RESUMEN

Non-contact physiological measurements based on image sensors have developed rapidly in recent years. Among them, thermal cameras have the advantage of measuring temperature in the environment without light and have potential to develop physiological measurement applications. Various studies have used thermal camera to measure the physiological signals such as respiratory rate, heart rate, and body temperature. In this paper, we provided a general overview of the existing studies by examining the physiological signals of measurement, the used platforms, the thermal camera models and specifications, the use of camera fusion, the image and signal processing step (including the algorithms and tools used), and the performance evaluation. The advantages and challenges of thermal camera-based physiological measurement were also discussed. Several suggestions and prospects such as healthcare applications, machine learning, multi-parameter, and image fusion, have been proposed to improve the physiological measurement of thermal camera in the future.

BACKGROUND: Wearable devices based on inertial measurement units through wireless sensor networks have many applications such as real-time motion monitoring and functional outcome assessment of stroke rehabilitation. However, additional investigations are warranted to validate their clinical value, particularly in detecting the synergy patterns of movements after stroke. AIM: The aim of this study was to explore the feasibility and efficacy of wearable devices for upper limb rehabilitation in patients with chronic stroke and to compare the intervention effects (e.g., neurological recovery, active range of motion, and deviation angle) with those in a control group. DESIGN: A single-blind, randomized-controlled pilot study. SETTING: Rehabilitation ward. METHODS: A total of 18 patients with chronic stroke were randomly distributed into a device group and control group. Both groups received conventional rehabilitation; nevertheless, the device group was additionally subjected to 15 daily sessions at least three times a week for 5 weeks. The outcome measures included the upper extremity subscores of the Fugl-Meyer assessment, active range of motion, and deviation angle. These measurements were performed pre- and post-treatment. RESULTS: All five Fugl-Meyer assessment subscores improved in both the device and control groups after intervention; in particular, the "shoulder/elbow/forearm" subscore (P=0.02, 0.03) and "total score" (P=0.03, 0.03) substantially improved. The active range of motion of shoulder flexion and abduction substantially improved at pre-post treatment in both the device (P=0.02, 0.03) and control (P=0.02, 0.03) groups. The deviation angle of shoulder external rotation during shoulder abduction substantially improved in the device group (P=0.02), but not in the control group. CONCLUSIONS: The designed wearable devices are practical and efficient for use in chronic patients with stroke. CLINICAL REHABILITATION IMPACT: Wearable devices are expected to be useful for future internet-of-things rehabilitation clinical trials at home and in long-term care institutions.

Heart rate is an important physiological parameter for healthcare. Among measurement methods, photoplethysmography (PPG) is an easy and convenient method for pulse rate detection. However, as the PPG signal faces the challenge of motion artifacts and is constrained by the position chosen, the purpose of this paper is to implement a comfortable and easy-to-use multi-PPG sensor module combined with a stable and accurate real-time pulse rate detection method on a computer mouse. A weighted average method for multi-PPG sensors is used to adjust the weight of each signal channel in order to raise the accuracy and stability of the detected signal, therefore reducing the disturbance of noise under the environment of moving effectively and efficiently. According to the experiment results, the proposed method can increase the usability and probability of PPG signal detection on palms.

Subject movement and a dark environment will increase the difficulty of image-based contactless pulse rate detection. In this paper, we detected the subject's motion status based on complexion tracking and proposed a motion index (MI) to filter motion artifacts in order to increase pulse rate measurement accuracy. Additionally, we integrated the near infrared (NIR) LEDs with the adopted sensor and proposed an effective method to measure the pulse rate in a dark environment. To achieve real-time data processing, the proposed framework is constructed on a Field Programmable Gate Array (FPGA) platform. Next, the instant pulse rate and motion status are transmitted to a smartphone for remote monitoring. The experiment results showed the error of the pulse rate detection to be within -3.44 to +4.53 bpm under sufficient ambient light and -2.96 to + 4.24 bpm for night mode detection, when the moving speed is higher than 14.45 cm/s. These results demonstrate that the proposed method can improve the robustness of image-based contactless pulse rate detection despite subject movement and a dark environment.

In this paper, we explore the possibility of embedding augmented reality (AR) in authentic inquiry activities to contextualize students' exploration of medical surgery, and investigate students' perceptions of the AR lessons and simulators, and their Science, Technology, Engineering, and Mathematics (STEM) interest. Thirty-two senior high school students participated in the two AR lessons related to medical surgery, "laparoscopic surgery" and "cardiac catheterization." The results showed that the students had positive perceptions of the AR lessons and simulators (overall mean = 4.1) after completing the two lessons. However, the authenticity of the simulators was perceived as the lowest ranking. In contrast, both the motivation and engagement triggered by the AR lessons were high, with most of the mean scores reaching 4.3. The AR lessons did evoke some students' STEM interest as the survey results indicated that 12 students considered an STEM major in university. This study provides a possible solution for the alignment of instructional approaches (authentic inquiry), technology design (AR), and learning experience in developing STEM lessons.

Six female patients with bilateral medial knee OA and 6 healthy controls were recruited. Patients with knee OA received a 6-week physiotherapist-supervised and home-based exercise program. Outcome measures, including the Western Ontario and McMaster Universities Arthritis Index and Short Form-36 Health Survey as well as objective biomechanical indices were obtained at baseline and follow-up. After treatment, no significant difference was observed in the knee abductor moment (KAM), lever arm, and ground reaction force. We, however, observed significantly improved pain and physical function as well as altered gait patterns, including a higher hip flexor moment and hip extension angle with a faster walking speed. Although KAM was unchanged, patients with bilateral knee OA showed an improved walking speed and altered the gait pattern after 6 weeks of supervised exercise. This finding suggests that the exercise intervention improves proximal joint mechanics during walking and can be considered for patients with bilateral knee OA. Non-weight-bearing strengthening without external resistance combined with stretching exercise may be an option to improve pain and function in individuals with OA who cannot perform high resistance exercises owing to pain or other reasons.

The anion exchanger (AE) plays critical roles in physiological processes including CO2 transport and volume regulation in erythrocytes and acid-base regulation in renal tubules. Although expression of the AE in inner-ear hair cells was reported, its specific localization and function are still unclear. Using in situ hybridization, we found that the AE1b transcript is expressed in lateral-line hair cells of zebrafish larvae. An immunohistochemical analysis with a zebrafish-specific antibody localized AE1b to stereocilia of hair cells, and the expression was eliminated by morpholino knockdown of AE1b. A non-invasive, scanning ion-selective electrode technique was applied to analyze mechanotransducer (MET) channel-mediated Ca2+ influx at stereocilia of hair cells of intact fish. Ca2+ influx was effectively suppressed by AE1b morpholino knockdown and inhibitor (DIDS) treatment. Elevating external Ca2+ (0.2 to 2 mM) neutralized the inhibition of DIDS. Taken together, this study provides solid evidence to show that AE1b in stereocilia is required for the proper functioning of MET channels.

Zebrafish lateral-line hair cells are an in vivo model for studying hair cell development, function, and ototoxicity. However, the molecular identification and properties of the mechanotransducer (MET) channel in hair cells are still controversial. In this study, a noninvasive electrophysiological method, the scanning ion-electrode technique (SIET), was applied for the first time to investigate properties of MET channels in intact zebrafish embryos. With the use of a Ca(2+)-selective microelectrode to deflect hair bundles and simultaneously record the Ca(2+) flux, the inward Ca(2+) flux was detected at stereocilia of hair cells in 2- to ~4-day postfertilization embryos. Ca(2+) influx was blocked by MET channel blockers (BAPTA, La(3+), Gd(3+), and curare). In addition, 10 µM aminoglycoside antibiotics (neomycin and gentamicin) were found to effectively block Ca(2+) influx within 10 min. Elevating the external Ca(2+) level (0.2-2 mM) neutralized the effects of neomycin and gentamicin. However, elevating the Mg(2+) level up to 5 mM neutralized blockade by gentamicin but not by neomycin. This study demonstrated MET channel-mediated Ca(2+) entry at hair cells and showed that the SIET to be a sensitive approach for functionally assaying MET channels in zebrafish.

The laser Doppler flowmeter has become an effective tool for the clinical study of microcirculation with non-invasive measurements and excellent velocity resolution. Microcirculation flow measurements cannot be used as reference criteria for circulation function. Thus, the relative change in fingertip microcirculation was measured through inflation and deflation of an automatic sphygmomanometer wrapped around the arm, and the blood pressure and functional parameters of circulation, such as biological zero (BZ), peak flow (PF) and time to peak flow (tp), were determined. For 50 healthy participants, the correlation coefficients for the laser Doppler flowmeter and oscillometric results were 0.950 and 0.838 for systolic and diastolic pressure, respectively. The mean and standard errors for both methods fell within the range specified in the Association for the Advancement of Medical Instrumentation standards. The BZ of an edema patient was about 5 times higher than normal. For patients with peripheral arterial occlusive disease (PAOD), the PF could not be determined, and the tp was about twice the normal value. The accuracy of blood pressure measurements using the laser Doppler flowmeter was comparable to that of the commonly used oscillometric sphygmomanometer, and the physiological circulation functional parameters were useful in identifying signs of edema and PAOD.

In this paper, we developed a wireless blood pressure monitoring system which provides a useful tool for users to measure and manage their daily blood pressure values. This system includes an ARM-based blood pressure monitor with a ZigBee wireless transmission module and a PC-based management unit with graphic user interface and database. The wireless blood pressure monitor can measure the blood pressure and heart rate and then store and forward the measuring information to the management unit through the ZigBee wireless transmission. On the management unit, user can easy to see their blood pressure variation in the past using a line chart. Accuracy of blood pressure measurement has been verified by a commercial blood pressure simulator and shown the bias of systolic blood pressure is ≤ 1 mmHg and the bias of diastolic blood pressure is ≤ 1.4 mmHg.

This paper proposes a mobile patient monitoring system, which integrates current personal digital assistant (PDA) technology and wireless local area network (WLAN) technology. At the patient's location, a wireless PDA-based monitor is used to acquire continuously the patient's vital signs, including heart rate, three-lead electrocardiography, and SpO2. Through the WLAN, the patient's biosignals can be transmitted in real-time to a remote central management unit, and authorized medical staffs can access the data and the case history of the patient, either by the central management unit or the wireless devices. A prototype of this system has been developed and implemented. The system has been evaluated by technical verification, clinical test, and user survey. The evaluation of performance yields a high degree of satisfaction (mean = 4.64, standard deviation--SD = 0.53 in a five-point Likert scale) of users who used the PDA-based system for intrahospital transport. The results also show that the wireless PDA model is superior to the currently used monitors both in mobility and in usability, and is, therefore, better suited to patient transport.
