Ulcerated lesions have been observed in SARS-CoV-2 patients and their treatment is a challenge. We aim to report the treatment of oral lesions with low-level laser therapy (LLLT) in a patient with SARS-CoV-2. The patient diagnosed with COVID-19 was admitted to the intensive care unit, on mechanical ventilation, and on a nasogastric tube diet that persisted due to oral lesions. Painful ulcerated lesions, clinically diagnosed as viral ulcer, were found on the tongue, palate, labial commissure, and oropharynx during the oral evaluation. An antimicrobial photodynamic therapy with methylene blue and red laser (high energy) was performed, followed by treatment with LLLT with red laser. Significant clinical improvement of the lesions was observed after seven sessions of LLLT, with the oral diet being reestablished. Oral mucosal lesions in patients with COVID-19 are usually symptomatic and with onset after systemic symptoms. LLLT has been used in the management of these lesions, with promising results.

Bovine papillomavirus (BPV) infects cattle cells worldwide, leading to hyperproliferative lesions and the potential development of cancer, driven by E5, E6, and E7 oncoproteins along with other cofactors. E6 oncoprotein binds experimentally to various proteins, primarily paxillin and MAML1, as well as hMCM7 and CBP/p300. However, the molecular and structural mechanisms underlying BPV-induced malignant transformation remain unclear. Therefore, we have modeled the E6 oncoprotein structure from non-oncogenic BPV-5 and compared them with oncogenic BPV-1 to assess the relationship between structural features and oncogenic potential. Our analysis elucidated crucial structural aspects of E6, highlighting both conserved elements across genotypes and genotype-specific variations potentially implicated in the oncogenic process, particularly concerning primary target interactions. Additionally, we predicted the location of the hMCM7 binding site on the N-terminal of BPV-5 E6. This study enhances our understanding of the structural characteristics of BPV E6 oncoproteins and their interactions with host proteins, clarifying structural differences and similarities between high and low-risk BPVs. This is important to understand better the mechanisms involved in cell transformation in BPV infection, which could be used as a possible target for therapy.

BACKGROUND: The incidence of visceral leishmaniasis (VL) has increased in the Southern region of Brazil in recent years, especially in the State of Paraná. New species have been suggested with potential to act as vector in VL endemic areas. OBJECTIVES: Identify the Leishmania species in sand fly specimens collected from 2016 to 2018 in the municipality of Itaperuçu, Vale do Ribeira, Paraná, Brazil. METHODS: Light traps were used for collections and for the analysis of sand fly were used the multiplex polymerase chain reaction (PCR) methodology and subsequent sequencing. FINDINGS: Among the collected specimens, 88.62% were attributed to the species Nyssomyia neivai, which were grouped into 176 pools. Three positive pools were detected: two with Leishmania (Viannia) braziliensis and one with L. (Leishmania) infantum. The positivity rate for the parasite was 0.25% based on the presence of at least one infected insect in the pool. MAIN CONCLUSIONS: The detection of L. infantum in Ny. neivai draws attention due to its abundance and anthropophily in the State of Paraná. Moreover, this finding is considered as an alert and suggests that the vector competence of Ny. neivai and the criteria for its incrimination should be carried out, given its wide distribution in southern of Brazil.

PURPOSE: To evaluate the effects of the experimental subcutaneous Walker-256 tumor and L-glutamine supplementation, an antioxidant, on the glomerular morphology of rats. METHODS: Twenty Wistar rats were distributed into four groups (n = 5): control (C); control treated with 2% L-glutamine (CG); rats with Walker-256 tumor (WT); and rats with Walker-256 tumor treated with 2% L-glutamine (WTG). Renal histological samples were submitted to periodic acid-Schiff and Masson's Trichrome staining to analyze glomerular density, morphometry of glomerular components and glomerulosclerosis; and to immunohistochemistry for fibroblast growth factor-2 (FGF-2). RESULTS: WT showed 50% reduction in body mass gain and cachexia index > 10%, while WTG demonstrated reduction in cachexia (p < 0.05). WT revealed reduction of glomerular density, increase in the glomerular tuft area, mesangial area, matrix in the glomerular tuft, decrease in the urinary space and synechia, and consequently higher glomerulosclerosis (p < 0.05). L-glutamine supplementation in the WTG improved glomerular density, and reduced glomerular tuft area, urinary space, mesangial area, and glomerulosclerosis compared to WT(p < 0.05). WT showed higher collagen area and FGF-2 expression compared to C (p < 0.05). WTG presented lower collagen fibers and FGF-2 expression compared to WT (p < 0.05). CONCLUSIONS: L-glutamine supplementation reduced cachexia and was beneficial for glomerular morphology of the rats, as well as it reduced kidney damage and improved the remaining glomeruli morphology.

BACKGROUND: Arrhythmias are one manifestation of the cardiotoxicity that results from doxorubicin (Doxo) administration. Although cardiotoxicity is an anticipated outcome in anticancer therapies, there is still a lack of treatment options available for its effective management. This study sought to evaluate the possible cardioprotective effect of complex d-limonene (DL) plus hydroxypropyl-ß-cyclodextrin (HßDL) during treatment with Doxo, focusing on the arrhythmic feature. METHODS: Cardiotoxicity was induced in Swiss mice with Doxo 20 mg/kg, with 10 mg/kg of HßDL being administered 30 min before the Doxo. Plasma CK-MB and LDH levels were analyzed. Cellular excitability and susceptibility to cardiac and cardiomyocyte arrhythmias were evaluated using in vivo (pharmacological cardiac stress) and in vitro (burst pacing) ECG protocols. Ca2+ dynamics were also investigated. The expression of CaMKII and its activation by phosphorylation and oxidation were evaluated by western blot, and molecular docking was used to analyze the possible interaction between DL and CaMKII. RESULTS: Electrocardiograms showed that administration of 10 mg/kg of HßDL prevented Doxo-induced widening of the QRS complex and QT interval. HßDL also prevented cardiomyocyte electrophysiological changes that trigger cellular arrhythmias, such as increases in action potential duration and variability; decreased the occurrence of delayed afterdepolarizations (DADs) and triggered activities (TAs), and reduced the incidence of arrhythmia in vivo. Ca2+ waves and CaMKII overactivation caused by phosphorylation and oxidation were also decreased. In the in silico study, DL showed potential inhibitory interaction with CaMKII. CONCLUSION: Our results show that 10 mg/kg of ßDL protects the heart against Doxo-induced cardiotoxicity arrhythmias, and that this is probably due to its inhibitory effect on CaMKII hyperactivation.

BACKGROUND: Implant design and apical stability are principal parameters involved in achieving successful primary stability. Using polyurethane models to simulate post-extraction sockets, we investigated the effects of using differing blade designs on the primary stability of tapered implants and the impact of apical depth. METHOD: Six polyurethane blocks were used to simulate post-extraction pockets. One of the implants presented self-tapping blades (Group A), while the other (Group B) did not. Seventy-two implants were placed at 3 different depths (5 mm, 7 mm, and 9 mm), and a torque wrench was used to measure the stability of the implants. RESULTS: When evaluating the implants (placed at 5 mm, 7 mm, and 9 mm apical to the socket), we observed that the torque of the Group B implants was higher than that of Group A implants (P < 0.01). At the 9-mm depth, there was no difference between the groups (Drive GM 34.92 Ncm and Helix GM 32.33 Ncm) (P > 0.001), and considering the same implant groups, those placed at 7-mm and 9-mm depths presented higher torques (p < 0.01) than those placed at 5-mm (p > 0.01). CONCLUSION: Considering both groups, we concluded that an insertion depth of greater than 7 mm is needed for initial stability, and in situations involving reduced supportive bone tissue or low bone density, a non-self-tapping thread design improves implant stability.

BACKGROUND: Obese individuals have higher rates of cancer incidence and cancer- related mortality. The worse chemotherapy outcomes observed in this subset of patients are multifactorial, including the altered physiology in obesity and its impact on pharmacokinetics, the possible increased risk of underdosing, and treatment-related toxicity. AIMS: The present review aimed to discuss recent data on physiology, providing just an overall perspective and pharmacokinetic alterations in obesity concerning chemotherapy. We also reviewed the controversies of dosing adjustment strategies in adult and pediatric patients, mainly addressing the use of actual total body weight and ideal body weight. METHODS: This narrative review tried to provide the best evidence to support antineoplastic drug dosing strategies in children, adolescents, and adults. RESULTS: Cardiovascular, hepatic, and renal alterations of obesity can affect the distribution, metabolism, and clearance of drugs. Anticancer drugs have a narrow therapeutic range, and variations in dosing may result in either toxicity or underdosing. Obese patients are underrepresented in clinical trials that focus on determining recommendations for chemotherapy dosing and administration in clinical practice. After considering associated comorbidities, the guidelines recommend that chemotherapy should be dosed according to body surface area (BSA) calculated with actual total body weight, not an estimate or ideal weight, especially when the intention of therapy is the cure. CONCLUSION: The actual total body weight dosing appears to be a better approach to dosing anticancer drugs in both adults and children when aiming for curative results, showing no difference in toxicity and no limitation in treatment outcomes compared to adjusted doses.

Objetivo: Evidências científicas sugerem que a deficiência de estrógeno e fatores genéticos influenciam o desenvolvimento do sistema estomatognático. Este estudo teve como objetivo avaliar a influência da deficiência de estrógeno na expressão gênica de TNF-α, IL-1ß, IL-6 e IL-10 durante o desenvolvimento dentário em modelo murino. Material e Métodos: Ratas Wistar Hannover foram divididas em dois grupos de acordo com a intervenção recebida: Grupo Hipoestrogenismo - cirurgia de ovariectomia e Grupo Controle - cirurgia fictícia. Para avaliar o desenvolvimento dentário, o incisivo inferior foi escolhido. O modelo de hipofunção dos incisivos inferiores foi realizado por ajuste incisal. O incisivo homólogo exercia hiperfunção dentária. Os animais foram avaliados durante todo o período puberal. Após a eutanásia, as hemimandíbulas foram removidas para avaliar a expressão gênica do TNF-α, IL-1ß, IL-6 e IL-10 na região odontogênica dos incisivos por meio de PCR em tempo real. Foi realizado o teste de Kruskal-Wallis e o pós-teste de Dunn. O nível de significância foi de 5%. Resultados: Houve diferenças estatisticamente significativas na expressão gênica de TNF-α e IL-1ß entre os grupos hipoestrogenismo e controle sob condição de hipofunção dentária (p=0,0084, p=0,0072, respectivamente). Conclusão: A deficiência de estrógeno influencia a expressão gênica de TNF-α e IL-1ß na região odontogênica de dentes hipofuncionais (AU)


Objective: Scientific evidence suggests that estrogen deficiency and genetic factors have an influence on the development of the stomatognathic system. This study aimed to evaluate the influence of estrogen deficiency on the gene expression of TNF-α, IL-1ß, IL-6 and IL-10 during dental development in a murine model. Material and Methods: Wistar Hannover rats were divided into two groups according to the intervention received: Hypoestrogenism Group - ovariectomy surgery and Control Group - fictitious surgery. To evaluate the dental development, the lower incisor was chosen. The mandibular incisor hypofunction model was performed by incisal adjustment. The homologous incisor exerted a hyperfunction. The animals were evaluated throughout the pubertal period. After euthanasia, the hemimandibles were removed to evaluate the gene expression of the TNF-α, IL-1ß, IL-6 and IL-10 in the odontogenic region of the incisors through real time PCR. Kruskal-Wallis test and Dunn's posttest were performed. The level of significance was 5%. Results: There were statistically significant differences of TNF-α and IL-1ß gene expression between the hypoestrogenism and control groups under hypofunction condition (p=0.0084, p=0.0072, respectively). Conclusion: Estrogen deficiency influences TNF-α and IL-1ß gene expression in the odontogenic region of the hypofunctional teeth. (AU)

Introdução: O acesso ao tratamento hospitalar adequado é importante para amenizar o impacto causado pelas questões socioeconômicas, auxiliando no combate às iniquidades em saúde. Objetivo: Analisar a diferença da atenção hospitalar pública e privada na mortalidade por Covid-19 em Florianópolis/SC. Métodos: Coorte histórica com dados de pacientes confirmados para Covid-19 entre 22 de fevereiro de 2020 e 09 de novembro de 2020. Utilizou-se abordagem de dupla-robustez. Na primeira etapa, parearam-se indivíduos notificados em hospitais públicos e privados por algoritmo genético. A seguir, estimou-se a probabilidade de óbito em hospitais públicos e privados, por meio de regressão logística. Analisou-se, então, a diferença entre as densidades de probabilidade de óbito dos dois tipos hospitalares. Resultados: Analisaram-se 2.497 pessoas, 1.244 de hospitais públicos e 1.253 de privados. A diferença entre a probabilidade condicional de óbito assumindo que todos os pacientes fossem notificados em hospitais públicos ou que todos fossem notificados em hospitais privados foi de -0,0002 (IC 95% -0,0013; 0,0005). Conclusão: A probabilidade de óbito por Covid-19 mostrou-se semelhante entre pacientes de hospitais públicos e privados.


Introduction: Access to adequate hospital treatment is important to alleviate the impact of socioeconomic issues, helping in the fight against health inequities. Objective: To analyze differences between public and private hospital care regarding COVID-19 mortality in Florianópolis/SC. Methods: This is a historical cohort study with data from patients who had a COVID-19 diagnosis confirmed between February 22, 2020 and November 9, 2020. We used a doubly robust approach. In the first stage, we paired individuals reported by public and private hospitals through a genetics algorithm. Subsequently, we estimated the probability of death in public and private hospitals using a logistic regression. We then analyzed the difference between probability densities of death in both hospital types. Results: This study analyzed 2,497 people, 1,244 public hospitals, and 1,253 private institutions. The difference between conditional probabilities of death assuming that all patients were reported by public hospitals or that all of them were reported by private hospitals was -0.0002 (95%CI -0.0013; 0.0005). Conclusion: The probability of death due to COVID-19 was shown to be similar between patients of public or private hospitals.

Abstract Polymyxins are antibiotics developed in the 1950s. Polymyxin-induced neurotoxicity has been often described in medical literature. The same cannot be said of nephrotoxicity or tubulopathy in particular. This report describes the case of a patient prescribed polymyxin B to treat a surgical wound infection, which led to significant increases in fractional excretion of calcium, magnesium, and potassium and subsequent persistent decreases in the levels of these ions, with serious consequences for the patient. Severe hypocalcemia, hypomagnesemia, and hypokalemia may occur during treatment with polymyxin. Calcium, magnesium and potassium serum levels must be monitored during treatment to prevent life-threatening conditions.


Resumo Polimixinas são um grupo de antibióticos desenvolvidos na década de 1950. Seus efeitos neurotóxicos são comumente descritos na literatura, porém há menos relatos sobre seus efeitos nefrotóxicos, especialmente tubulopatias. O objetivo deste relato é descrever o uso de polimixina B em uma paciente para tratamento de infecção de ferida operatória, promovendo grande aumento das frações de excreção de cálcio, magnésio e potássio e acarretando reduções graves e persistentes desses íons, com sérias consequências para a paciente. Hipocalcemia, hipomagnesemia e hipocalemia severas podem ocorrer durante terapia com polimixina e é sugerido que sejam monitorizadas as concentrações séricas desses eletrólitos durante o tratamento como forma de evitar condições de risco à vida.

Since 1994, Brazil has been offering Primary Health Care (PHC) services based on the Family Health Strategy. The ESF has achieved important results. During this period, the PHC financing model did not undergo major methodological changes. In this article, the results of the new financing model for PHC approved in a tripartite manner in 2019, Previne Brasil, are laid out, which is composed of (i) weighted capitation, (ii) incentives for specific and strategic actions and, (iii) payment for performance. These first results reveal the increase of more than 50 million people with qualified and unique registration, associated with the record number of more than 52 thousand FH / eAP teams financed by the Ministry of Health, with more than 35 thousand of which (67%) using electronic medical records . In addition, the registration of people and professionals of the Brazilian PHC, together with SISAB, is the largest set of demographic and clinical health data in the world. These advances favor the overcoming of difficulties to achieve greater access, longitudinality and coordination of care, qualifying the Brazilian PHC in search of better health outcomes.


O Brasil, desde 1994, oferta serviços de atenção primária à saúde (APS) a partir da Estratégia Saúde da Família (ESF). A ESF alcançou resultados importantes. Nesse período, o modelo de financiamento da APS não sofreu alterações metodológicas importantes. Neste artigo, descreve-se os resultados do novo modelo de financiamento para APS aprovado de forma tripartite em 2019, o "Previne Brasil", que é composto por (i) capitação ponderada, (ii) incentivos a ações específicas e estratégicas e (iii) pagamento por desempenho. Esses primeiros resultados revelam o incremento de mais de 50 milhões de pessoas com cadastro qualificado e único, associado ao número recorde de mais de 52 mil equipes de Saúde da Família/Equipe de Atenção Primária (EAP) financiadas pelo Ministério da Saúde, sendo mais de 35 mil (67%) com uso de prontuário eletrônico. Além disso, o cadastro das pessoas e dos profissionais da APS brasileira junto ao Sistema de Informação em Atenção Básica (SISAB) se configura como o maior conjunto de dados demográficos e clínicos de saúde do mundo. Esses avanços favorecem a superação das dificuldades para o alcance de maior acesso, longitudinalidade e coordenação do cuidado, qualificando a APS brasileira em busca de melhores resultados em saúde.

BACKGROUND: The aim of this study was investigated if estrogen deficiency during puberty affects the expression of miRNA30a and miRNA503 in maxillary and mandibular growth centers, and also evaluated if ERα and ERß are correlated with miRNA30a and miRNA503 expressions. METHODS: Samples from 12 female Wistar rats randomized into experimental group (OVX) and control group (SHAM). At an age of 45 days animals were euthanized for miRNA expression analyses. RT-qPCR was performed to determine miRNA30a and miRNA503 expression in growth sites: midpalatal suture, condyle, mandibular angle, symphysis/parasymphysis and coronoid process. The data was carried out using the parametric tests at 5% of significance level. RESULTS: miRNA 30a and miRNA503 presented higher levels in the condylar site in SHAM group when compared with OVX (p = 0.002 and p = 0.020, respectively). In the growth centers, a statistical significant difference was observed only for miRNA30a (p = 0.004), when compared mandibular angle with condyle the in OVX group (p = 0.001). A strong positive correlation between miRNA503 and ERα in the condyle of OVX group was observed (r = 0.90; p = 0.039 and it also between miRNA503 and ERß in the coronoid process of the OVX group (r = 0.88; p = 0.05). CONCLUSION: The results suggested that estrogen regulates specific miRNAs in maxillary and mandibular growth centers, which may participate in posttranscriptional regulation of estrogen-regulated genes.

RESUMEN

Traumatic brain injury (TBI) is a serious cause of disability and death among young and adult individuals, displaying complex pathophysiology including cellular and molecular mechanisms that are not fully elucidated. Many experimental and clinical studies investigated the potential relationship between TBI and the process by which neurons are formed in the brain, known as neurogenesis. Currently, there are no available treatments for TBI's long-term consequences being the search for novel therapeutic targets, a goal of highest scientific and clinical priority. Some studies evaluated the benefits of treatments aimed at improving neurogenesis in TBI. In this scenario, herein, we reviewed current pre-clinical studies that evaluated different approaches to improving neurogenesis after TBI while achieving better cognitive outcomes, which may consist in interesting approaches for future treatments.

Polymyxins are antibiotics developed in the 1950s. Polymyxin-induced neurotoxicity has been often described in medical literature. The same cannot be said of nephrotoxicity or tubulopathy in particular. This report describes the case of a patient prescribed polymyxin B to treat a surgical wound infection, which led to significant increases in fractional excretion of calcium, magnesium, and potassium and subsequent persistent decreases in the levels of these ions, with serious consequences for the patient. Severe hypocalcemia, hypomagnesemia, and hypokalemia may occur during treatment with polymyxin. Calcium, magnesium and potassium serum levels must be monitored during treatment to prevent life-threatening conditions.

Traumatic brain injury (TBI) is the main cause of pediatric trauma death and disability worldwide. Recent studies have sought to identify biomarkers of TBI for the purpose of assessing functional outcomes. The aim of this systematic review was to evaluate the utility of TBI biomarkers in the pediatric population by summarizing recent findings in the medical literature. A total of 303 articles were retrieved from our search. An initial screening to remove duplicate studies yielded 162 articles. After excluding all articles that did not meet the inclusion criteria, 56 studies were gathered. Among the 56 studies, 36 analyzed serum biomarkers; 11, neuroimaging biomarkers; and 9, cerebrospinal fluid (CSF) biomarkers. Most studies assessed biomarkers in the serum, reflecting the feasibility of obtaining blood samples compared to obtaining CSF or performing neuroimaging. S100B was the most studied serum biomarker in TBI, followed by SNE and UCH-L1, whereas in CSF analysis, there was no unanimity. Among the different neuroimaging techniques employed, diffusion tensor imaging (DTI) was the most common, seemingly holding diagnostic power in the pediatric TBI clinical setting. The number of cross-sectional studies was similar to the number of longitudinal studies. Our data suggest that S100B measurement has high sensitivity and great promise in diagnosing pediatric TBI, ideally when associated with head CT examination and clinical decision protocols. Further large-scale longitudinal studies addressing TBI biomarkers in children are required to establish more accurate diagnostic protocols and prognostic tools.

Resumo O Brasil, desde 1994, oferta serviços de atenção primária à saúde (APS) a partir da Estratégia Saúde da Família (ESF). A ESF alcançou resultados importantes. Nesse período, o modelo de financiamento da APS não sofreu alterações metodológicas importantes. Neste artigo, descreve-se os resultados do novo modelo de financiamento para APS aprovado de forma tripartite em 2019, o "Previne Brasil", que é composto por (i) capitação ponderada, (ii) incentivos a ações específicas e estratégicas e (iii) pagamento por desempenho. Esses primeiros resultados revelam o incremento de mais de 50 milhões de pessoas com cadastro qualificado e único, associado ao número recorde de mais de 52 mil equipes de Saúde da Família/Equipe de Atenção Primária (EAP) financiadas pelo Ministério da Saúde, sendo mais de 35 mil (67%) com uso de prontuário eletrônico. Além disso, o cadastro das pessoas e dos profissionais da APS brasileira junto ao Sistema de Informação em Atenção Básica (SISAB) se configura como o maior conjunto de dados demográficos e clínicos de saúde do mundo. Esses avanços favorecem a superação das dificuldades para o alcance de maior acesso, longitudinalidade e coordenação do cuidado, qualificando a APS brasileira em busca de melhores resultados em saúde.


Abstract Since 1994, Brazil has been offering Primary Health Care (PHC) services based on the Family Health Strategy. The ESF has achieved important results. During this period, the PHC financing model did not undergo major methodological changes. In this article, the results of the new financing model for PHC approved in a tripartite manner in 2019, Previne Brasil, are laid out, which is composed of (i) weighted capitation, (ii) incentives for specific and strategic actions and, (iii) payment for performance. These first results reveal the increase of more than 50 million people with qualified and unique registration, associated with the record number of more than 52 thousand FH / eAP teams financed by the Ministry of Health, with more than 35 thousand of which (67%) using electronic medical records . In addition, the registration of people and professionals of the Brazilian PHC, together with SISAB, is the largest set of demographic and clinical health data in the world. These advances favor the overcoming of difficulties to achieve greater access, longitudinality and coordination of care, qualifying the Brazilian PHC in search of better health outcomes.
