RESUMEN

Experience of treatment of disorders of peripheral blood circulation in 120 patients with diabetes mellitus was summarized. Conservative treatment, conducted in 85 patients, included insulinization, preparations of sulfanilurea and glibenclamids, antibacterial therapy (beta-lactams, aminoglycosides, metronidazole) during 7-10 days, acidum acetylsalicylicum, trental, solcoseryl, vitamins. Amputation of lower extremity was done 35 patients, in 15 of them--on the hip level. Good result was noted in the treatment of diabetic foot with pronounced trophic changes in the third stage of ischemia when fraxiparine was applied locally.

RESUMEN

A total of 5,555 workers from 6 industrial enterprises in Cheboksary were screened. The most prevalent cause of death appeared to be violent death which encompassed deaths caused by accidents, poisonings, injuries, suicides and comprised 35.4 percent of the overall pattern of mortality causes. In 76.8 percent of cases violent death was caused by alcohol abuse. Deaths from alcohol poisoning during a 9-year period comprised 11.3 percent of total causes of death. The second place by prevalence was given to ischemic heart disease, including sudden coronary death (20.9%) and moreover almost in each case death was developing as a result of alcohol abuse. In 1986 after the adoption of the Decree of the Central Committee of the CPSU in May 1985 "On the measures to combat heavy drinking and alcoholism", the incidence of violent deaths caused by alcohol abuse, as compared with midyear rates for previous 8 years decreased by 4.7 times (p less than 0.01) which contributed to the development of a marked trend towards the decline in general mortality rates by 31.4 percent. The results obtained showed that despite certain shortcomings, measures targeted to combat heavy drinking and alcohol abuse promoted the prevention of death among men of productive age.

RESUMEN

An attempt is made to demonstrate a relationship between changes in arterial blood pressure and sex steroids, gonadotrophins and mineralocorticoids in 30- to 55-year-old females with essential hypertension, employed in textile industry in Cheboksary. Climacteric and menopausal patterns of sex steroid and gonadotrophin secretion are discussed. A definite relationship between essential hypertension, elevated estradiol and testosterone and lowered gonadotrophins is demonstrated. It is suggested that climacteric and menopausal blood pressure disorders may be related to changes in gonadal and adrenal activity.

RESUMEN

The effect of active arterial-hypertension preventive programs on the epidemiologic situation was studied over a nine-year period, 1977 through 1985. In the first five years, the number of effectively treated cases increased, while essential hypertension morbidity associated with temporary disability diminished, as did mortality and the incidence of myocardial infarction and cerebral accidents among 40- to 54-year-old male industrial workers. After an active preventive program was abandoned at one factory, the parameters examined deteriorated considerably, and a sample selected for comparison showed an abrupt drop in treatment efficiency within three years after annual screenings were discontinued.

RESUMEN

Standard ECG recording, prolonged ECG monitoring and bicycle ergometry were used in 329 coronary patients. Ventricular extrasystole of grades 2-5 (Lown's classification) was diagnosed in 126. The patients were randomly allocated to the main and control groups. Prolonged continuous treatment of the main-group patients with propranolol and other anti-arrhythmic drugs resulted in reduced occurrence of arrhythmic episodes, and lower heart rates and arterial blood pressure as well as alleviation of clinical anginal symptoms, as compared to the controls treated in local outpatient clinics.

RESUMEN

In the course of five years, arterial hypertension was epidemiologically studied in a population of industrial workers (n = 7883), divided into 3 groups according to the kind of preventive intervention. In the active prevention group a statistically significant improvement of the epidemiological situation was observed. A certain improvement in the percentile distribution of the diastolic pressure level was also attained in the control group in which detected hypertensive persons were referred to routine policlinical care without special cardiological consultation. In the second control group, in which no measures were applied, the indicators of the prevalence of hypertension and of the effectiveness of treatment have worsened. Regression analysis showed a dependence of subsequent values of diastolic pressure on preceding values.

RESUMEN

Altogether 3875 women aged 30 to 50 engaged in the textile industry, were examined. Primary prevalence of arterial hypertension was 7.0-7.2%. The implementation of a special program of active prophylaxis resulted in a significant rise of the number of effectively treated patients as compared to the control group.

RESUMEN

One-hour monitoring and bicycle ergometric testing revealed frequent (more than 10 per hour) and complicated ventricular extrasystoles in 105 patients. They were distributed between the treatment and control groups. Long-term treatment with beta-blocking agents reduced the frequency of sudden death and repeated myocardial infarction in the treated group as compared to the controls.

RESUMEN

Epidemiological examination of males from 40 to 54 years of age at nine industrial enterprises revealed arterial hypertension in 19.5% of cases. As a result of intensive treatment of patients in a prophylactic cardiological department the incidence of arterial hypertension reduced, the patients learned more about their disease, the efficacy of treatment improved, and there was a decrease in losses of working time due to temporary disability of workers.

RESUMEN

The results of a 3-year epidemiological study of the population according to the standard program of the myocardial infarction register elaborated by WHO (1970) are discussed. The data on the morbidity and mortality in myocardial infarction and acute coronary insufficiency among the population ranging in age from 20 to 64 years are shown. The study was conducted according to the standard program employing common criteria for the diagnosis of myocardial infarction and acute coronary insufficiency.
