Thirty-eight patients with 40 varicoceles underwent spermatic venography and 27 varicoceles were treated by transvenous embolization under local anaesthesia as an outpatient procedure. The indications for the procedure were infertility (21), swelling (3), pain and discomfort (9) and asymptomatic varicoceles (5).

Seventy-four patients with impaired renal function were investigated by ultrasound. By assessing renal length, renal echogenicity and collecting system dilatation, one can establish whether renal function is impaired by chronic or acute renal changes or by obstructive changes.

A patient with retrograde ejaculation following a transurethral resection of the prostate was able to father a child using spermatozoa recovered from the bladder. The method of recovery of these spermatozoa is described.

Vaying degrees of testicular dysfunction are found in men with traumatic spinal cord damage. Eighteen paraplegic men have been studied and the gonadotropin response to luteinizing hormone-releasing hormone (LRH) measured. Basal serum testosterone estimations were made and in eight of the patients testicular testosterone reserve was assessed by the testosterone response to human chorionic gonadotropin (HCG). Testicular biopsies were performed in seven cases. In three of these patients, the testicular biopsies were abnormal. Five of the patients had elevated Follicle stimulating hormone levels and abnormalities of Luteinizing hormone kinetics were found in the same five patients. There was no significant difference between the plasma testosterone levels of the paraplegic patients when compared to the control group. In all the patients tested, there was an adequate testosterone reserve, and this included the three patients with the abnormal testicular biopsies. No relationship was found between the level of cord lesion and any of the hormonal parameters measured. This study confirms the primary nature of the seminiferous tubular damage which occurs in some patients with paraplegia.

Two patients presented with macroscopic haematuria and dysuria. Cytoscopic examination showed haemorrhagic cystitis. The findings of casts in the urine and of impaired renal function led to renal biopsies. These showed glomerulonephritis with deposits of IgA in the mesangium. No evidence of viral infection was found. A third patient diagnosed on biopsy 9 years previously as having focal glomerulonephritis developed gross haematuria and dysuria. Cystoscopy revealed haemorrhagic cystitis, and deposits of immunoglobulins were present in the bladder wall. It is suggested that haemorrhagic cystitis may occur as part of a syndrome with glomerulonephritis and may be due to an immunological mechanism.

Computed axial tomography (CAT) allows pinpoint renal localization for percutaneous renal biopsy. A method for using CAT is described. This method may also be used for accurate localization of masses in the retroperitoneal space, lung, kidney, liver, bone or pancreas.

Four patients with aneurysms of the renal artery are presented. They underwent reconstructive vascular surgery with preservation of renal function. In 1 patient the extracorporeal or 'bench-top' surgical technique was used, with cold renal perfusion and autotransplantation of the kidney to the pelvis.
