RESUMEN

The results of complex treatment of 37 patients operated on for the various acute abdominal surgical diseases were presented. Among them 22 patients for locally treated 0.02% antiseptical solution Decametoxin were used. Positively impact clinical course of the diseases reducing their purulent and septic morbidity rate in particular were evidenced separately in the patients operated on for acute necrotic pancreatitis.

RESUMEN

The most important teoretical aspects of the enteral insufficiency syndrome formation in an acute abdominal surgical diseases were suggested. Basing on the treatment results analysis of 19 patients with different abdominal surgical diseases, using hypertonic sorbitol solution, the authors confirm high therapeutic efficacy of the preparation, administration of which have had promoted the intestinal motor function restoration and optimization, as well as adapting action of the preparation in enteral probe nutrition conduction in patients during ealy postoperative period.

RESUMEN

The data, obtained as a result of the protective systems state (immunity, hemostasis, organism natural resistance) investigation in 20 patients with complicated course of necrotic pancreatitis, are adduced. There was determined, that the lowering of the oxygen dependent and oxygen independent functions of neutrophils, the lysosomic cationic proteins content correlates significantly with course of diseases severity. High concentration in the blood serum of anti-inflammatory cytokines (the leucocytes depression phenomenon) in combination with the oxygen dependent and oxygen independent function of phagocytes depression constitutes the criterion of complicated and unfavourable course of necrotic pancreatitis. Theoretical substantiation of the results is presented.

RESUMEN

While application of dynamical omentopancreatopexy for the treatment of necrotic pancreatitis there were noted the purulent and visceral complications frequency lowering from 3.34 in every patient--while the closed interventions usage down to 2.02 and of mortality--from 57.2 down to 33% accordingly.

A test group of 40 patients and a control group of 20 matched controls with pulmonary cancer and suppuration were exposed to preoperative and postoperative electromyostimulation (EMS) with adjuvant intravascular laser radiation of the blood (ILRB) to prevent thromboembolism versus conventional chemotherapy, respectively. At the preoperative examination all the patients exhibited secondary immunodeficiency. It was found that introduction of EMS and ILRB corrects immunity and activates fibrinolysis. A positive trend in coagulation is also attributed to immunomodulating effects of EMS and ILRB.

RESUMEN

The results of treatment of 176 patients with sepsis at the department of urgent surgery in 1985-1990 are discussed. The authors suggest a stage complex program of acute surgical sepsis prevention, which consists of purposeful measures applied in the pre-, intra-, and postoperative period and includes the following recommendations: conduction of skin tests, preoperative injection of antibiotics, preoperative immunization of patients, immunological methods for the choice of antibiotics, methods for rational immunologic correction, phagotherapy. Besides, the authors developed and introduced into practice an individual method for prognosticating the course and outcome of acute surgical sepsis based on E. V. Gubler's (1978) mathematical method. It is concluded that the suggested methods are very effective, with their application the sepsis mortality rate reduced from 75% to 37.5%, i.e. by half.
