RESUMEN

RATIONALE AND OBJECTIVES: This study aims to explore the feasibility of the deep learning radiomics nomogram (DLRN) for predicting tumor status and axillary lymph node metastasis (ALNM) after neoadjuvant chemotherapy (NAC) in patients with breast cancer. Additionally, we employ a Cox regression model for survival analysis to validate the effectiveness of the fusion algorithm. MATERIALS AND METHODS: A total of 243 patients who underwent NAC were retrospectively included between October 2014 and July 2022. The DLRN integrated clinical characteristics as well as radiomics and deep transfer learning features extracted from ultrasound (US) images. The diagnostic performance of DLRN was evaluated by constructing ROC curves, and the clinical usefulness of models was assessed using decision curve analysis (DCA). A survival model was developed to validate the effectiveness of the fusion algorithm. RESULTS: In the training cohort, the DLRN yielded area under the receiver operating characteristic curve values of 0.984 and 0.985 for the tumor and LNM, while 0.892 and 0.870, respectively, in the test cohort. The consistency indices (C-index) of the nomogram were 0.761 and 0.731, respectively, in the training and test cohorts. The Kaplan-Meier survival curves showed that patients in the high-risk group had significantly poorer overall survival than patients in the low-risk group (P < 0.05). CONCLUSION: The US-based DLRN model could hold promise as clinical guidance for predicting the status of tumors and LNM after NAC in patients with breast cancer. This fusion model can also predict the prognosis of patients, which could help clinicians make better clinical decisions.

Aramid nanofibers (ANFs) as an innovative nanoscale building block exhibit great potential for novel high-performance multifunctional membranes attributed to their extraordinary performance. However, the application of aramid nanofibers in the field of surface enhanced Raman scattering (SERS) sensing has been rarely reported. In this work, aramid nanofibers derived from commercial Kevlar fibers were synthesized by a facile dimethyl sulfoxide/potassium hydroxide (DMSO/KOH) solution treatment. The monodispersed silver nanoparticle-decorated aramid nanofiber (m-Ag@ANF) membranes were constructed by an efficient vacuum filtration technique. Taking advantages of unique intrinsic properties of ANF, the m-Ag@ANF substrates exhibit good flexibility, excellent mechanical properties and prominent thermal stability. Besides, due to the abundance of positively charged amino-group on the ANF substrates, the negatively charged m-AgNPs were uniformly and firmly deposited on the surface of ANF substrate through electrostatic interactions. As a result, the optimal flexible m-Ag-9@ANF SERS substrate exhibits high sensitivity of 10-9 M for methylene blue (MB) and excellent signal reproducibility (RSD = 6.37 %), as well as outstanding signal stability (up to 15 days). Besides, the 2D Raman mapping and FDTD simulations further reveal prominent signal homogeneity and strong electric field distribution for flexible m-Ag-9@ANF SERS substrate. Finally, it is demonstrated that the flexible m-Ag-9@ANF SERS substrate can also be used for detection of toxic molecules on irregular surfaces by a feasible paste-and-read process. The m-Ag@ANF paper exhibits potential applications as a flexible, low-cost, robust and stable SERS sensing platform for trace detection of toxic materials.

RESUMEN

Phytochemical investigation on the leaves of Rabdosia serra led to the isolation of two new abietane diterpenoids, raserranes A (1) and B (2), and four known phenylpropanoids (3-6). Their structures were determined by spectroscopic data. Compound 1 represented the rare examples of abietane diterpenoids featuring a 16-methoxycarbonyl group.

OBJECTIVE: To observe the effects of puerarin on ADRP gene mRNA expression in fatty tissue of type 2 diabetes mellitus rats (T2DM). METHOD: Wiastar rats of T2DM model were made by feeding with high glucose and fat diet and injecting with small dose of streptozocin (25 mg x kg(-1)). 40 model rats were randomly divided into model control group and three puerarin groups (40, 80, 160 mg x kg(-1)), another 10 rats were selected as normal control group. FBG and FINS were measured to calculate IR after rats were injected consecutively for 6 weeks. The level of ADRP gene mRNA in fatty tissue was determined by RT-PCR after rats were injected eight weeks. RESULT: Compared with model control group, high and middle dosage of puerarin can decreased ADRP gene mRNA expression in fatty tissue obviously, FBG, IR level in each puerarin group and FINS in high and middle dosage puerarin groups decreased obviously. CONCLUSION: Puerarin can decrease the blood glucose level of T2DM by downregulating ADRP mRNA expression and depressing the insulin resistance.

OBJECTIVE: To evaluate the clinical therapeutic effect of electric acupuncture (EA) at Sishencong (EX-HN 1) on insomnia. METHODS: Two hundred and seventy-six patients were randomly assigned to 2 groups, 138 in each group, the EA group treated with EA at Sishencong, and the control group with oral administration of Tianmeng Capsule. The treatment course for both groups was 3 weeks. The quality and related parameters of sleep before and after treatment were evaluated with a multi-channel sleep detector. RESULTS: After treatment, the quality of sleep was improved in both groups (P < 0.05), as compared with before treatment, the difference in related parameters was significant respectively (P<0.05 or P <0.01), however, the improvement in the EA group was superior to that in the control group (P < 0.01). CONCLUSION: EA at Sishencong has obvious effect on insomnia.
