Melanoma is a malignant neoplasm that arises in melanocytes, pigment-producing cells present primarily in the skin. However, not all malignant melanomas originate from the skin, and the other sites of origin include the uveal (eye) or mucosa (rectal or oral); it can have different patterns of mutations. While targeted therapies and immunotherapies have transformed the treatment of this disease in the metastatic setting, resistance mechanisms can still pose challenges for patients and their healthcare providers. We present a case of a male patient with metastatic melanoma and discuss its clinical presentation, diagnostic workup, treatment options, and outcomes. By exploring the complexities of this multifaceted disease process, clinicians and researchers can gain valuable insights into potential areas for improved management strategies. Ultimately, we should aim to deepen our understanding of melanoma and work towards better patient outcomes.
RESUMEN
Paroxysmal nocturnal hemoglobinuria (PNH) is an uncommon genetic disorder that affects red blood cell production, causing symptoms like fatigue, abdominal pain, and shortness of breath. This condition can also result in dark urine and an increased risk of infections. Diagnosis of PNH involves genetic testing and flow cytometry, which can confirm the presence of the condition. Once a diagnosis is confirmed, personalized treatment plans should be developed to effectively manage the symptoms and improve the patient's quality of life. Treatment options for PNH may include bone marrow transplantation, blood transfusions, and the use of recombinant monoclonal antibody, eculizumab. Regular monitoring is also essential to identify and manage any complications that may arise due to this condition. With proper management and treatment, patients with PNH can lead a healthy and fulfilling life. In this case study, we present a young adult male with PNH who also suffers from renal failure, highlighting the importance of personalized care and ongoing monitoring for this complex condition.

SARS-CoV-2 can induce a hypercoagulable state, occasionally resulting in pulmonary venous thrombosis (PVT) due to inflammation and endothelial injury. Documented cases of PVT with active COVID-19 and post-COVID are rare. We report a 40-year-old postpartum female with unprovoked PVT following a recent nonhospitalized COVID-19 infection. She presented with cough, right pleuritic chest pain, and worsening dyspnea. Imaging confirmed right lower lobe PVT with ground glass infiltrates. Despite a negative hypercoagulable workup, the patient's postpartum and post-COVID status suggest an unprovoked PVT. Treated with a high-intensity heparin drip and transition to apixaban, she showed resolution of the thrombus. This case underscores the importance of considering COVID-19 as a potential risk factor for venous thromboembolism and highlights the need for vigilant monitoring in post-COVID-19 patients.
