INTRODUCTION: Although the prevalence of ragweed allergy in Europe is still low, it is increasing according to recent statistics. In contrast, in Hungary ragweed is the most common allergen with very high sensitisation in the general Hungarian population. AIM: We investigated the number and clinical characteristics of ragweed allergy in the Hungarian population, as the first comprehensive study to evaluate ragweed allergy in Hungary. METHOD: One thousand Hungarian adult subjects were screened with a questionnaire for ragweed allergy. People with ragweed allergy answered further specific questions about their disease history, symptoms and medication use. RESULTS: 305 subjects reported allergy from which 218 patients had symptoms during ragweed pollination suggesting ragweed allergy. 40% of these had symptoms for more than 5 years. Asthma was reported in 18.3%. Around 24% of these patients were undiagnosed; they did not take any medications or visited professionals because of their symptoms. Compared to the non-ragweed allergic patients, subjects with ragweed allergy were older and better educated, but the dominance of female gender was less prominent than in the non-ragweed group (all p<0.05). CONCLUSIONS: Around 22% of the Hungarian population suffers from ragweed allergy. Studies should focus on eradication strategy and improvement of patient care. Orv. BACKGROUND: The casual and severity distribution of allergic rhinitis (AR) in Hungary is unknown.The aim of this survey was to evaluate symptom perception, disease severity, concomitant asthma frequency and the impact of AR on everyday life activities in a cross-sectional, multicenter study in Hungary under the supervision of Hungarian Respiratory Society. METHODS: Data were recorded by 933 AR patients (65.93% women) and their treating specialists. The perceptions of patients regarding the symptoms (nasal, ocular and others) of AR and its severity, together with its impact on everyday life were assessed. Physicians recorded data regarding the diagnosis and severity of AR, and comorbidities. RESULTS: 52.5% of patients suffered from seasonal AR, 35.1% from perennial AR. A large proportion of patients had moderate to severe disease (MS-AR) (57.34%), persistent disease (98.0%) and concomitant asthma (53.32% in the mild, 57.52% in the MS-AR group). MS-AR was more frequent among women. Despite the treatment used, in MS-AR the proportions of patients reporting moderate to severe rhinorrhoea, nasal obstruction, ocular itching/redness, watering, itchy throat and sneezing were as high as 52.0%, 54.0%, 33.8%, 26.5%, 44.0% and 31.2%, respectively. Overall, there was a poor agreement between disease severity reported by patients and specialists. The adherence to oral antihistamines and intranasal corticosteroids was found to be between 50 and 65%; mostly depending on the dosage form. CONCLUSIONS: AR remains a significant health problem in Hungary because of the burden of symptoms, high rate of concomitant asthma and the significant proportion of MS-AR affecting general well being.

Noninvasive imaging methods can be valuable tools for diagnosing thoracic diseases, especially malignancies. The aim of this study was to compare the effectiveness of conventional and virtual bronchoscopy in the follow-up of patients with large airway stenosis. Twenty-three consecutive patients with stenoses of the trachea and/or the main bronchi were enrolled in this prospective observer study. The causes of stenosis included malignant or benign tumours, goiter, and postintubation stenoses. Patients were evaluated before and after treatment (which included mechanical dilation, laser photocoagulation, stent implantation, radiotherapy, chemotherapy, and surgical resection). The mean time between baseline and follow-up endoscopy was 140 days. No significant differences were observed between the estimated and measured data from bronchofibroscopy and virtual bronchoscopy. Exact measurement of stenoses was performed with virtual bronchoscopy.

The aim of the study was to compare the utility and effectivity of conventional and virtual bronchoscopy in the follow up of patients with main airway stenosis. 23 patients with trachea or main bronchi stenosis were enrolled in the study. The causes of the stenosis were malignant or benign tumor, struma and stent. The stenoses were evaluated before and after the different interventions (i.e. mechanical dilatation, laser photocoagulation, stent implantation, radiotherapy, chemotherapy or surgical resection.) The mean time between the first and second endoscopic and virtual bronchoscopic examinations was 140 days. The authors found good correlation between bronchofiberoscopy and virtual bronchoscopy findings. The data of virtual bronchoscopy were more exact. Based on this study the virtual bronchocopy can replace the traditional invasive method to follow-up patients with main airway stenosis.

RESUMEN

Allergic rhinitis and bronchial asthma are characterized by an Ig-E-mediated allergic reaction of the upper and lower airways. The Hungarian and international epidemiological data show significant increase of prevalence of these diseases with a relevant increase of the cost producing a relevant socioeconomic problem. Authors present the frequent inhalative allergens, summarize diagnostical algorithms and therapy based on international therapeutic guidelines.
