OBJECTIVES: To study the influence of environmental pollution on the mineralization of dental hard tissues by using biosubstrates: teeth and hair. : STUDY DESIGN: At the first stage epidemiological survey was conducted in polluted and less polluted areas of Tbilisi (Georgia). We studied 525 children aged 3 and 4 years. Caries prevalence and intensity was defined by the methodology of World Health Organization. At the second stage the chemical elements content was studied in hair and teeth hard tissues of 24 children by X-ray fluorescent spectroscopy method. RESULTS: The prevalence of dental caries in the polluted region was 46%, caries intensity--1.92 (± 2.842). In the less polluted region prevalence was 37%, caries intensity--1.47 (± 2.571). These data are statistically reliable (p < 0.05). The study of hair and tooth tissues showed that the toxic elements (Pb, Hg, Sn, Ti) content in these tissues was higher in environmentally unfavorable than in favorable conditions. CONCLUSION: Hair and dental tissues can be used as indicators of environmental pollution. Our survey showed that toxic elements content in dental hard tissues was higher and the level of essential elements was less in polluted than in less polluted areas of Tbilisi.

OBJECTIVE: To define the risk of development of periodontal diseases at vitamin D-resistant rickets. The material for the study was based on the data from 13 children of 0-18 years with vitamin D-resistant rickets (I group), 68 children with vitamin D-dependent rickets (II group), and the control group included 61 children of the same ages. The patients were divided into 3 age groups: I group of 0-5 years (deciduous/milk teeth occlusion), II group of 6-12 of years (mixed bite), III group of 13-18 years (permanent occlusion). For the qualitative data the differences between the groups were detected by using F criteria but for the quantitative data the differences between the groups were detected by Student's t test for independent selection. Mathematical processing was performed using statistical software SPSS-22. In all three groups the gingivitis of mild and moderate forms were fixed, among them acute course was revealed only in the group of vitamin D-resistant rickets. The rate of chronic, local, catarrhal and generalized gingivitis in the group of vitamin D-resistant rickets was reliably higher in comparison with both vitamin D-dependant rickets and control groups. In addition, a mild form of periodontitis with chronic course was revealed in all three groups the rate of which was reliably higher in the group of vitamin D-resistant rickets comparing with the ones of vitamin D-dependant rickets and control groups. CONCLUSION: the rate of inflammatory periodontal diseases in children with vitamin D-resistant rickets is higher than in children with vitamin D-dependent rickets and the control groups.

Child's organism is a kind of marker of increased sensitivity to environmental conditions. Lack of vital (essential) elements may lead to disease occurrence, caused by deficiency, excess or imbalance of micro- and macro elements in the organism. To assess the level of trace elements content in the human body, the hair is a good informative material as it concentrates trace elements and most fully reflects the level of both toxic and essential elements content. In order to conduct a comparative analysis of the elemental composition of hair in children and assess the relationship between the elemental composition of hair and the state of oral health status, we have examined 28 patients who underwent simultaneous determination of 28 chemical elements in the hair. According to the obtained data, in elemental composition of hair in children of group I, living in the regions with moderate environmental contamination, there was a minimal decrease of essential elements (except Ca), a slight increase of toxic chemical elements - Pb and Cd and decompensated form of dental caries have been observed. Investigation of the second group of children living in environmentally unfavorable regions showed that children with decompensated form of caries prevalence are characterized by the significant reduction of hair essential chemical elements in average of 28- to 56% and by a high content of toxic elements such as Pb, Hg, and Zr. The fact indicates enhanced excretion of essential elements under the influence of heavy metals, which in its turn affects the whole body and dental health.

The influence of eco-pathogenic factors on mineralization of teeth hard tissues at early age was studied in 525 young children, 3 to 4 of ages. Tooth caries index was detected according to the indexes provided by WHO (World Health Organization): prevalence, intensity (dmf) and Significant Index of Caries (SIC). The studies revealed that the average rate of dental caries for studied group living in regions with ecologically favorable conditions is relatively lower than in ones living in regions with unfavorable conditions and equals to 39.6% and 63.6%, respectively. Comparative analysis of average and significant indexes ​ of dental caries showed that the difference is minimal in children population living in favorable conditions (1: 1.7) in comparison with the children living in ecologically contaminated regions, where the significant index exceeds the average one approximately three times as much. According to the obtained results, it should be concluded that the dynamics of dental caries is mostly expressed in young children living in regions where air-pollution (with chemically aggressive substances) index several times exceeds the maximum allowable coefficient. Consequently, development and implementation of the special programs for caries-prevention in children population with low rate of caries-resistance, living in the above-mentioned conditions is of great importance.

RESUMEN

According the demographic changes taking place in the world we are experiencing the process of radical aging. It is very essential to evaluate the quality of orthopedic treatment. For this purpose we should identify the prosthesis of different constructions of the patient's mouth cavity. It is very important to evaluate also their availability and validity. The necessity of orthopedic treatment in 50-59 age groups was equal to 54% and in 80-89 age groups this number was increased to 84%. In case of loosing all the teeth only qualitative prosthesis can guarantee normal crunching of the food and avoid the tract disease of stomach-intestine.

The aim of the study was to define interrelations between complications after veneer treatment with initial parameters of an oral cavity, state of an organism, social and biological factors. 65 patients with veneers treated between 2005 and 2009 at Margvelashvili Dental Clinic (Tbilisi) were investigated. Correlation analysis on Spearmen was spent with application of the package statistical programs SPSS 11.5. The purpose of the research is to study local immunology of mouth cavity at complete prostheses with acrylic and polypropylene bases. Immunological parameters of saliva (sIgA, IgG and IgE), lysozyme activity at complete prostheses of teeth with acrylic and polypropylene bases in 40 patients from 45 to 75 were investigated. Lysozyme activity of mixed saliva was defined by turbidimetric method, concentration sIgA, IgG, and IgE - by method Mancini. The statistical data processing included calculation of average arithmetic sizes (X) and standard deviation. The authentic distinctions in immune parameters between groups were calculated (Student's coefficient). The analysis of the data was carried out with the help of a package of the applied programs SPSS 11.5. The comparative analysis immune of parameters of a saliva after complete prostheses with acrylic and polypropylene bases has shown that in a case acrylic of basis in comparison with polypropylene base, parameters IgA and lysozyme activity there are less, is higher IgG and IgE and factor immune of equation. Thus, after prostheses with acrylic of basis it is marked immunological shifts that can be infringement allergy by action acrylic base.

The world's population is aging at an accelerated rate. People aged 65 and over now comprise a greater share of the world's population than ever before, and this proportion will increase during the 21st century. Increased lifetime in old age is combined with a concern about its quality. Oral health care of the elderly is one of the important aspects affecting the quality of life. The aim of the present study was to describe stomatologic status among middle-aged and elderly people in western Georgia. 355 subjects aged 55-90 years old were surveyed. According to oral health survey data, a high prevalence and intensity of dental caries and periodontal disease were apparent. Prosthetic status and treatment need were recorded in standard condition using oral health assessment forms. It is concluded that a high percentage of elderly were dentate but the average number of teeth was low and their status was poor, with very high need for therapeutic and prosthetic treatment.

Efficiency of bioactive osteoplastic composition HA-phage at a surgical stage of combined treatment of chronic generalized periodontitis was examined. The results obtained in the control and experimental groups proved that HA-phage allows reducing the time of treatment and to achieve remission of the disease. Out of 92 patients with chronic generalized periodontitis 52 undertook microplasmic scalpel-irradiator in complex treatment. The treatment course comprises 3-5 procedures. The quantity of manipulations is determined by the pathological process. Before and after the treatment the radiographic investigations were carried out in persons of both groups. 18 months later after the operation a bone regeneration was shown in 87,8% cases in the investigated group and 60,1%--in the control group patients. The therapy has shown positive results. So, the new method of treatment of chronic generalized periodontitis may prove to be more practical in clinical dentistry. Currently, it is possible to isolate highly purified BMSSCs from bone marrow using STRO-1. The management of periodontal defects has been an ongoing challenge in clinical periodontics. This is mainly a result of the fact that the tissues which comprise the periodontium, the periodontal ligament, and the cementum and alveolar bone represent three unique tissues in their own right. Thus, reconstruction of the periodontium is not just a simple matter of regenerating one tissue but involves at least three quite diverse and unique tissues. Taking into account the results of recent studies we hypothesize that mesenchymal stem cells transplantation should be a perspective approach in the treatment of periodontal defects. For this purpose we plan to perform an experimental investigations which will include an induction of periodontitis, purification of the bone marrow stem cells and their transplantation.

The degree of adhesion and colonization was studied among 27 patients with generalized paradontitis of moderate severity and in remission stage. All these patients had different material teeth prostheses. 12 patients had acrylic plastic "Ftorax" material prostheses and 15 of them polypropylenes prostheses. Prostheses prepared from acryl plastic were intensively colonized by stable microflora of oral cavity, as well as by paradontopathogens, whereas usage of polypropylenes prostheses is rather effective for patients with paradontitis, because it is followed by partial recovery of stable microflora and lack of secretion of parodontopathogenic types, such as actynomycetes, prevotellae, fusobacteria.

Calcium and Phosphorus are the basic elements in making the inorganic matrix of bone tissue. With the purpose of our research was to study the local effect of hydoxypatite and Platelet Rich Plasma (P.R.P.) on the course of reparative osteogenesis. We used hydroxyapatite granules made by the firm "Polystom" with the chemical formula Ca(10)(PO(4))(6)(OH)(2). Platelet Rich Plasma we have got from blood of rabbits by centrifuging. Experiment was made on 30 rabbits. by operation the standard defects were created with the diameter of 66mm and the depth of 4-5mm on the mandibular bone of the rabbits. In the laboratory group defects were filled with hydroxyapatite and P.R.P., in control group with their own blood. After 7,14,21,35 and 50 days after operation three rabbits were taken from each group. Mandibular bone fractions with defects were relieved from soft tissue, were fixed in 10% neutral formalin and were stained with hematoxyn-eozine. The study showed that in the study group at the initial stage of differentiation from the moment of traumatization the pre-osteoblast and osteoblast contour permanently appears. And in the control group the resorbtion processes were clearly expressed. At the end of the experiment of 50 days in the experimental group the compaction of osteogenic structure was observed in all defect areas. Newly generated tissue underwent compaction. The results of the experiment prove that the hydroxypatite and Platelet Rich Plasma has a stimulative role in the process of osteogenesis. Activation of the reparative processes is far more intensive at the final stage of osteogenesis.

The aim of the study was to compare the serum levels of interferon-gamma (INF-gamma) and neopterin (NPT) in 10 patients with mild form of chronic periodontitis before and after operations. Overall objective of these operations is the elimination of periodontal and osseous pockets. 10 blood donors were included in the control group. The serum levels of INF-gamma and NPT were measured using enzyme immunoassay technique. INF-gamma and neopterin in patients with chronic periodontitis were assessed before and 4 to 6 weeks after the operation. The mean serum INF-gamma in patients with mild form of chronic periodontitis (n=10) was 13,2+/-4,03 pg/mL. The mean serum INF-gamma level in patients with mild form of chronic periodontitis (n=10) after the operations was 10,05+/-3,12 pg/mL. Serum levels of INF-gamma in patients with mild form chronic periodontitis were significantly higher than those in the control group. There was no significant difference in serum levels of INF-gamma in patients with mild form before and after operations. The mean serum level of NPT in the control was 5,3+/-2,39 nmol/L. The mean serum NPT level in patients (n=10) with mild form chronic parodontitis was 14,4+/-10,05 nmol/L. After operations NPT concentration were reduced (6,9+/-3,39 nmol/L). Serum levels of NTP in patients with mild form chronic periodontitis were significantly higher than those in the control group. Significant difference was observed in serum levels of NPT in patients with mild form of chronic periodontitis before and after operations. Obtained data allow specifying the mechanism of positive influence of autograft operations on the pathogenesis of chronic periodontitis.

The aim of the study was to compare the serum levels of interferon-gamma (INF-gamma ) and neopterin (NPT) in 20 patients with chronic periodontitis and the control group (10 blood donors). The mean serum INF-gamma level in chronic periodontitis patients (n=20) was 13.08+/-4.6 pg/mL, in the I group (n=10) it was equal to 12.09+/-4.8 pg/mL; in the II group (n=10) - 13.2+/-4.03 pg/mL. Serum levels of INF-gamma in patients with chronic periodontitis were significantly higher than those in controls (p=0,024). Significant difference was observed in serum levels of INF-gamma between patients with moderate form and control group (p=0,043). There was no significant difference in serum levels of INF-gamma between patients with mild form and the control group (p=0,11). There was no significant difference between patients from I and II groups (p=0.9). The mean serum level of NPT in the control was 5.3+/-2.39 nmol/L. The mean serum NPT level in patients (n=20) with chronic periodontitis was 11.83+/-7.5 nmol/L, in I group it was equal to 9.27+/-1.84 nmol/L; in the II group -- 14.4+/-10.05 nmol/L. Serum levels of NTP in patients with chronic periodontitis were significantly higher than those in controls (p=0.001). Significant difference was observed in serum levels of NPT between patients with mild form and control group (p=0.0006). There was significant difference in serum levels of NPT between patients with moderate form and the control group (p=0.02). There was no significant difference between patients from the I and II groups (p=0.13).

RESUMEN

Special working features of stomatology distinguishes it from other medical disciplines. This makes necessary to establish ergonomic environment of work for dentists and follow all necessary principles. The paper presents the major indices or ergonomic expertise in stomatologic institutions of Tbilisi, Georgia. It was shown that 43% of doctors-stomatologists express their displeasure by the working conditions. Almost half of the doctors suffer form various diseases which are caused specifically by labor of stomatologists and by unsatisfactory labor conditions. In these organizations the major principles of the President (No 564 of 01.10.1999) and the Minister of Health of Georgia (No309\N of 05.11.2002) are violated, which affects the quality of the jobs performed by the doctors and thus infringes the rights of patients.

A total of 288 patients (100 females and 188 males) aged 25 to 60 with prognathic and progenic occlusion were examined and treated. Dentition deformations were revealed in 182 (63.1%) patients. Clinical and x-ray signs of traumatic occlusion were observed in 219 (76.04%) cases. Abnormal mobility of teeth was detected in 181 (79%) subjects with prognathic occlusion, dentogingival and osseous pouches in 164 (71.6%) subjects. Reduced height of occlusion was found in 68 (25.3%), dysfunction of the temporomandibular joint in 19 (8.2%) cases. Traumatic occlusion was observed in 38 of the 59 patients with progenic occlusion. In sagittal abnormalities of occlusion, traumatic occlusion was associated with dysfunctions of the masticatory muscles and of local circulation in the periodontium, which were recorded by electromyography and rheoparodontography. The treatment was carried out by therapists, surgeons, and orthodontists. Orthodontic treatment consisted in levelling of the occlusion surface of dentitions, orthodontic preparation of the maxillodental system, fitting with splints and prostheses. Remote results in periods from 1 to 3 years were good in 94.8% of patients.

Altogether 568 patients, 308 women and 260 men aged 30 to 64 with abnormal teeth abrasion were examined by clinical, anthropometric, X-ray (spot and panoramic radiography of the teeth and jaws, tomography of the temporomandibular joint), electromyography, and rheoparodontography. Pathological teeth abrasion was associated with various morphological and functional disturbances in the maxillodental system. Two types of abrasion were distinguished: with and without masticatory function disorders. The latter condition was detected in 252 (44.36%) patients. Painful dysfunction of the temporomandibular joint was observed in 66 (11.6%) patients. Three factors contribute to the pathogenesis of these functional disorders in abnormal teeth abrasion: reduction of occlusion height (interalveolar distance) and shortening of the lower third of the face, blocking of mandibular movements, lateral and distal shift of the mandible.

RESUMEN

A total of 262 patients aged 32 to 58 were examined and 246 patients aged 30 to 60 were treated. Of the 944 cermet crowns examined complications were found in 88 (9.34%). Ceramic fractures occurred in 20 crowns, inflammatory processes in the gingiva developed in 22 cases, functional overexercise of the periodontium in 21, failure of cement fixation in 18, apical periodontitis in 7 cases. The causes of these complications are analyzed and measures to prevent them worked out. If all preventive measures were adhered to in the treatment of 246 patients fitted with dentures, complications occurred three times more seldom (in only 3.14% of cases).

RESUMEN

Orthodontic treatment using cermet dentures was carried out in 496 patients aged 26 to 60. Therapeutic surgical and orthodontic treatment of the maxillodental system was carried out before denture fixation. Late results in periods from 1 to 5 years were followed up in 396 patients. No complications were observed in 384 (96.9%). Only 12 patients developed complications: porcelain fracture in 4, periodontal inflammations in 5, and a partial relapse of occlusion reduction and distal dislocation of the mandible in 3.

RESUMEN

A total of 598 patients with various occlusions and dentition defects were examined; 112 (18.7%) of these presented with distal dislocation of the mandible. Shortening of interalveolar distance was found to play an important role in the pathogenesis of this dislocation. This shortening occurs in abnormal teeth abrasion and loss of many molars and premolars maintaining the occlusion height. Masticatory function was found disordered in 56 (50% of patients with distal dislocation of the mandible. Orthodontic treatment of these patients was carried out by sagittal shift of the mandible to a distance of 1 to 2 mm and subsequent fitting with prostheses. Good results were attained in 94.55% of cases.
