INTRODUCTION: Pre-pandemic, telepractice was not globally implemented despite its effectiveness. Clinicians reported challenges related to technology, confidence, and inadequate resources. OBJECTIVES: To document global telepractice, identify current obstacles and measure the impact of a possible solution. The timing of this research facilitated tracking telepractice changes during the pandemic. METHODS: Two surveys measured practitioners' experience and attitude towards telepractice. Survey 1 was completed in February-March 2020. Participants then received two specialized lesson kits to trial if desired. Survey 2 was a follow-up after 4-6 weeks. RESULTS: Between surveys, the proportion of participants providing telepractice increased from 47.6% to 91.7%. The lesson kits were trialled by 74.3%. Their use had a positive impact on three of the top five factors affecting the delivery of telepractice: parent coaching, clinician experience and accessing resources. Conclusion: Telepractice was rapidly adopted globally during the pandemic. The specialized resources were helpful in overcoming some of the barriers to delivery.

WHAT IS ALREADY KNOWN?: The benefits of measuring PCT in the Emergency Department (ED) are not yet fully characterised.PCT is widely used in the intensive care setting to guide antimicrobial prescribing. WHAT THIS ADDS?: Measurement of PCT as a routine in the emergency department for all patients treated for possible sepsis identifies a high-risk cohort. KEY IMPROVEMENT IN PATIENT CARE: A PCT measurement of >0.2ug/L in the Emergency Department identifies a patient at increased risk of deterioration and of in-hospital death. BACKGROUND: Early recognition and management of sepsis in the Emergency Department (ED) is a clinical challenge. Our aim was to determine if measuring the biomarker PCT in patients with suspected sepsis enables the identification of patients at increased risk of deterioration or in-hospital death in the ED setting of a district general hospital in the United Kingdom. METHODS: A prospective observational study was conducted on all patients aged 18 and over presenting to ED fulfilling NICE criteria for moderate to high risk of sepsis admitted to hospital. Patients had a PCT test alongside the sepsis six protocol. PCT was measured using Brahms's chemiluminescent micro particle assay (CMIA) for the quantitative determination of PCT in human serum and plasma on the Abbott Alinity I analytical platform. The cost per test was approximately 13 GBP.The analysis was performed on patients having a PCT in ED over a 7-month period, with in-depth scrutiny of an appropriate subgroup. A high level quality improvement (QI) approach was used in the study. RESULTS: A total of 1242 patients were included in the study. Mean/median age was 67.9/72, (range 18-102). 88.7% of deaths occurred in patients over 65 years of age. 42.4% (n=532) had a PCT level in ED of >0.2 ug/L. This identified a high risk group with a 2.4 fold increase in mortality rate (7.7%:18.2% p value <0.001). The median length of stay (LOS) was 5 (IQR 9) and 8 days (IQR 11) in patients with a first PCT of ≤0.2 ug/L versus >0.2 ug/L respectively. CONCLUSION: An immediate PCT on patients presenting to ED with signs of sepsis in a non-specialised acute trust identifies those patients at an increased risk of deterioration and in hospital death.

Since the beginning of Operation Enduring Freedom and Operation Iraqi Freedom, the US military has treated more than 51,000 casualties and sustained more than 6,600 deaths. The past decade of conflict has solidified major advances in the use of blood component therapy and the liberal use of fresh whole blood during damage control resuscitation. This resuscitation strategy, combined with far forward damage control surgery, rapid aeromedical evacuation, and major improvements in critical care air transportation and personal protective equipment has led to a 90% to 92% survival rate in US casualties. We describe 2 cases treated by a Forward Surgical Team serving in Afghanistan during Operation Enduring Freedom in 2014. Both patients suffered severe trauma and required massive blood transfusion and damage control surgery. In describing these 2 cases, we wish to share our experience with damage control, resuscitation in an austere environment, as well as advocate for the critical role of the Certified Registered Nurse Anesthetist in advancing the knowledge and execution of this lifesaving strategy in both military and civilian trauma centers. In addition, we suggest alternatives to the current transfusion strategy, which will mitigate limitations currently encountered.

RATIONALE: Bronchopulmonary dysplasia (BPD) is a frequent cause of morbidity in preterm infants that is characterized by prolonged need for ventilatory support in an intensive care environment. BPD is characterized histopathologically by persistently thick, cellular distal airspace walls. In normally developing lungs, by comparison, remodeling of the immature parenchymal architecture is characterized by thinning of the future alveolar walls, a process predicated on cell loss through apoptosis. OBJECTIVES: We hypothesized that minimizing lung injury, using high-frequency nasal ventilation to provide positive distending pressure with minimal assisted tidal volume displacement, would increase apoptosis and decrease proliferation among mesenchymal cells in the distal airspace walls compared with a conventional mode of support (intermittent mandatory ventilation). METHODS: Accordingly, we compared two groups of preterm lambs: one group managed by high-frequency nasal ventilation and a second group managed by intermittent mandatory ventilation. Each group was maintained for 3 days. MEASUREMENTS AND MAIN RESULTS: Oxygenation and ventilation targets were sustained with lower airway pressures and less supplemental oxygen in the high-frequency nasal ventilation group, in which alveolarization progressed. Thinning of the distal airspace walls was accompanied by more apoptosis, and less proliferation, among mesenchymal cells of the high-frequency nasal ventilation group, based on morphometric, protein abundance, and mRNA expression indices of apoptosis and proliferation. CONCLUSIONS: Our study shows that high-frequency nasal ventilation preserves the balance between mesenchymal cell apoptosis and proliferation in the distal airspace walls, such that alveolarization progresses.
