RESUMEN

Although relationship factors are an important component in the evaluation and treatment of hypoactive sexual desire (HSD) disorders, a sexual intimacy paradox often occurs wherein many couples increase the intimacy in their relationship while their sexual desire continues to decline. To resolve the apparent paradox and to further our understanding of the relationship between intimacy and eroticism, we offer a developmental model of sexual intimacy as a guide to treating the interpersonal aspects of HSD and other sexual dysfunctions. This model expands Erikson's developmental phase of intimacy into five elements: conflagration, merger, fusion, differentiation, and integration. The model is described and case examples are presented as illustrations of the therapeutic issues and resolutions within each element.

RESUMEN

Bullae arose in two patients with scleroderma and in one patient with a similar condition, eosinophilic fasciitis. Biopsy specimens from two of the patients demonstrated prominent dermal lymphatic dilation. We hypothesize that these lesions occurred after the development of lymphatic obstruction secondary to dermal sclerosis.

Utilization of positive and negative feedback was investigated in a sample of 20 depressed and 20 nondepressed psychiatric inpatients. Patients, selected on the basis of Minnesota Multiphasic Personality Inventory scores and clinical diagnoses, received primarily positive or negative feedback after completing a moral judgment task. Results suggest that depressed patients are likely to utilize positive, personally relevant information to enhance their low self-esteem, whereas nondepressed patients are more likely to counteract negative feedback that is inconsistent with their self-evaluations. The results fail to confirm the presence of memory deficits for positive feedback in depression, proposed by Kovacs and Beck (Am. J. Psychiatry, 135: 525-532, 1978), and are more consistent with the prediction of Mischel et al. (J. Consult. Clin. Psychiatry, 44: 92-103, 1976) that people tend to be cognitively biased toward success and toward perpetuating positive emotional states.

Nine men with chronic erectile dysfunction (three primary, six secondary) who had no regular sexual partner were treated in two 12-session all-male psychoeducational therapy groups. Treatment intervention addressed specific factors which inhibited adequate sexual function with a focus on coping skills to overcome those factors. Pre, post, and follow-up behavioral self-report data and responses on a goal attainment scale questionnaire indicated that the treatment groups were successful for five men with secondary and one man with primary erectile dysfunction. Subjective report and pre- and posttreatment fantasy productions to TAT cards for the first group indicated that all men significantly improved their attitudes about sexuality and their sexual self-concept. The results suggest that this is a viable, cost-effective treatment for secondary erectile dysfunction, but not for primary erectile dysfunction unless supplementary individual therapy is provided.

Results of a study comparing the effectiveness of two formats for treating men with premature ejaculation and their female partners with orgasmic dysfunctions are described. In one treatment format, a couple was treated by a male and female cotherapy team once a week for 12 sessions. In the second treatment format, which also consisted of 12 weekly sessions, three or four couples were treated simultaneously in a group led by a cotherapy team. The five couples treated alone and the ten couples treated in the group format all received a standardized therapy program consisting of sex education, attitude restructuring, and specific suggestions for acquiring ejaculatory control for the men and an increased range of orgasmic response for the females. The results showed significant improvement for couples in both treatment formats. The group format initially showed a slight tendency toward more rapid progress than did the couple format, but by the 2-month follow-up there were no significant differences. The study demonstrates that couple group treatment is a cost-effective means for treating common male and female sexual problems.

The present study examined the ability of parents to bias home-observation data by influencing their child's behavior in socially desirable and undesirable directions. Parents of 12 deviant and 12 non-deviant boys, aged 4-8, were given the response set to present their child as "good", "bad", and usual (normal) during 6 45-minute observations of family interaction. Deviant and non-deviant children showed significantly more deviant behavior under the bad condition as compared with normal, whereas their good and normal conditions did not differ. Parent positive and negative behaviors were related to changes in child deviant behavior for the deviant sample; parent positive and commanding behaviors were related to changes in child deviant behavior for the nondeviant sample. Parent self-report of behavior change was generally consistent with the observation data. The results suggested implications for naturalistic observations of families and for child behavior change interventions.
