RESUMEN

The role of neutrophil leukocytes in the development of the polyorgan failure syndrome during an unfavorable course of destructive pancreatitis is discussed. The examinations of 52 patients demonstrated a high correlation of neutrophil functional activity tests (nitroblue tetrazolium reduction test and leukocyte-platelet aggregation) and disorders of microcirculation and hepatorenal involvement (detected clinically and by laboratory tests). The "critical mass" of leukocytosis has been estimated: at least 18 . 10(9)/liter, at which the risk of polyorgan failure appreciably increases. The authors discuss the advisability of selective cytopheresis in a complex of intensive care for critical states involving endogenous poisoning and leukocytosis.

RESUMEN

A case of successful treatment of severe form of Guillain-Barre polyradiculoneuritis complicated with pneumonia, myocarditis and nephritis in a child aged 12 is described. Complex intensive therapy including discrete plasmapheresis led to the patient's extubation on day 21, complete recovery of arm and leg movements on week 5, and regression in signs of myocarditis, nephritis, and pneumonia. The stay in hospital accounted for 56 days, which is twice as little as compared to the literature data. Six months after treatment the child's condition was satisfactory, no pathology in neurological status was observed.

RESUMEN

The results of management of 156 patients with severe forms of acute peritonitis have been reviewed. The authors have used complex intensive care which included rational application of antibiotics, detoxication techniques, immunomodulation, prolonged epidural anesthesia, correction of homeo- and hemostasis, trigger system inhibitors, etc. Complex intensive care improved considerably the results of management even in elderly and old patients with acute peritonitis: out of 156 patients only 21 patients (13.4%) aged 70 and more died.

RESUMEN

Two variants of anesthesiological management have been used in 135 patients with gastrointestinal tumors. The first variant was intubation anesthesia with N2O+O2, ketamine or sodium hydroxybutyrate combined with classical neuroleptanalgesia technique. Pentamine was used to potentiate protection. This anesthesia technique was used in patients under 50 who had no concomitant diseases. The second variant was used in patients over 50 with concomitant diseases. In these cases combined prolonged epidural anesthesia was accompanied in the intraoperative period with intubation anesthesia with controlled lung ventilation using N2O+O2 and additional administration of promedol and droperidol. The second variant of anesthesia not only ensured a safe intraoperative period but also optimized the management of patients in the postoperative period.

Two ways of calculating the content of individual fatty acids in the foods based on the data of gas liquid and thin-layer chromatography (through average values calculation) or only on those of the first (through addition of standard) are advisable. Most reliable in the last case is addition of trimargarine as a standard. The evaluation of the calculation errors is given. A variation of acidic-alkaline methanolysis of fat and esterification of free fatty acids in a closed system is proposed. By using this procedure the yield of methyl esters of fatty acids forms 90 +/- 7 per cent.

RESUMEN

The completness of the lipids removal from the fish muscles and fish products was investigated by making extraction in a filtering separating funnel (FSF) formerly proposed for determining lipids in oil-bearing seeds and cereals. The new modified procedure was contrasted against the standard one (accepted by the State Standards) used in separating and quantitative determination of the lipids content in fish by using the Socklet apparatus for extraction. The proposed variant of extraction was found to secure a higher yield of the lipids. The pattern of analysis used in determining the lipids content in vegetable products may well be used also in investigating animal products, such as, fish and fish products.
