The authors have studied 256 patients with double fractures of the bones of the crus, the basis of the analysis of the causes, the mechanism of the origin and the clinico-roentgenologic picture a classification of the lesions and a number of new methods of treatment have been worked out which provide reposition and fixation of the fragments, prevent the development of early and late complications and contribute to reduction of the terms of treatment and disability. The method of treatment of double fractures of the bones of the crus with permanent skeletal traction has been improved. A method of insertion of metal rods into the tibia in cases of double fractures of the tibia with oblique planes of the fractures has been technically substantiated. A method of intraosseous administration of antibiotics and drugs and a method of subcutaneous fasciotomy (a. c. No. 1113098) with a fasciotome of the author's construction has been proposed. The application of a shock-proof clip of our construction (a. c. No. 310659) and slides in the joints (a device with rollers for a hinged plaster cast) contributed to an early recovery of the support function of the injured extremity. The long-term results of the treatment have been studied in 211 patients with double fractures of the bones of the crus and an analysis of rational use of the proposed methods of treatment has been made. To evaluate the results of the treatment the authors have applied clinical, roentgenologic, electrophysiologic, biomechanical and statistical methods of investigation. The results are positive in most cases.

The authors have studied 103 patients with multiple fractures of the long bones of the upper extremities who have been treated at the basic clinic of the Department of orthopaedics and traumatology, at the clinics of the Donetsk Research Institute of Orthopaedics and Traumatology and at the regional traumatologic hospital during the past 15 years. The severity and the diversity of multiple injuries of the upper extremities were evident from the fact that in the 103 patients 220 segments were injured and there were 227 fractures. A classification of the injuries under study and the most rational methods of treatment have been worked out, which ensured positive results in most cases. They are supported by the analysis of the long-term results of the treatment which has been carried out in 61.16% (63) of the patients.

Under study there were 278 patients with 285 subcutaneous ruptures of tendons and muscles of the upper and lower extremities. The tendon of the long head of the humeral biceps muscle and Achilles tendon are most liable to subcutaneous ruptures (123 patients--43.2% and 97 patients--34% respectively). Pathohistological examinations have shown that subcutaneous ruptures of tendons and muscles usually take place against the background of dystrophic alterations reducing their mechanical strength. The authors find it expedient to use polymer materials (kapron, lavsan) for the restoration of injured tendons and muscles.

On the basis of an experience with the treatment of complicated forms of tuberculous spondylitis the authors discuss indications for the operation, methods of preoperative preparing, variants of the operative measures depending on localization of the process, postoperative management. Good remote results were obtained in 27 patients after surgery, satisfactory results in 29 patients.
