RESUMEN

This study aimed to systematically evaluate the efficacy and safety of different Chinese patent medicines in the treatment of idiopathic membranous nephropathy. The relevant randomized controlled trial(RCT) was retrieved from PubMed, EMbase, Cochrane Library, CNKI, SinoMed, Wanfang, and VIP with the time interval from database inception to December 2022. The Cochrane risk of bias assessment tool was employed to evaluate the quality of the included RCT, and Stata 15.0 and GEMTC to perform the Bayesian network Meta-analysis. Finally, 51 RCTs were included, involving 9 Chinese patent medicines and 3 591 patients. The results of network Meta-analysis showed that in terms of the total effective rate and the increase in plasma albumin, the top three interventions were Zhengqing Fengtongning Sustained Release Tablets + conventional western medicine, Bailing Capsules + conventional western medicine, and Tripterygium Glycosides Tablets + conventional western medicine. In terms of reducing 24-hour urine total protein, the top three interventions were Zhengqing Fengtongning Sustained Release Tablets + conventional western medicine, Shenfukang Capsules +conventional western medicine, and Huangkui Capsules + conventional western medicine. In terms of reducing serum creatinine, the top three interventions were Shenfukang Capsules + conventional western medicine, Bailing Capsules + conventional western medicine, and Zhengqing Fengtongning Sustained Release Tablets + conventional western medicine. In terms of safety, Chinese patent medicines combined with conventional western medicine had fewer adverse reactions than the control group. The results suggest that Chinese patent medicines combined with conventional western medicine can improve the therapeutic effect on idiopathic membranous nephropathy, and differentiated medications can be adopted according to the specific symptoms of patients in clinical treatment. Further validation needs to be carried out in the future with multi-center, large-sample, and high-quality RCT.

OBJECTIVE: To access the therapeutic effect of Tuina for treatment of infants eczema. METHODS: Two hundred and forty children with eczema were randomly divided into a Tuina group and a medication group, 120 cases in each group. The Tuina group was treated with Tuina on ten points using the thumb and middle finger, and the medication group was treated with oral administration of Chlorpheniramine and topical application of zinc oxide ointment or Youzhuoer ointment, etc. The therapeutic effects were evaluated after 3 weeks. RESULTS: The cured-markedly effective rate and total effective rate were 94.2% and 99.2% in the Tuina group and 98.0% and 100.0% in the medication group, respectively, the therapeutic effects were similar in the two groups (both P>0.05); 6 months after treatment, the recurrence rate of 3.8% in the Tuina group was significantly lower than 42.9% in the medication group (P<0.01), and there were no adverse reactions in the whole research process. CONCLUSION: Tuina on ten points for treatment of infants eczema has unequivocal short-term effect, a stable long-term effect, and low recurrence rate.

OBJECTIVE: To analyzed a large group of invasive breast cancers with long-term follow-up information to evaluate the clinicopathologic, morphological and prognostic features of basal-like breast cancers in Chinese population. METHODS: Immunohistochemistry was used to detect the expression of ER, HER2, CK5/6, EGFR on tissue microarray with 1311 invasive breast cancers. Based on the results, these cases were categorized into luminal A, luminal B, basal-like, HER2-overexpressing and null subtypes. Clinicopathological features and survival rates were compared between these groups. RESULTS: Basal-like breast cancers constituted 17.0% of 1311 invasive breast cancers with a significantly larger size, higher grade and higher incidence of the medullary carcinoma, frequent recurrence and infrequent node metastasis. Morphologically, basal-like breast cancers showed a significantly more solid architecture and ribbon-like architecture associated with necrosis (more geographic necrosis) and central scar, a more pushing margin, lymphocytic infiltration and a higher mitosis score, more syncytial growth, presence of basaloid cells, spindle cells and squamous metaplasia. The disease-free survival and overall survival of basal-like breast cancers were significantly poorer than that of luminal A subtype, but similar to the other ER-negative subtypes. Basal markers were not independent prognostic factors. CONCLUSIONS: Basal-like breast cancers in Chinese population has a similar prevalence to that of the western populations. They have distinct clinicopathologic features compared to other non-basal breast cancers, but overlapping with other ER-negative breast cancers. Morphological features are strongly associated with basal-like breast cancers although they are not very specific. The survival of basal-like breast cancers is poorer than luminal A, but similar to the other ER-negative breast cancers, and basal markers are not independent prognostic factors of breast cancers.

OBJECTIVES: Basal cell-like breast cancer is one of the subtypes using molecular typing, and this subtype attracted a wide spread attention. Currently, no uniform diagnostic criteria are available. Most studies demonstrated poor outcomes, but contradictory conclusions appeared recently. The prognosis of basal cell-like breast cancer using different immunohistochemical criteria were analyzed. METHODS: Two hundred and eighty-four invasive breast cancers with a follow-up information over 5 years were evaluated for ER, PR, HER2, CK5/6, CK14, EGFR expression on tissue microarray immunohistochemically. Based on the results, these cases using four different diagnostic criteria were categorized, namely: Nielsen (ER-/HER2-, CK5/6+ and/or EGFR+), Kim (ER-/PR-/HER2-, CK5/6+ and/or CK14+ and/or EGFR+), Triple-negative (ER-/PR-/HER2-), and basal-CK (CK5/6+ and/or CK14+). 5-year survival information was compared between groups. RESULTS: The prevalence of basal cell-like breast cancer by Nielsen, Kim, Triple-negative and basal-CK were 15.5% (44/284), 14.8% (42/284), 43.3% (123/284) and 21.1% (60/284) respectively; the recurrence rates were 18.2% (8/44), 21.4% (9/42), 10.6% (13/123) and 11.7% (7/60) respectively. These were higher than recurrence rates for other subtypes, but only the differences by Nielsen's and Kim's criteria were significant. Using Nielsen's and Triple-negative's criteria, basal-like tumors showed shorter 5-year disease-free survival (both P < 0. 01) and overall survival (P < 0.05 and 0.01) than luminal A subtype, using Kim's criteria, basal-like tumors showed a lower 5-year disease-free but not overall survival than luminal A subtype (P < 0.01); no significant difference was found on 5-year survival between basal-like and non-basal-like tumors when typed by basal-CK. CONCLUSION: Basal cell-like breast cancers are more likely to show more recurrence and worse outcome, but different immunohistochemical diagnostic criteria have an influence on their prognostic analysis, so a uniform diagnostic criteria is essential for the further study of basal-like breast cancers.

RESUMEN

A new borogermanate open-framework, FJ-18, with intersecting 12-/8-, 9-, and 9-ring channels in multidimensions was synthesized by using an organic diamine as structure-directing agent (SDA). X-ray diffraction showed that its structure is constructed by flexible connection of strictly alternate B(4)O(9) clusters (with fictitious tetrahedral geometry) and GeO(4) tetrahedra to form a zeotype framework with the CrB(4) topology. The topology and structural relations between FJ-18 and FJ-16, a known borogermanate based on the linkage of B(4)O(9) clusters and GeO(4) units, are described in detail. Different guest SDAs can, by means of their shape, size, and charge, directly influence the structure of the host framework, and the origin of these phenomena can be attributed to fundamental parameters such as host-guest symmetry and charge matching through H-bonding interactions, the concept of which has for the first time been extended to tetrahedral-triangular frameworks. This result is a part of our ongoing work aimed at making new large-pore materials constructed from B-O polyanions and Ge-O cluster units.

The self-assembly of Ge-O polyhedra by metal-complex templates leads to initial examples of open germanate structures under mild solvothermal conditions. These materials are constructed from Ge-O cluster building blocks (Ge7X19 (X = O, OH, or F) or Ni@Ge14O24(OH)3) and span the full range of dimensionalities from 1D chains of Ge7O13(OH)2F3 x Cl x 2 [Ni(dien)2] (FJ-6) to 2D layers of Ge7O14F3 x 0.5 [In(dien)2] x 0.5 H3dien x 2 H2O (1) and 3D frameworks of Ni@ Ge14O24(OH)3 x 2 [Ni(L)3] (FJ-1 a/FJ-1 b) (dien = diethylenetriamine, L = ethylenediamine (en) or 1,2-diaminopropane (enMe)). The Ge7X19 cluster in FJ-6 and 1 is formed by condensation of four GeX4 tetrahedra, two GeX5 trigonal bipyramids, and one GeX6 octahedron with a mu3-O atom at the center of the cluster, whereas the Ni@ Ge14O24(OH)3 cluster in FJ-1 a/FJ-1 b is formed by condensation of nine peripheral GeO4 tetrahedra and five interior GeO3Ni units with one mu5-Ni atom at the center of the cluster. FJ-6 is characterized by a pair of racemic Ge7O14(OH)2F3 cluster chains and represents only one example of 1D germanates; 1 exhibits unique germanate layers with uniform 10-membered-ring apertures encapsulating an unknown indium complex, and the framework structure of FJ-1 a/FJ-1 b with large 24-membered-ring channels is the first example of porous materials that contain metal-metal bonds (Ge2+-Ni+). These initial examples of germanates from metal-complex templates provide a useful model system for understanding the mechanisms of host-guest interactions, which may further facilitate the design and development of new porous materials "on demand". It is shown that the symmetry and configuration of the guest metal complex can be imprinted onto the host inorganic framework through hydrogen bonding between host and guest.

Two new niobium and zinconiobium fluorophosphates, NbOF(PO4)2(C2H10N2)2 (1) and Zn3(NbOF)(PO4)4-(C2H10N2)2 (2), have been prepared under hydrothermal conditions using ethylenediamine as a template. The structures were determined by single crystal diffraction to be triclinic, space group P1 (No. 2), a = 8.1075 (6) A, b = 9.8961 (7) A, c = 10.1420(8) A, alpha = 111.655(1) degrees, beta = 111.51(1) degrees, gamma = 93.206(1) degrees, V = 686.19(9) A3, and Z = 2 for 1 and orthorhombic, space group Fddd (No. 70), a = 9.1928(2) A, b = 14.2090(10) A, c = 32.2971 (6) A, V = 4218.66(12) A3, and Z = 8 for 2, respectively. Compound 1 is an infinite linear chain consisting of corner-sharing [Nb2P2] 4-MRs bridged at the Nb centers with organic amines situated between chains, and compound 2, containing the chains similar to that in 1, forms a zeotype framework with organic amines situated in the gismondine-type [4684] cavities. The topology of 2 was previously unknown with vertex symbol 4 x 4 x 4 x 4 x 8 x 8 (vertex 1), 4 x 4 x 4 x 82 x 8 x 8 (vertex 2), 4 x 4 x 8 x 8 x 82 x 82 (vertex 3), and 4 x 4 x 4 x 82 x 8 x 8 (vertex 4). The topological relationships between the 4-connected network of 2 and several reported (3,4)-connected networks were discussed.

Dysregulation of a genomic imprinting gene can contribute to carcinogenesis. Here, delta-like 1 homolog (Drosophila) (DLK1), a paternally expressed gene, was found to be significantly up-regulated in 60 (73.2%) of a total of 82 hepatocellular carcinoma (HCC) specimens using reverse transcription-polymerase chain reaction. In addition, immunohistochemistry staining was performed in another 88 HCC specimens, of which 50 (56.8%) cancerous tissues were considered as positive. The expression of DLK1 was obviously induced in HCC cells, Bel-7402 and MHCC-H, by a demethylation agent, 5-aza-2'-deoxycytidine. Furthermore, both demethylation of the DLK1 promoter (-565 to -362) and hypermethylation of the imprinting control domain in the region upstream of maternally expressed gene 3 were identified in a few HCC specimens. This implies that deregulation of genomic DNA methylation of the imprinted domain could be attributed to the up-regulation of DLK1 in HCC, although the undoubtedly complex mechanisms involved in the epigenetic event should be further investigated in HCC. Surprisingly, the expression of DLK1 in HCC was confirmed to be monoallelic specific, not biallelic, in three HCC specimens with a single nucleotide polymorphism as at T852C (rs2295660). Importantly, the exogenous DLK1 can significantly promote the cell proliferation of SMMC-7721 cells, a HCC cell line, whereas the suppression of endogenetic DLK1 through RNA interference can markedly inhibit cell growth, colony formation and tumorigenicity of HepG2, Hep3B and HuH-7 cells. These data suggest that DLK1 as an imprinted gene could be significantly up-regulated in HCC due to certain epigenetic events and contribute to the oncogenesis of this tumor.
