RESUMEN

A study was undertaken to evaluate the effectiveness of the MS-2 (Abbott Laboratories, Dallas, Tex.) in screening urine specimens in a large clinical laboratory. A total of 15,319 urine specimens (9,954 midstream specimens and 5,365 catheter specimens) were evaluated with the MS-2 and by a surface streak procedure. The study was conducted in two phases, differing in that phase II urine specimens were evaluated in the MS-2 by using a program software update (03.01). For midstream urine specimens, MS-2 detection rates in phases I and II were, respectively, 94.5 and 94.3% at a plate count of greater than 10(5) CFU/ml, 74.4 and 65.3% at plate counts of 5 X 10(4) to 1 X 10(5) CFU/ml, 55.0 and 52.4% at plate counts of 1 X 10(4) to 5 X 10(4) CFU/ml, 31.2 and 20.5% at plate counts of 10(3) to 10(4) CFU/ml, and 15.7 and 6.4% at plate counts of less than 10(3) CFU/ml. For catheter urine specimens, the MS-2 detection rates in phases I and II were, respectively, 95.4 and 96.8% at plate counts greater than 10(5) CFU/ml, 74.4 and 85.7% at plate counts of 5 X 10(4) to 1 X 10(5) CFU/ml, 50.0 and 44.4% at plate counts 10(4) to 5 X 10(4) CFU/ml, 25.6 and 14.9% at plate counts of 10(3) to 10(4) CFU/ml, and 14.7 and 5.2% at plate counts less than 10(3) CFU/ml.

RESUMEN

One of the complications of the mucocutaneous lymph node syndrome is acute hydrops of the gallbladder. Although surgery may be required occasionally, spontaneous resolution of gallbladder hydrops in patients is common. Ultrasound is the optimal method for evaluating these patients.

RESUMEN

CT staging was performed in 49 consecutive patients with known carcinoma of the bladder. The overall accuracy of CT staging in 25 patients with surgically confirmed disease stage was 64%. Most diagnostic errors in this series were related to the determination of perivesical fat involvement by tumor. The overall accuracy of CT in predicting lymph node metastasis was 92%. The sensitivity was 60% and the specificity ws 100%. Because of the current management of bladder carcinoma, the major role of CT is in the evaluation of lymph nodes for metastatic disease. Since CT cannot detect metastasis to nonenlarged lymph nodes, it has only a limited role in the staging of bladder carcinoma at this time.

RESUMEN

The results of a prospective, randomized comparative study of the efficacy and toxicity of clindamycin, chloramphenicol, and ticarcillin in the treatment, concomitantly with gentamicin to ensure complete aerobic coverage, of 175 patients with serious mixed aerobic/anaerobic intraabdominal or female genital tract sepsis are reported. In the group with intraabdominal sepsis, 33 (79%) of 42 treated with clindamycin, 43 (81%) of 53 treated with chloramphenicol, and 35 (90%) of 39 treated with ticarcillin were cured. In the group with genital tract sepsis, 16 (94%) of 17 treated with clindamycin, 11 (100%) of 11 treated with chloramphenicol, and 12 (92%) of 13 treated with ticarcillin were cured. Diarrhea occurred most frequently in patients treated with clindamycin (P < 0.001), hematologic suppression occurred most frequently in patients treated with chloramphenicol (P < 0.01), and hypokalemia occurred most frequently in patients treated with ticarcillin (P < 0.01). Clindamycin, chloramphenicol, and ticarcillin, each in combination with gentamicin, are equally effective in therapy for intraabdominal or female genital tract sepsis.
