RESUMEN

The level of serum leptin was studied in 20 patients before and after liposuction carried out for excessive subcutaneous fatty tissue. The dynamics of indices depending on the volume of the ablated fatty tissue was analyzed. It was found that in most patients the level of blood serum leptin correlated with the body mass and fatty tissue index. A removal of limited volumes of fatty tissue results in the elevated level of blood serum leptin. The aspiration of great volume of fatty tissue is followed by the primary decrease of the leptin level which further reaches the initial level and even surpasses it.

RESUMEN

On the own experience and medical practice the authors summarize the peculiarities and advantages of a new method of human skin recovery. Keratinocyte sheets greatly improve the treatment of allogeneic cell cultures' accelerates wound healing of the second degree deep burns. The efficacy of the method can be improved by cultivation and follow-up grafting of keratinocytes on the surface of microcarriers.

RESUMEN

The method of cultivation of keratinocytes from burned patients allows obtaining a new type of wound coverings having advantages over those used at present in clinical practice. The use of autologous cell elements results in a rapid and perfect taking of the transplanted layers. The method allows a more rapid (3-4 weeks after trauma) and more efficient (the plasty coefficient 1:100 and more) recovery of the skin as compared with traditional methods of treatment. Long-term results of such plasty were satisfactory.

RESUMEN

On the basis of experimental and clinical investigations the authors have shown that extensive burns in patients with true deficiency of donor resources might be treated by the method of micro-transplantation of the skin which consists in plasty of the wounds by pieces of auto-skin of small size. The reduction of size of the skin pieces gives the enlargement of the total line of marginal epithelialization and great plasty coefficient as compared with traditional methods of surgical treatment.

Experiments with rats of different ages have established the potential of microautotransplants (MATR) of skin (0.5-3.0 mm2 in size) to produce neoepidermis on fresh and granulating wounds. The MATR induced a 40-fold increased rate of closing of the wound surface. Contraction of the re-epithelialized wounds followed the same trend found for an open wound. The rate of contraction of re-epithelialized wounds depends on the age of the rat and the degree of maturity of the granulation tissue. The most extensive contraction was observed in young animals. Neoskin does not contain skin appendages, shows increased exfoliation of horn flakes from the neoepidermis surface and contains epitheloid cysts filled by a horny substance. The use of the MATR technique in the clinic also leads to wound re-epithelialization. Human neoskin histologically is similar to that of animals. These results suggest that the MATR technique is an alternative approach in the treatment of extensive skin burns.

RESUMEN

Transplantation of autologous bone marrow was performed in 62 patients with carcinoma of the thoracic portion of the esophagus for prophylactics of disturbance of hemopoiesis and severe infectious complications in the postoperative period. With the help of myelokariocytopheresis procurement of the bone marrow was made 2-3 days before starting preoperative radiotherapy. After exfusion of the bone marrow mixture erythrocytes were separated and returned completely and immediately to the patient, myelokaryocytes being frozen. The volume of the bone marrow suspension from 1 patient made up on the average from 300 to 800 ml, the amount of the bone marrow cells in it from 4.5 to 11.2.10(9). Studies of the dynamics of concentrated and volumetric parameters of blood were performed. The functional specific features of the bone marrow erythropoiesis were estimated in the sternal punctate. The use of automyelotransfusion substantially reduced the frequency of infectious complications and, in the author's opinion, it is a fairly effective method of prophylactics and treatment of postoperative anemia.

RESUMEN

Results of 67 plasties with fasciocutaneous flaps in 59 patients operated upon for burns and their sequellae are described. The correlation of length and width of the flaps was from 1.5:1 and 3:1. Good results were obtained in 90% of the patients. Causes of unfavorable outcomes are analyzed. The method is recommended for the introduction into practice of treatment of burns and their sequellae.

On the basis of studying the concentration and volume indices of the erythron, iron plasmic pool, proliferation activity of the erythron and iron kinetics in 137 patients operated for pulmonary diseases the authors note a depression of the bone marrow red stem as a characteristic feature of postoperative anemia. A scheme of the main steps of postoperative anemia is presented.

RESUMEN

The authors have shown hyporegeneratory character of anemia developing after operations on the lungs followed by massive blood loss which was adequately compensated by conserved homologous blood. The transfusion of the autologous bone marrow prepared before operation results in a pronounced stimulation of production of erythrocytes, contributing to the quickest recovery of the circulating erythron.
