RESUMEN

Miller's Class III gingival recessions (GRs) have always posed a challenge to the clinicians in terms of achieving complete root coverage (CRC). In the present case, a cell-based therapy with autologous fibroblasts seeded onto a Type 1 collagen membrane, through an in-vitro culturing method was utilized. The fibroblasts-seeded membrane was surgically placed under a laterally repositioned flap. The patient presented with a CRC, which was stable even at the postoperative period of 45 months. In addition, a 3-mm substantial gain in the width of keratinized tissue was achieved and maintained throughout the postoperative period. Hence, the results of the cell-seeded therapy emphasize that it can serve as an effective alternative method for the management of Miller's Class III GRs.

Dengue is a public health problem with an increasing global incidence and geographic distribution in almost all tropical and subtropical countries, with a transition from epidemic to endemic occurrence. In this study, we report a six-year analysis (2009-2014) performed at the Department of Virology, King Institute of Preventive Medicine, Chennai, Tamil Nadu, India. Our data confirm earlier findings that dengue is highly endemic in Chennai. In the present study, 10,099 serum samples from suspected dengue cases were tested for IgM ELISA (NIV Capture) and IgG Panbio ELISA (Australia). Of these suspected cases 6,798 and 3,301 were pediatric and adult cases, respectively, and 1,927 (19.08 %) were confirmed serologically as dengue. Of these, 1,752 (25.7 %) and 175 (5.3 %) were pediatric and adult cases, respectively. The aim of this study was to highlight the occurrence of DHF and DSS, mainly among the pediatric population, in which the infection causes higher mortality and morbidity. The overall positivity was higher in the pediatric group than in the adults. Detection of both IgM and IgG positivity will be useful for monitoring infection rates, the disease spectrum, and the prevalence of the different serotypes, which will give us insight about the circulating serotypes and pathogenicity. These data will be valuable for providing an early warning to predict an impending epidemic leading to major clinical manifestations of DHF and DSS.

BACKGROUND & OBJECTIVES: Dengue is one of the most important Arboviral diseases in man with outbreaks in Southeast Asia and India. We report a retrospective analysis of the dengue positivity in the referred samples for three years period (2006 to 2008) at the Department of Virology, King Institute of Preventive Medicine, Chennai, Tamil Nadu, India. METHODS: Serum samples from 1593 suspected cases (968 male and 625 female) were obtained. Of the 1593 cases screened, 1204 (75.5%) were paediatric cases and 389 (24.4%) adults. The samples were subjected to MAC ELISA and IgG ELISA. RESULTS: Of the 968 patients, 686 (43.0%) were positive, of which 579 (84.0%) were in the paediatric age group (<14 yr) and 107 (15.5%) were adults. The IgM positivity being 356 (36.7%) in males and 330 (52.8%) in females. Of the 686 positives, 113 (16.47%) were positive for both IgM and IgG denoting secondary infection. There was a noticeable increased occurrence during the cooler months and during the monsoon and post-monsoon months. INTERPRETATION & CONCLUSION: The dengue IgM seropositivity among the suspected cases indicates active dengue virus activity. Increase in the probable secondary infections especially in a country like ours where multiple serotypes are prevalent raises concern over probable increase in the incidence of the more serious DHF/DSS. Studies need to be done to identify circulating serotypes of dengue virus to design preventive strategies.

RESUMEN

Platelet-associated Ig (PAIg) is raised in patients with idiopathic thrombocytopenic purpura. However, it is also raised in conditions not traditionally considered to be immune mediated. The latter finding suggests that raised PAIg may be an epiphenomenon of thrombocytopenia. The assays currently available do not distinguish between PAIg present due to a specific Fab binding or present due to non-specific binding of Ig via Fc or complement receptors. In this paper we show that by prior incubation with normal rabbit serum, an assay for PAIg can be made specific for Fab binding.

Various techniques have been described for quantitating IgG or complement (C3) on red cells (RBCs). The techniques either are cumbersome, as the complement consumption test, or use radioactivity. This paper describes an antiglobulin consumption assay using an enzyme-linked immunosorbent method that can be used to quantitate IgG, IgM, and C3. With this technique RBCs from normal, healthy donors gave a mean value of 106 +/- 60 molecules of IgG per RBC, 4.5 +/- 3 molecules of IgM per RBC, and 37 +/- 28 molecules of C3 per RBC, respectively. The RBCs of hospital patients, particularly of those with infections or inflammatory conditions, contain increased amounts of nonspecifically bound immunoproteins. The availability of a common method to quantitate RBC-associated IgG, IgM, and C3 allows easy monitoring or study of the immune mechanism of autoimmune hemolytic anemia.

ABO incompatibility of the newborn is one instance where immune hemolysis may present with a negative direct antiglobulin test (DAT) and therefore a simple sensitive test for detecting sensitization would be useful in this clinical situation. To evaluate the usefulness of ELAT in detecting in-vivo sensitized red cells, 1608 maternal-baby pairs were screened for ABO incompatibility over a period of 10 mth. Of 251 ABO-incompatible babies, there were 49 (19.5%) with positive DAT, but an additional 67 (26.7%) were ELAT positive. These were eluate positive as well, indicating that the increased number with sensitized cells as shown by ELAT is due to detection of in-vivo sensitized cells. The positive predictive value for ABO hemolytic disease of the newborn (HDN) is 48%, which is two times that of DAT. Calculating the difference of the absorbance from baseline (delta OD) may give an indication of degree of sensitization which together with the maternal antibody titre would aid us in the estimation of antigen dosage on the baby's red cells and in the appraisal of the role of antigen dosage in HDN.

The population in Singapore is predominantly Asian, with Chinese forming the major ethnic group. The incidence of haemolytic disease of the newborn (HDN) due to Rh incompatibility is very low. The true incidence of HDN due to ABO incompatibility is unknown. Early discharge is practised in Singapore making it important to predict severe HDN due to ABO incompatibility as this would constitute the main cause of haemolysis next to G6PD deficiency. One thousand, six hundred and eight baby-maternal pairs were typed for ABO, Rh, and tested for direct Coombs' test, maternal titre, cord bilirubin and haptoglobin levels. Two hundred and fifty-one were found to be ABO incompatible, with 141 group A and 110 group B babies. The incidence of HDN due to ABO incompatibility was 3.7% of all group O mothers. Coombs' test, maternal antibody titre, cord bilirubin and haptoglobin levels were of low predictive value for severe HDN due to ABO incompatibility. The data further support the notion that it is not cost effective to screen for ABO incompatibility.
