RESUMEN

Genital metastases are very rarely indicative of breast cancer; they are exceptionally located at the cervix. These atypical locations are more common when it comes to a metastatic breast cancer or a histological infiltrating lobular type. The simultaneous association of a lobular and a ductal infiltrating cancer under a synchronous bilateral breast cancer still remains a rare entity. In this work, we report the observation of a woman aged 48 who has a synchronous bilateral breast cancer, of different histological types, and who reported at first a genital bleeding which is caused by a metastasis in the cervix of the uterus.

RESUMEN

Tuberculosis of the breast is a very rare infection, it occurs chiefly in women of childbearing potential, usually as an apparently primary infection and constitute a diagnosis and therapeutic challenge. Administration of antituberculous agents is the mainstay of therapy. Surgery is required in some cases. We report two cases of breast tuberculosis. The diagnosis was put on histology with good outcome under anti bacillary treatment. Through the literature data we recall the epidemiological, clinical, diagnostic and treatment of this pathology.

RESUMEN

Takayashu arteritis is a chronic inflammatory disease of the large arteries, usually affecting the aorta and its large branches and the pulmonary arteries, with a higher incidence during the childbearing years. We report the case of a 33-year-old patient, primigravida with Takayashu arteritis diagnosed three years ago. At 37 weeks of gestation, she was admitted for a pre-eclampsia and a left ventricular insufficiency. Elective caesarean section under general anesthesia after joint decision between the attending obstetrician and the medical and anesthetic consultants, and allowed the extraction of a hypotrophic baby. The association of pregnancy with Takayashu's arteritis is almost always uneventful. It is associated with high values of maternal blood pressure and severe intra-uterine growth retardation.

Sickle cell disease is a major cause of maternal and neonatal mortality in tropical regions. Treatment consists of regular blood transfusion during pregnancy and delivery. Transfusion may be performed selectively (in function of the clinical status of the mother, fetus, and pregnancy) or prophylactically. Both management strategies result in a significant improvement in the obstetric outcome. However systematic prophylactic transfusion has been shunned due to the risk of transmitting infections and allo-immunization. The16-case series reported here demonstrates a real benefit for prophylactic blood transfusion during pregnancy complicated by sickle cell disease in tropical zones. Improvements in our transfusion curves resulting from better donor selection, follow-up, and phenotyping support arguments previously made over one decade ago in favor of the prophylactic approach especially in countries with high transfusional risks. In our experience the risks associated with more transfusions was offset by the greater safety margin and better efficacy of the prophylactic approach.

RESUMEN

La localisation cerebrale des thromboses veineuses est rare au cours de la grossesse et le post partum. Nous rapportons le cas d'une patiente de 33 ans 3eme geste 3eme pare; cesarisee a 34 semaine d'amenorrhee pour placenta praevia hemorragique; les suites post operatoires sont marquees au 6eme jour par des cephalees suivies de crises convulsives et un deficit moteur de l'hemicorps droit. Le diagnostic de thrombose veineuse cerebrale est confirme par l'angio-IRM cerebrale. Le bilan etiologique restait negatif. L'evolution etait favorable sous anticoagulant. Les auteurs mettront l'accent sur les aspects physiopathologiques; cliniques; paracliniques; therapeutiques et evolutifs de la TVC du post partum
