RESUMEN

Giardiasis is a common infection, and many of its symptoms are similar to those of Crohn's disease. Despite a long discussion on the role of microbiologic agents in Crohn's disease, giardiasis has never been investigated. We studied giardiasis as assessed by the occurrence of cysts in 86 patients with Crohn's disease, in 82 patients with other gastrointestinal disease, and in 52 patients without gastrointestinal disease. In addition, in 20 patients with Crohn's disease the effects of metronidazole on giardiasis and disease activity were studied. Frequency of giardiasis was 61.6% in patients with Crohn's disease, 31.7% in patients with other gastrointestinal disease, and 5.8% in the control group (p less than 0.01). Stool frequency, disease activity, and humoral signs of inflammation in patients with Crohn's disease showed no relationship to giardiasis. All but two patients treated with metronidazole became free of cysts. Crohn's disease activity index decreased in 14 of 20 patients (p less than 0.05). In conclusion, giardiasis is a common finding in patients with Crohn's disease. Treatment of giardiasis can, in individual cases of Crohn's disease, result in a quick recovery from symptoms of high disease activity.

A total of 4656 serum samples from suspect persons were investigated for Toxocara antibodies over the last 10 years. Using the Ouchterlony technique antibodies were demonstrated in 707 cases, and in 409 cases with the help of the microprecipitation test using secondary larvae. It was shown that the living larva test is highly specific and very sensitive so that a positive reaction can be taken as definite evidence of infection. Clinical symptoms were characterised by high eosinophilia, ophthalmologic changes, lung involvement, central nervous affection and increase of gamma-globulins. A history of contact with dogs or cats was an additional indicator. Treatment of larva migrans syndrome has to date remained unsatisfactory as no effective and well tolerated drug is known.

RESUMEN

The microprecipitation test on second generation larvae of Toxocara canis has proved to be highly specific for the diagnosis of human larva migrans visceralis infection caused by Toxocara species. Out of a total of 2864 samples examined from 1969 to 1976, 248 were found to be positive. Besides a very frequently occurring eosinophilia eye affections, lung changes and involvement of the CNS are prominent clinical symptoms. The investigations show that larva migrans visceralis infections also occur in West Germany, even if more rarely than in other countries. The diagnosis is only seldom made because of the lack of uniformity of the clinical symptomatology.
