RESUMEN

Despite repeated campaigns promoting transplantation, the high donation refusal rate remains unchanged. We targeted a well-educated population to assess the impact of our current transplantation promoting programs and personal feelings toward new approaches to organ donation. A questionnaire was proposed in five universities to students and university staffs that would have been likely to benefit from previous information campaigns in two South American and three European countries. All of the 2321 people interviewed replied to at least one question. Organ shortage was considered as a serious public health issue. However, there was a widespread ignorance of religious precepts concerning transplantation that contributed to the low acceptance rate of organ sharing after death. Financial rewards for donors or their families remain controversial. There was a general agreement for early educational programs in schools. Most people still consider organ donation as a gift, but many would now agree to readily share body parts after death. This biased population of well-educated people has still little knowledge of organ donation. The negative impact of ignorance surrounding religious precepts and the high acceptance rate of educational programs in schools, justify supporting an intensive international effort in education that should also include Church leaders.

CONTEXT: Regarding the increasing gap between demand and supply of donor organs the question is increasingly discussed, if families of organ donors should receive a financial incentive for consenting to organ donation. However, little attention has been paid to the psychological consequences of such incentives. OBJECTIVE: We discuss the question of financial incentives for families of presumed organ donors in the light of relevant psychological theories. DATA SOURCES AND STUDY SELECTION: Overview of the psychological literature. Only well established theories were included. CONCLUSIONS: We summarise, that financial benefits for consent to organ donoation could affect adversely the public opinion toward organ donation and the whole process of transplantation and thus could counterproductively influence the consent rates.

RESUMEN

Ethical controversies in transplantation medicine are frequent and are not only discussed in the medical societies, but also by the pubic at large. Recently, the shortage of donor organs has led to a discussion of the commonly accepted ethical principles applied in transplantation medicine. In this paper, we highlight some of the thoughts expressed in the literature and discuss them in the context of transplantation.

RESUMEN

One of the main obstacles of organ transplantation all over the world is the insufficient number of available cadaveric organs. Various attempts are considered to increase the consent rate. Financial incentives as a possibility to raise the number of procured organs is a very controversial topic. Arguments for and against financial incentives are presented as well as the different models.
