervals (95%CI). Results: among the 1551 mothers contacted, 305 lived in the rural area, leaving 1,246 eligible mothers living in urban area. For the 1-year cohort follow-up, 74 non-twin babies were assessed. Most of the mothers reported to have mixed skin color (79%), are over 21 years old (72°%o), more than 10 years of schooling (72%>) and with unpaying job (54%). The children's age ranged from 10 to 15 months. The frequency of continued breastfeeding was 69,4%> (95%oCI=66.0-72.6). The factors negatively associated with continued breastfeeding were the use of bottle feeding (PR=1.44; CI95%> =1,33-1.56) and pacifier (PR=2.54; CI95%> =1.98-3.27), after adjusting for maternal age and socioeconomic variables. Conclusion: the frequency of continued breastfeeding in Cruzeiro do Sul was higher than the national estimates, but below the WHO recommendations for breastfeeding up to two years of age.


Resumo Objetivos: investigar frequência e fatores associados ao aleitamento materno (AM) continuado. Métodos: estudo de coorte em Cruzeiro do Sul, Acre. Na maternidade, todas as parturientes de julho de 2015 a junho de 2016 foram convidadas à participação. Dados socioeconómicos, demográficos, obstétricos e neonatais dos bebês foram obtidos por entrevista. As razões de prevalência (RP) e intervalos com 95% de confiança (IC95%) foram estimados utilizando modelos múltiplos de regressão de Poisson com variância robusta. Resultados: dentre 1551 mães contatadas, 305 residiam em área rural, sendo elegíveis 1246 mães residentes em área urbana. No seguimento de 1 ano da coorte, 774 bebês não gemelares foram avaliados. A maioria das mães referiu ser de cor parda (79%o), maior de 21 anos (72%), >10 anos de escolaridade (72%) e não exercer atividade remunerada (54%). A idade dos bebês variou de 10 a 15 meses. A frequência do AM continuado foi de 69,4%o (IC950%= 66,0-72,6). Os fatores inversamente associados ao AM continuado foram uso de mamadeira (RP= 1,44; IC95% 1,33-1,57) e chupeta (RP= 2,55; IC95%= 1,99-3,28), após ajuste por idade materna e variáveis socioeconómicas. Conclusão: a frequência de AM continuado em Cruzeiro do Sul foi superior às estimativas nacionais, porém aquém das recomendações da OMS para amamentação até dois anos de idade.

OBJECTIVE: To determine the agreement between body self-image (based on the Stunkard figure rating scale) and nutritional status and to evaluate body satisfaction among the Khisêdjê indigenous people of Parque Indígena do Xingu (Xingu Indigenous Park). METHODS: A cross-sectional study involving 131 natives aged 20 and older. Data on body image, body mass index and waist circumference were collected. Kappa statistics, χ2 (p < 0.05), crude and adjusted prevalence ratios and Student's t-test were used for data analysis. RESULTS: The prevalence of overweight and obesity was respectively 42 and 5.3%. The percentage of satisfaction with body profile was 61.8% with no difference between the sexes. There was good agreement between actual and ideal self-image (p < 0.001), but poor agreement between actual and ideal self-image with nutritional status for both sexes. A higher prevalence of body dissatisfaction due to overweight was detected in individuals with central obesity and overweight. CONCLUSION: The results suggest that body self-image evaluated by the Stunkard silhouette scale has little applicability as an indicator of nutritional status among the indigenous Khisêdjê of Xingu Indigenous Park.

RESUMO: Objetivos: Verificar a concordância entre autoimagem corporal (escala de silhuetas de Stunkard et al.) e estado nutricional e avaliar a satisfação corporal em indígenas khisêdjê do Parque Indígena do Xingu. Métodos: Estudo transversal que incluiu 131 indígenas khisêdjê, com 20 anos ou mais. Coletaram-se dados sobre imagem corporal, índice de massa corporal e perímetro da cintura. Foram utilizados a estatística kappa, o teste χ2 (p < 0,05), as razões de prevalências brutas e ajustadas e o teste t de Student. Resultados: As prevalências de sobrepeso e obesidade foram, respectivamente, 42 e 5,3%. A porcentagem de satisfação com o perfil corporal foi de 61,8%, sem diferença entre os sexos. Houve boa concordância entre autoimagem real e autoimagem ideal (p < 0,001), porém baixa concordância entre a autoimagem real e ideal e o estado nutricional para ambos os sexos. Maior prevalência de insatisfação corporal por excesso de peso foi detectada entre indivíduos com obesidade central e excesso de peso. Conclusão: Os resultados sugerem que a autoimagem corporal avaliada por meio da escala de silhuetas de Stunkard et al. tem pouca aplicabilidade como indicador do estado nutricional de indígenas khisêdjê do Parque Indígena do Xingu.

We investigated linear growth and weight attained among 772 children at 10-15 months of age in the first population-based birth cohort in the Brazilian Amazon. Sociodemographic, maternal and birth characteristics were collected in interviews soon after birth at baseline. Anthropometric evaluation was conducted at 10-15 months. Multiple linear regression models were fitted for length-for-age (LAZ) and body mass index (BMI)-for-age Z scores (BAZ), considering a hierarchical conceptual framework with determinants at distal, intermediate and proximal levels, with adjustment for the child's sex and age. Mean LAZ and BAZ were 0.31 (SD: 1.13) and 0.35 (SD: 1.06), respectively. Overall, 2.2% of children were stunted and 6.6% overweight. Among socioeconomic factors, household wealth index was positively associated with LAZ (p for trend = 0.01), while children whose families received assistance from the Bolsa Família conditional cash transfer program were 0.16 Z score thinner (95% CI: -0.31, -0.00). Maternal height and BMI were positively associated with both LAZ and BAZ at 10-15 months of age (p for trend <0.001). Child's size at birth was positively related with LAZ (p<0.001 for both birth weight and length). BAZ was 0.34 (95% CI: 0.24, 0.44) higher, but 0.11 lower (95% CI: -0.21, -0.02), for each increase in 1 Z score of birth weight and length, respectively. Children with at least one reported malaria episode within the first year of life were 0.58 (95% CI: -1.05, -0.11) Z score shorter. Socioeconomic and intergenerational factors were consistently associated with LAZ and BAZ at 10-15 months of age. The occurrence of malaria was detrimental to linear growth. In a malaria endemic region, reduction of inequalities and disease burden over the first 1,000 days of life is essential for taking advantage of a critical window of opportunity that can redirect child growth trajectories toward better health and nutrition conditions in the long term.

The aim of this study was to identify the incidence of metabolic syndrome and related diseases in the Khisêdjê population living in the Xingu Indigenous Park, Mato Grosso State, Brazil, from 1999-2000 to 2010-2011. The study included 78 individuals aged ≥20 years. Data were analyzed using Student t test, linear regression, and Poisson regression. In 10 years of follow-up, cumulative incidence rates were 37.5% for metabolic syndrome, 47.4% for hypetriglyceridemia, 38.9% for arterial hypertension, 32% for central obesity, 30.4% for excess weight, 29.1% for hypercholesterolemia, 25% for low HDLc, 10.4% for high LDLc, and 2.9% for diabetes mellitus. Age proved to be a risk factor for incidence of hypertension, diabetes, and elevated LDLc, regardless of gender; male gender was a protective factor against incidence of central obesity, independently of age. The study showed deterioration of most target health indicators and exposure of the Khisêdjê to high cardiometabolic risk. These results may be related to changes in traditional lifestyle.

El objetivo del estudio fue identificar la incidencia del síndrome metabólico y de enfermedades asociadas en la población Khisêdjê, que reside en el Reserva Indígena de Xingú, Mato Grosso, Brasil, durante los años de 1999‐2000 a 2010‐2011. Fueron incluidos 78 individuos con edad ≥20 años. Los datos fueron evaluados a través de las pruebas estadísticas t de Student, regresión lineal múltiple y regresión de Poisson. La incidencia acumulada en los 10 años de seguimiento fue de 37,5% de síndrome metabólico, 47,4% de hipertrigliceridemia, 38,9% de hipertensión arterial y 32% de obesidad central. Independientemente del sexo, la edad está relacionada como factor de riesgo para la incidencia de hipertensión arterial, diabetes mellitus, y LDLc elevado. Ser de sexo masculino se constituye en un factor de protección para la incidencia de obesidad central, independientemente de la edad. Fueron identificados deterioros significativos en gran parte de los indicadores de salud evaluados en los Khisêdjê expuestos a riesgo cardiometabólico elevado. Este resultado puede estar relacionado con la alteración del estilo de vida tradicional de la población estudiada.

Em dez anos de seguimento verificou-se a incidência acumulada de 37,5% de síndrome metabólica, 47,4% de hipetrigliceridemia, 38,9% de hipertensão arterial, 32% de obesidade central, 30,4% de excesso de peso, 29,1% de hipercolesterolemia, 25% de baixo HDLc, 10,4% de elevado LDLc e 2,9% de diabetes mellitus. A idade foi identificada como um fator de risco para incidência de hipertensão arterial, diabetes mellitus e elevado LDLc, independentemente do sexo; ser do sexo masculino foi fator de proteção para incidência de obesidade central, não importando a idade. Identificou-se deterioração de grande parte dos indicadores de saúde avaliados, estando os Khisêdjê expostos a elevado risco cardiometabólico. Esse resultado pode estar relacionado a alterações no estilo de vida tradicional.

RESUMEN

Este estudo objetivou verificar a existência de associação entre o grau de atividade física e a presença de síndrome metabólica entre indígenas Khisêdjê. Foram avaliados 170 indivíduos com 20 anos ou mais. Obtiveram-se dados sociodemográficos, de testes físicos e análise de material biológico. Para análise dos dados, utilizou-se a estatística do qui-quadrado (p < 0,05), razões de prevalências (por ponto e por intervalo de 95% de confiança) brutas e ajustadas e teste t de Student. Resultados satisfatórios foram observados em relação aos testes de resistência cardiorrespiratória, flexibilidade, flexão de braço e tronco, além de na avaliação do nível de atividade física segundo o número de passos/dia. A prevalência de síndrome metabólica foi de 27,8%, sendo maior entre mulheres, entre indígenas nas faixas etárias de 39-49 anos e 50 anos ou mais e entre aqueles com desempenho inferior no teste de resistência cardiorrespiratória, impulso horizontal e número de passos/dia. Os resultados indicam a necessidade de maior vigilância no controle e prevenção dos fatores de risco que compõem a síndrome metabólica.
