RESUMEN

Eighty one patients with inoperable non-small cell lung carcinoma (NSCLC) were entered in a randomized phase II trial comparing split-dose irradiation alone to combined treatment radiotherapy and polychemotherapy (C.A.P. + V.D.S.). The quality of life and the survival of the patients were studied. We have defined three classes of quality of life responses based on the time elapsed before the performance status index drops. A higher quality of life failure rate was observed in the combined treatment group (p non-significant) but the time elapsed before the Karnofsky index drops is longer in the combined treatment group for the quality of life "no change" subgroup (p = 0.15). Survival and quality adjusted survival are similar in both treatment groups. The same conclusion holds for retrospective stratified treatment groups. The results of the study are presented according to the decision tree theory. We conclude that as far as the quality of life is concerned, polychemotherapy combined with the particular split-dose irradiation schedule used is an effective treatment of inoperable NSCLC. Its efficiency is comparable to, but not better than, the same radiotherapy schedule alone taken as a reference baseline.

RESUMEN

Among 95 consecutive patients with acute nonlymphocytic leukemia (ANLL), 61 were treated with a high-dose chemotherapeutic induction regimen consisting of daunorubicin, vincristine, and cytosine arabinoside (DOA). The complete remission (CR) rate was 66%. Although young patients responded better than older patients, only sex was found to be of prognostic significance for achieving CR (84% CR in men versus 47% in women, P less than 0.005, independent of age). Patients in CR received consolidation, monthly maintenance therapy, and late intensification for a period of 2 years. Median remission duration was 57 weeks and median survival, 93 weeks. Projected CR rate at 5 years was 30%. CR and survival duration were significantly longer in patients 40 to 60 years old when compared with younger and older patients (P = 0.023). They were also longer in male than in female patients (P = 0.018), but this may be due to an effect of age. In the 34 patients who did not receive DOA treatment because of age or poor clinical conditions, there was no survival beyond 57 weeks, except in a 73-year-old woman who reached a spontaneous remission during acute infection. ANLL characteristics at presentation revealed a tendency toward a smaller tumor mass (P less than 0.05) and rarer Auer rods present (P = 0.03) with increasing age. Features and treatment of ANLL in elderly patients are discussed in view of the poor results obtained with conservative management.

A direct positive relation has been established between the height (x) and the distance (y) between the phlebostatic plane and the sino-carotid plane in adults. This distance is equal in women to 0.134x + 8.86 and in men to 0.228x-8.90, expressed in cm.

RESUMEN

The graft into the footpad of Jackson mice of B16 melanoma cells which are only slightly pigmented, induces the simultaneous appearance of achromatic and well-pigmented metastatics. We have analysed histologically both types of metastases from the 8th to the 20th day after the resection of the primary tumor. The percentages of mitoses are higher in the achromatic metastases than in the pigmented ones. Both are infiltrated by lymphocytes and by a small number of macrophages and polymorphonuclears. A statistical analysis demonstrates however that the percentage of lymphocytes is not related to the importance of the metastase pigmentation. The discussion is devoted to the relations between the infiltration by lymphocytes, the immunogenicity and the differentiation of the tumor.

Blood plasma lipids of women about seventy years old who had suffered from cardiovascular diseases (AVC) were studied and compared with the plasma lipids of women of about the same age without cardiac troubles. A great difference appears in the unesterified fatty acids of the phospholipids from cardiac women: we have observed a decreasing level of polyunsaturated fatty acids with a carbon chain of more than 18 carbons, and a decreasing ratio between the total concentration of the two precursors (linoleic acid omega and alpha-linolenic acid omega 3) and the total concentration of their long chain polyunsaturated derived fatty acids. On the contrary, we did not observe any significant variation of the unesterified fatty acid level in the plasma. The triglyceride and cholesterol levels also were not different between the two groups of patients. The hypothesis of a lack of elongation and of desaturation of linoleic and alpha-linolenic acids by competitive inhibition with oleic acid seems improbable, since the plasmatic concentration of oleic acid was not statistically different.

RESUMEN

Rat palatal shelves explanted on gestational day 15 or 16 are cultivated with their medial-edge epithelial surface in contact. Observations of histological sections showed that, before and during the fusion of the processes, mitotic activity is always lower in the epithelium of the expected fusion zone (EFZ) than in the oral epithelium and progressively decreases during the processus of the fusion.

RESUMEN

VO2 max, maximum oxygen uptake, has been measured in 4 normal young men, before and after beta-adrenergic blockade (0.5 to 5 mg Pindolol by mouth). Pindolol induces bradycardia and reduces VO2 max. A statistically significant positive correlation appears between posology of Pindolol and bradycardia, this posology and reduction of VO2 max, and finally between bradycardia and reduction of VO2 max. These correlations indicate that the reduction of VO2 max is best explained by a circulatory limitation of oxygen supply to active muscles.
