RESUMEN

Substitution of lateral malleolar ligament with a periosteal flap Authors describe their operative procedure for chronic instability of the lateral malleolar ligament: substitution of this ligament with a periosteal flap. This operation is suitable, according to follow up examinations, both in isolated talofibular and talofibular-calcaneofibular ligamental deficiencies, for the stable restitution with good function of the talorural joint. The advantage of the operation is that less burden is imposed on the patient, the technique is relatively simple, and the need of instruments is minimal. Control examinations have shown very good or good results in the majority of 16-49 years old operated patients.

RESUMEN

Authors report on the functional bracing of the fractured humerus. The advantage of the method is the preserved muscle and joint function with the use of a brace. This means ease for the patient in respect of the personal hygiene and living conditions. In elderly patients all other type of fixation around the chest means a substantial danger to the patients life because of the breath's movements depression.

RESUMEN

Authors describe their new combined method for treatment of lower leg fractures. To this a new type fixator was developed which makes dynamization also possible. The other novelty in this country is the use of functional external fixing brace. In all of the 38 patients they had succeeded to obtain healing. No complication was found until now.

RESUMEN

Scintigraphy with bone-specific 99m-technetium-Sn-ethylenediphosphonate (phosphon) was performed in 27 patients having undergone total hip replacement before and, in case of complaints, also after operation. On suspicion of infection a study with 67 gallium citrate was also made. Completion of clinical, roentgenological and laboratory findings with an isotope study may help in deciding whether complaints are only due to the loosening of the prosthesis or to infection.

RESUMEN

A useful technique for the investigation of skeletal fractures is the application of 99mTc phosphon isotope. This method may complete the X-rays and physical examinations, and it is especially useful in cases, when the date of vertebral compression is unknown.

RESUMEN

5 year follow up assessment of 224 malleolar fractures showed that the prognosis was significantly worse if they were associated with fractures of the anterior or posterior tibial rim. In cases without tibial rim fractures severe late x-ray and clinical changes were found only below 4%, while with small marginal fragments they were below 20% and with greater fragments above 30%. Chances are worse in tibial rim fractures even with no or minimal involvement of the joint surface. This is explained by the fact that forces causing fracture-dislocation are more detrimental for the joint surface in the sagittal than in the lateral plane.

RESUMEN

Based on review examination of 224 patients 5 years after their ankle fractures, the authors demonstrate a significant worsening of prognosis with fractures of the anterior or posterior tibial margin. In cases without marginal fractures severe late X-ray, or clinical changes remained less than 4%, in cases with small marginal fractures their number was under 20%, with great articular surface fractures however over 30%. The unfavourable trend becomes evident even in marginal fractures not extending to the articular surface or involving only a minor part of it. It is concluded, that a dislocating force in the sagittal direction is more damaging for the joint than a force acting in the lateral direction.

RESUMEN

Authors with regard to primary or delayed surgical treatment analyse the mortality of 2,612 patients with proximal femur fractures treated in the National Institute of Traumatology during the years 1971--75. Within this material 2,055 case history of surgically treated femoral neck and pertrochanteric fractures are evaluated. An important difference in favour of primary intervention was found between the mortality of primary and delayed operations both in femoral neck and pertrochanteric fractures. In pertrochanteric fractures the mortality increased gradually starting from the day of injury. Regarding age related mortality, there was a significant difference in each age group in favour of primary operations. Thus the increased mortality encountered after delayed operations can't be explained by a selection with regard to age.
